How to File for a Lost Title in Oregon: Form, Fees, and Timeline

To file for a lost title in Oregon, complete the Application for Replacement Title (Form 735-515), pay the title fee (currently $101 to $192 depending on the vehicle), and submit the packet to the Oregon DMV by mail or in person. Processing runs about a week once your paperwork arrives.¹²

The Application for Replacement Title is a fillable PDF on the Oregon DMV website. Fill it in carefully. Knowingly making a false statement on a title application is a Class A misdemeanor in Oregon, punishable by up to one year in jail, a fine of up to $6,250, or both.³

You’ll need the Vehicle Identification Number (VIN), which appears on the driver’s side of the dashboard near the windshield or on a sticker in the driver’s door jamb, plus your current Oregon license plate number. Every registered owner has to be listed with full legal name, date of birth, and home address exactly as they appear in DMV records.³

Odometer Reading

Only some vehicles need an odometer disclosure. Federal rules require it for vehicles that are model year 2011 or newer, less than 20 years old, and rated at 16,000 pounds gross vehicle weight or below.⁴ Older cars and heavy trucks are exempt. The form’s instructions spell out which vehicles need the disclosure.³

If There’s a Lien on the Vehicle

Still paying off the vehicle? List the lienholder’s name and address on the application. The replacement title will be issued with the lien still recorded on it.³

If the loan is paid off and you want the lien removed from the new title, submit an original lien release from the lender with your application and check the “Replacement Oregon Title” box in the Certifications section.¹ Get the release letter from your lender before you file, or you’ll end up doing this twice.

What It Costs

Oregon doesn’t charge a separate duplicate-title fee. You pay the standard title fee, which depends on the vehicle’s type and fuel economy.²

For passenger vehicles and trucks rated at 26,000 pounds GVWR or less:

0–19 MPG combined: $101
20–39 MPG combined: $106
40+ MPG combined: $116
All-electric: $192

Motorcycles, mopeds, travel trailers, motor homes, ATVs, and similar vehicles pay a flat $101. Heavy vehicles over 26,000 pounds GVWR pay $90.² Some fees changed effective December 31, 2025, so confirm the current amount on the DMV fee page before mailing a check.

Update Your Address First

The replacement title is mailed to the primary owner’s address on file, no matter how you submit the application.¹ If you’ve moved, fix the address before you apply. Oregon law requires you to notify the DMV within 30 days of an address change; you can do it online or by calling 503-945-5000.⁵ A title sent to an old address can end up in the wrong hands.

You can file by mail or at any Oregon DMV office. In person, a clerk can flag a missing document on the spot.

By Mail

Send the completed Form 735-515, any lien release, and a check or money order payable to “Oregon DMV” to:

Mail applicants pay by check or money order only. If anything is missing or unsigned, the packet gets returned and you start over.

In Person

Bring the completed form, photo ID, any lien release, and payment. Offices accept cash, checks, money orders, debit cards, Visa, Mastercard, Discover, and mobile payments.⁷ Checks and money orders have to be for the exact amount.

How Long It Takes

The DMV’s posted processing time for replacement titles is about a week.¹ Add mail time on each end if you file by mail. Titles are printed centrally, not at branch offices, so even in-person filers receive the title in the mail.

Oregon’s expedited title service, which costs an extra $100, is only available to licensed Oregon vehicle dealers. Individual owners cannot use it.⁸ If your title hasn’t arrived after a reasonable wait, call 503-945-5000 to check the status.⁶

Replacing a Title When the Owner Has Died

When the owner has died, the paperwork you add to the replacement application depends on whether the title carried survivorship and whether the estate went through probate.¹

Title has survivorship (Y/Y or Y/N in the survivorship box): the surviving owner submits proof of death, such as a death certificate or a court document indicating death. If all owners are deceased, include proof of death for each and estate documents for the most recently deceased owner.
No survivorship, estate not probated: each heir signs a notarized Inheritance Affidavit. If a simple estate affidavit was filed with the county, include a copy of that court-filed affidavit or a Simple Estate Certification.
No survivorship, estate being probated: include a copy of the court-issued Letters Testamentary or Letters of Administration, plus an original release or bill of sale from the personal representative.
No survivorship, estate closed: include a copy of the court-issued Decree of Final Distribution showing who received the vehicle, plus original releases from any previous owners other than the deceased.

Survivorship status appears in a box on the original title. Since that title is what you’re trying to replace, a DMV employee can look up the survivorship status for you at any office.¹
