How to Complete the Oklahoma Motor Vehicle Bill of Sale Form 722-1

Oklahoma Form 722-1, the Declaration of Vehicle Purchase Price, is the sworn statement a buyer uses to report what they paid for a vehicle when no written bill of sale exists. It does not transfer ownership on its own; it tells Service Oklahoma what figure to use when calculating excise tax, and you submit it alongside the properly assigned certificate of title.1Service Oklahoma. When Form 722-1 Is the Right Document

The form is for private sales where the parties never wrote up a contract or receipt and the buyer cannot get one from the seller after the fact. By signing it, you certify that no contract or bill of sale exists and that you are unable to obtain one.1Service Oklahoma. Declaration of Vehicle Purchase Price

If you already have a written purchase agreement or a dealer invoice showing the price, use that instead. Service Oklahoma lists a purchase agreement or Form 722-1 as the price document for an out-of-state title transfer, so either one works.2Service Oklahoma. Out-of-State Title Transfers

Two situations call for a different form entirely. If the vehicle has an electronic title with no physical certificate, Oklahoma uses Form 718, the Bill of Sale of an Electronic Title, which also must be notarized.3Service Oklahoma. And one hard limit worth stating plainly: Form 722-1 cannot stand in for a missing title. If the seller has lost the certificate, they must request a duplicate from Service Oklahoma for $11 and sign that over to you.3Service Oklahoma. How to Fill Out the Form

Download Form 722-1 from the Service Oklahoma website or pick up a copy at any Licensed Operator location (formerly called Tag Agencies).3Service Oklahoma. Vehicle Titles It is one page, but every field matters because you are signing it as a sworn statement.

The vehicle section asks for three things: the model year as stamped on the title, the make (Ford, Toyota, Chevrolet, and so on), and the Vehicle Identification Number. Copy the VIN character by character from the title or from the metal plate visible through the lower-left corner of the windshield. A single transposed digit can cause the transfer to be rejected. The form does not include a field for the vehicle’s model; the VIN alone identifies the exact vehicle in state databases.1Service Oklahoma. Declaration of Vehicle Purchase Price

Next come the transaction details. Write the date you and the seller completed the deal, the full name of the person or entity you bought from, and the total purchase price in dollars, excluding any trade-in credit. Report the actual price honestly. That figure determines your excise tax, and misrepresenting it on a sworn, notarized document carries the risk of fraud charges.

Finally, fill in your own information as the affiant, meaning the person swearing the statement is true: printed name, the date you are signing, a daytime phone number, and an email address. Do not sign yet. You will sign in front of a notary.

Getting It Notarized

Form 722-1 is a sworn affidavit, so the notary needs to witness your signature and verify your identity. Do not sign at home and bring it in already signed.1Service Oklahoma. Declaration of Vehicle Purchase Price Only the buyer signs the form.

The title assignment also requires notarization. When the seller endorses the back of the certificate of title, that signature must be sworn before a notary or another person authorized to take acknowledgments.7New York Codes, Rules and Regulations. Oklahoma Code 1107 – Sale or Transfer of Ownership of Vehicle – Violations – Penalties Many buyers and sellers handle both notarizations in one sitting: sign the title, sign the 722-1, and have the notary stamp everything at once.

Oklahoma caps notary fees at $5 per notarial act.8Justia. Oklahoma Code 49-5 – Notarial Seal – Penalties – Fees Banks, UPS stores, and some Licensed Operator offices offer notary services. Two notarizations should run no more than $10.

What to Bring and What You’ll Pay

Take everything to a Service Oklahoma office or a Licensed Operator location. For an in-state title transfer, bring:

  • The certificate of title, assigned and notarized by the seller, with any lien releases attached.
  • Your notarized Form 722-1 (or a written purchase agreement or dealer invoice showing the price).
  • Proof of Oklahoma liability insurance on the vehicle.

The 30-Day Deadline

Oklahoma law gives the buyer 30 days from the date the seller assigns the title to present it for transfer and pay the $11 fee and applicable taxes.7New York Codes, Rules and Regulations.
