To file for disability in Georgia, you submit a Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) application to the federal Social Security Administration — online at ssa.gov, by phone at 1-800-772-1213, or at a local field office — and Georgia’s Disability Adjudication Services then reviews your medical evidence and issues a decision.1Georgia Vocational Rehabilitation Agency. Social Security Services The process is the same one used nationwide, but knowing which program to file under, what to gather first, and which forms carry the weight of your claim will save you months.
Decide Whether You’re Filing for SSDI, SSI, or Both
SSDI and SSI use the same medical standard — your condition must significantly limit your ability to do basic work activities for at least 12 consecutive months, or be expected to result in death.2Social Security Administration. Disability Benefits – How Does Someone Become Eligible What separates them is everything else.
SSDI is for workers who have paid enough Social Security taxes to be “insured.” Most adults need 40 work credits, with 20 of those earned in the 10 years before the disability began.2Social Security Administration. Disability Benefits – How Does Someone Become Eligible Younger workers can qualify with fewer. If you’re not sure where you stand, create a “my Social Security” account at ssa.gov and pull your earnings statement.
SSI is needs-based. Work history doesn’t matter, but your countable resources must stay below $2,000 as an individual or $3,000 as a couple, and things like bank accounts and vehicles count.3Social Security Administration. Who Can Get SSI
Either way, if you’re currently earning wages, they generally can’t exceed the substantial gainful activity threshold, which is $1,690 per month in 2026 ($2,830 if you are blind).4Social Security Administration. Who Can Get Disability You can file for both programs at once if you might qualify for each.
Gather Your Documents Before You Start
Incomplete applications get delayed, so pull everything together first. You need two categories of records.
Personal and Financial Records
Have your Social Security number, a certified birth certificate or other proof of U.S. citizenship, and current contact information. If you’re applying for SSI, add detailed financial records: bank statements, property records, and documentation of any household income.3Social Security Administration. Who Can Get SSI
Medical Evidence
Your medical evidence is the single most important part of your file. Gather:
- Names, addresses, phone numbers, and patient ID numbers for every hospital, clinic, and doctor who has treated your condition.
- Dates of hospital stays, surgeries, diagnostic tests, and ongoing treatments.
- A full list of every prescription and over-the-counter medication, including dosages and the prescribing doctor.
- Copies of imaging, lab results, and other diagnostic reports if you have them.
You don’t have to pay for your own records before filing. Georgia’s Disability Adjudication Services will request them from your providers during the review. Having copies just speeds things up.
Work History
Compile the last five years of work: employers, dates, pay, hours, and the physical and mental demands of each job — how much you lifted, how long you stood or sat, what tools you used, whether the job required interacting with others.5Social Security Administration. Work History Report SSA-3369-BK The agency will compare those demands to your current limitations to decide whether you can go back to any past work.
The Forms That Carry Your Claim
All of these are available at ssa.gov.
Application for Disability Insurance Benefits (SSA-16)
This is the primary SSDI application. It captures basic demographic information and establishes your legal intent to file under Title II of the Social Security Act.6Social Security Administration. Application for Disability Insurance Benefits Form SSA-16 It takes roughly 20 minutes.
Adult Disability Report (SSA-3368-BK)
This is where the medical substance of your claim lives. You list every physical and mental condition that limits your ability to work, your medications and treatments, your education, and your recent work.7Social Security Administration. Disability Report – Adult SSA-3368-BK It also asks for two contacts who know about your conditions. Take your time here. Vague answers weaken the claim.
Work History Report (SSA-3369-BK)
A detailed breakdown of each job from the five years before your disability began: pay rate, hours, tasks, equipment, and specific time spent standing, sitting, stooping, kneeling, and lifting.5Social Security Administration. Work History Report SSA-3369-BK Only include jobs you held more than 30 calendar days.
Function Report (SSA-3373-BK)
Georgia’s Disability Adjudication Services may send you this form after you file. It asks how your condition affects daily life — cooking, dressing, driving, managing money, socializing. Not every applicant gets one. If you do, answer specifically. “I can’t do much” tells the examiner nothing; describe exactly what you struggle with and how the condition limits each activity.
Three Ways to Submit
- Online at ssa.gov. You can file for SSDI, and in many cases SSI, electronically and get a confirmation number immediately.
- By phone or in person. Call 1-800-772-1213 or schedule an appointment at a local Social Security field office to complete the application with a representative’s help. This is useful if you have questions about specific fields.
- By mail. Send completed, signed forms with supporting documents to your local field office. Use certified mail with return receipt so you have proof of the submission date.
However you file, the field office first checks non-medical factors: work credits for SSDI, income and resources for SSI.8Social Security Administration. Disability Determination Process Clear that screening and your file moves to the medical review stage.
The Medical Review at Georgia DAS
Georgia’s Disability Adjudication Services (DAS) is the state agency that handles disability determinations for the Social Security Administration.1Georgia Vocational Rehabilitation Agency. Social Security Services A DAS disability examiner and medical consultant review the clinical evidence and may contact your doctors directly for updated records or clarification.
If your existing records aren’t enough to decide the claim, DAS will schedule a consultative examination: a one-time evaluation by an independent doctor, paid for by the government.9Social Security Administration. Consultative Examination Guidelines Go. Missing this appointment can produce a denial based on insufficient evidence.
The initial decision typically takes six to eight months from your application date.10Social Security Administration. How Long Does It Take to Get a Decision After I Apply for Disability Benefits DAS mails you a formal notice explaining the outcome and its reasons.
Tracking Your Application
Sign into your “my Social Security” account, scroll to “Your Benefit Applications,” and select “View Details” to see where the claim stands.11Social Security Administration. How to Check Your Application Status You can also call your local field office or 1-800-772-1213.
If You’re Denied
Many claims denied at the initial level are approved later. You have 60 days from receiving the denial notice to appeal at each stage; the agency assumes you received the notice five days after it was mailed.12Social Security Administration. Information About Requesting Review of an Administrative Law Judges Hearing Decision There are four levels.
Reconsideration
A different examiner at Georgia DAS reviews your original file plus any new evidence you submit.13Social Security Administration. Request Reconsideration This is your opportunity to add updated medical records, new test results, or statements from your doctors that weren’t part of the initial claim.
Hearing Before an Administrative Law Judge
If reconsideration is denied, you can request a hearing before an administrative law judge. Many denials are overturned here because you appear in person or by video and can explain your limitations directly. Expect a 6-to-12-month wait after requesting the hearing.
Appeals Council Review
If the judge denies the claim, you can ask the Appeals Council in Falls Church, Virginia, to review. The Council may grant your claim, send it back for a new hearing, or decline to review the decision.14Social Security Administration. Appeals Process – Understanding SSI
Federal Court
If the Appeals Council denies or declines your case, your final option is filing a civil suit in federal district court. Most claimants hire an attorney for this stage.
Do You Need a Representative
You can file the initial application on your own, and many people do. You can also hire an attorney or non-attorney representative at any stage. Most work on contingency: under the standard fee agreement approved by the Social Security Administration, the fee is the lesser of 25 percent of your past-due benefits or $9,200, and the SSA withholds it directly from your back pay.15Social Security Administration. Fee Agreements You pay nothing upfront. Representation tends to matter most at the hearing stage, where a representative can present medical evidence and question vocational experts on your behalf.
