For a North Carolina CNA renewal, the employment verification form is an online form a Registered Nurse submits through the Division of Health Service Regulation (DHSR) confirming you performed at least eight hours of paid, RN-supervised nurse aide work during your current 24-month listing period. You do not fill it out yourself. It has to be submitted alongside your own online renewal form, and both must reach DHSR before your listing’s expiration date. There is no fee.1North Carolina Department of Health and Human Services. Nurse Aide Renewal Form
Who Submits the Form
A Registered Nurse completes and submits the employment verification through a separate online portal from your renewal form.2N.C. Department of Health and Human Services. Forms and Applications After you submit your own renewal form, you notify an RN to go to the Online Employment Verification Form and submit it on your behalf.
The RN does not have to be your direct supervisor. Any RN with access to employment records who can verify that you meet the renewal requirements may complete it. What the RN cannot do is verify work they have no written documentation or facility records to support. An RN who personally supervised you on a given date can verify based on firsthand knowledge, but guesswork or secondhand accounts do not count.1North Carolina Department of Health and Human Services. The RN is not evaluating your competency. They are confirming employment facts.
What Work Qualifies
Federal regulations require states to remove nurse aides from the registry if they perform no nursing or nursing-related services for 24 consecutive months, and North Carolina enforces that rule through this verification process.3eCFR. 42 CFR 483.156 — Registry of Nurse Aides The work the RN is attesting to must meet all four criteria:4North Carolina Department of Health and Human Services. - It was paid.
- The tasks fall within the NC Board of Nursing Nurse Aide I task list.
- A Registered Nurse delegated and supervised the duties.
- You worked at least eight hours total during your current listing period.
The work can happen in a nursing home or another health care setting. If you worked outside a nursing home, the responsibility falls on you to confirm your duties actually qualify and that an RN is in a position to verify them.4North Carolina Department of Health and Human Services. Private duty employment does not qualify. If a licensed health care facility or agency hired you to provide nursing services in someone’s home, though, that is not considered private duty. For nurse aides working in Medicaid Home- and Community-Based Services, documentation of participation in the Community Alternatives Program for Disabled Adults (CAP/DA) or the Community Alternatives Program for Children (CAP/C) is required.5North Carolina Department of Health and Human Services. What the RN Enters
On the online form the RN enters your name, the last four digits of your Social Security Number, and your listing number, and confirms that you performed at least eight hours of paid nursing or nursing-related services under RN supervision during your current listing period.5North Carolina Department of Health and Human Services. If You Cannot Get an RN to Verify in Time
If you are still listed as active and in good standing but cannot meet the employment verification requirement, you can apply for a Training Waiver under eligibility category E10. This lets you take the state-approved competency examination without repeating a full training program. You can apply no more than 90 days before your listing expiration date, and you must pass both the written and skills examinations before the listing expires.9N.C. Department of Health and Human Services. Eligibility and Training Waiver Requirements The application is handled through Credentia’s CNA365 platform.
If the listing lapses first, this option is gone. Once expired, the path back is retraining through a state-approved Nurse Aide I program and passing the competency examination again.6N.C.
