How to Complete the NY DMV Certification of Residence (MV-44NYR)

The MV-44NYR form is a one-page certification that someone who knows you — a parent, landlord, neighbor, employer, spouse, co-resident, or school official — signs to confirm to the New York DMV that you live at a given address. You bring it to any DMV office as one of your residency proofs when applying for a learner permit, driver license, or non-driver ID. One limitation is printed on the form itself: it cannot be used for a standard credential. It only satisfies residency for a REAL ID or Enhanced credential.1New York State Department of Motor Vehicles. MV-44NYR Certification of Residence

Which Credentials It Works For

The form states plainly that it “is not acceptable for proof of residency for a standard credential.”1New York State Department of Motor Vehicles. MV-44NYR Certification of Residence In New York, “standard” means the basic license, permit, or non-driver ID — the credential type available under the Green Light Law. If that is what you are applying for, prove residency through documents on the ID-44 acceptable list instead.

For REAL ID and Enhanced applications, the DMV requires two proofs of residency.2New York State Department of Motor Vehicles. The second must be in your own name: a bank statement, pay stub, piece of postmarked mail, or utility bill dated within the past year all appear on the ID-44 list.

Who Can Sign the Form

The certifier does not have to be a relative. The form lists seven eligible relationships:1New York State Department of Motor Vehicles. MV-44NYR Certification of Residence

  • Parent or legal guardian
  • Spouse
  • Co-resident (a roommate or anyone sharing the address)
  • Landlord
  • Employer
  • Neighbor
  • School official

Whoever signs is making a legal statement that you live at the address written on the form. Only ask someone who can confirm your residence firsthand.

What Else to Bring to the DMV

The MV-44NYR covers residency only. You still need to meet the full six-point identity requirement on the ID-44 worksheet, using documents like a passport, birth certificate, and Social Security card.2New York State Department of Motor Vehicles. ID-44 How to Apply for a New York Learner Permit, Driver License, or Non-Driver ID The certification itself carries zero identity points.

Have these ready before your visit:

  • Your full legal name, spelled exactly as it appears on your identity documents.
  • Your residential street address — the physical location where you live, not a P.O. Box.
  • A second proof of residency in your own name, dated within the past year, from the ID-44 list.
  • Enough other identity documents to reach six points on the ID-44 worksheet.

Because the form includes a DMV employee signature line, call your local office if there is any question about whether the certifier needs to appear in person or whether a completed, pre-signed form is enough.

Completing the Form

Download the MV-44NYR from the NY DMV website or pick up a copy at any DMV office. The current revision is dated 2/26 and fits on a single page. There are no separate parts; both the certifier and the applicant complete their sections on the same sheet.1New York State Department of Motor Vehicles. MV-44NYR Certification of Residence

The certifier fills in most of the fields. They print their full name, mark their relationship to you from the seven categories, write your full name and residential address, and read the certification statement. That statement confirms you currently live at the stated address, that the name given is your full name, and that the certifier understands false statements are a criminal offense. The certifier then signs and prints their name.

You print and sign your name in the applicant fields. A DMV employee adds a third signature when accepting the form at the counter. Make sure every name is legible and matches the spelling on your supporting ID. A mismatch is the fastest way to get sent home for a second trip.

Submitting It

Bring the signed MV-44NYR with your other required documents to your DMV visit. The DMV strongly encourages a reservation; some offices with long waits may only admit visitors who have one.3New York State Department of Motor Vehicles. DMV Office Locations You can book through the DMV website by selecting your local office.

At the counter the clerk checks the form for completeness, confirms the certified address matches your application, verifies your second residency proof, and totals your identity documents against the six-point requirement. Once accepted, the residency piece is cleared and your application moves to the next step, such as a vision screening or scheduling a road test.

Penalties for a False Statement

The bottom of the form warns in bold that making a false statement is punishable by law. Under New York Penal Law 210.45, knowingly making a false statement in a written instrument that carries that warning is a Class A misdemeanor.4New York State Senate. New York Penal Code 70.15 – Sentences of Imprisonment for Misdemeanors and Violation and the maximum fine is $1,000.6New York State Senate. New York Consolidated Laws Penal Law PEN 80.05

That risk falls on the person who signs as certifier, since they are the one attesting to the facts. An applicant who conspires with a certifier to submit false information could face separate charges. Only ask someone to sign if you genuinely live at the address, and only sign for someone whose residence you can confirm.
