The Minnesota PS2828 form is the professional verification you submit to qualify for a reduced-fee state ID card that costs $0.50 instead of the standard price. You fill in your personal information at the top, a licensed medical professional or case manager signs the verification section confirming you meet one of three qualifying conditions, and you bring the completed form to a Driver and Vehicle Services (DVS) office with your identity documents. DVS staff cannot verify your eligibility on their own, so the form has to be signed before you walk in.
Who Can Use Form PS2828

- Developmental disability as defined in Minnesota Statutes section 252A.02, subdivision 2.
- Serious and persistent mental illness as defined in Minnesota Statutes section 245.462, subdivision 20, paragraph (c). Fresh verification is required at your initial application and again at every renewal.
- Physical disability as defined in Minnesota Statutes section 169.345, subdivision 2.

Filling In Your Section
Write your full legal name and date of birth exactly as they appear on the identity documents you plan to bring. Any mismatch between the name on PS2828 and the name on your birth certificate, passport, or other primary document will cause the transaction to stop at the counter. Double-check spellings, hyphens, and middle names before you hand the form to your provider.

Getting the Professional Verification Section Signed
This is the part that controls whether you get the reduced fee. A licensed medical professional or a case manager has to sign the form and attest that you have a qualifying condition.2Minnesota Department of Public Safety. An incomplete verification section will be rejected.
Timing matters. Get the form signed as close to your planned DVS visit as you can. What To Bring With the Form
PS2828 only proves your eligibility for the reduced fee. You still have to meet Minnesota’s standard identity requirements to get any ID card. For a standard (non-REAL ID) card, bring two primary documents, or one primary and one secondary document.
Primary documents include an unexpired U.S. or foreign passport, a certified birth certificate from any U.S. state, territory, or foreign jurisdiction, a Certificate of Birth Abroad (FS-545 or DS-1350) or Consular Report of Birth Abroad (FS-240), a certified adoption certificate showing your full name and date of birth, an unexpired tribal identification card with security features, a Certificate of Naturalization or Certificate of Citizenship (N-550, N-570, N-560, N-561, N-578, or N-645), an unexpired Permanent Resident Card (Form I-551), or an unexpired U.S. military ID. Secondary options include a Social Security card, a driver’s license or ID card from another state (unexpired or expired five years or less), a certified marriage certificate, a certified court order containing your name and date of birth, or school transcripts certified by the issuing institution.4Minnesota Department of Public Safety. If you want a REAL ID-compliant card, add proof of your Social Security number, proof of lawful status, and two documents proving your Minnesota address. Accepted residency documents include a current insurance card, your most recent federal or state tax return, a Minnesota property tax statement, a home utility bill no more than 12 months old, a residential lease with signatures, or a bank statement no more than 12 months old with account numbers redacted. Submitting the Form and Paying the Fee
Take the completed PS2828, your identity documents, and payment to a DVS exam station or a deputy registrar office that handles ID card transactions. The reduced fee is $0.50, paid at the counter when you submit the paperwork.
If you do not have a stable mailing address, ask your provider or caseworker whether the card can be sent to a social service agency, shelter, or their office. Arrange this before you apply so the address on file is somewhere mail actually reaches you.
Renewing With PS2828
When your reduced-fee card expires, you repeat the process with a current PS2828 carrying a fresh professional signature.
