Oregon child support forms come from two places depending on your case. The Oregon Department of Justice (DOJ) Child Support Program provides forms for administrative cases, and the Oregon Judicial Department’s Guide & File portal provides forms for child support handled inside a divorce, separation, or custody case in circuit court. In either track, the document that does the real work is the Uniform Support Declaration, a sworn financial statement both parents must complete so the state can run the guideline calculation.

Which Track Applies to You
The DOJ handles administrative cases, typically those involving families who receive or recently received public assistance, or where a parent applies directly for enforcement services. Circuit courts handle child support that is being set as part of a divorce, separation, or custody case filed by a parent or attorney. A parent already receiving TANF has support rights automatically assigned to the state, but the DOJ provides services to any applicant, not only public-assistance families.4Oregon State Legislature. The Application to Open a DOJ Case
To open an administrative case, start with the Application for Child Support Services, Form CSF 03 0574. If parentage has not been established, you also complete the Declaration in Support of Establishing Parentage, Form CSF 11-0112.5Oregon Judicial Department. In counties where the district attorney handles non-assistance child support cases, the DOJ forms page will point you to the correct local office.
The Uniform Support Declaration
Whether your case is administrative or judicial, both parents must complete a Uniform Support Declaration, Form 8.010.5. Every figure you enter feeds directly into the Oregon Child Support Guidelines formula. You sign it under penalty of perjury, so inaccurate or incomplete entries can carry legal consequences and produce a support amount that doesn’t match reality.6Oregon Judicial Department. Page one is a summary: number of joint children, whether any are between 18 and 21 and attending school, your gross monthly income, and whether your household receives TANF or public health coverage for the children. Each item appears again in more detail later in the form.
Income
Section 2 asks for monthly income before taxes from every source. For wages, you enter gross hourly rate and average weekly hours, and the form converts to a monthly figure. Tips, commissions, and bonuses go in separately. A second subsection covers non-employment income: self-employment earnings, dividends, interest, trust distributions, Social Security, workers’ compensation, unemployment, disability, and expense reimbursements that reduce your personal living costs.7Oregon Judicial Department. Oregon’s definition of income for support is broader than taxable income. Employee benefits like a company car, employer-provided housing, or a phone plan count when they reduce your living expenses. Voluntary payroll deductions for health savings accounts, dependent care flexible spending accounts, and non-mandatory retirement contributions are also treated as income even though they don’t appear on your W-2 taxable wages.8Oregon Department of Justice. Health Care, Childcare, and Parenting Time
Sections 3 through 5 collect the numbers that adjust the final support amount. You report the monthly health insurance premium you pay for yourself alone, the premium you pay to cover the joint children, any out-of-pocket medical costs for the children, and any subsidies that offset insurance costs. If your children are 12 or younger and you pay for childcare, you enter those expenses along with the city where care is provided.
Section 5 asks for the number of annual overnights each joint child spends with you. This figure matters because Oregon’s guidelines reduce the paying parent’s obligation based on the share of parenting time. Self-Employment Income
Self-employed parents report differently. The guidelines start with gross business receipts and subtract legitimate business expenses under OAR 137-050-0715(1)(f). Judges and hearing officers don’t accept every deduction the IRS allows. Depreciation reduces taxable income on paper but not the cash a parent actually has. Vehicle expenses that mix personal and business use, meals and entertainment that benefit the parent personally, and payments to family members that aren’t market-rate work are routinely added back to income for support purposes.
Attach a profit-and-loss statement with your tax return. The more cleanly you separate real overhead from discretionary write-offs, the less room the other parent has to argue your income is higher than what you reported.
Documents to Attach
Gather these before you start filling anything out. The declaration includes a checklist:
- Four most recent pay stubs
- Most recent tax return
- Benefit statements for Social Security, SNAP, disability, or similar programs
- Copies of existing support or parenting time orders from this or any other relationship
- Proof of health insurance premiums and any subsidies received
- Proof of out-of-pocket medical expenses for the joint children
- Proof of childcare expenses
- Evidence supporting any rebuttal factors if you’re asking the court to deviate from the guideline amount under OAR 137-050-0760

Filing Fees and Waivers
The first-appearance filing fee in circuit court is $301 as of January 2026.10Oregon Judicial Department. Oregon Judicial Department Circuit Court Fee Schedule Many child support filings are exempt from fees, though. Petitions and motions filed under ORS 25.020, 107.085, 107.135, 109.100, 109.103, and several other statutes carry no filing fee, and parties eligible for DOJ support enforcement services also pay nothing.11Oregon State Legislature. Oregon Code 21 – State Court Fees In practice, a standalone motion to establish or modify child support costs $0, while the $301 fee applies when you’re filing a broader dissolution or custody case that includes support.
If you can’t afford the fee on a broader case, a judge can waive it entirely when household income is at or below 133 percent of the federal poverty guidelines, or defer it when income falls between 133 and 185 percent.12Oregon Judicial Department. Serving the Other Parent
Once the court accepts your filing, you must deliver copies of the documents to the other parent. This is service of process. Oregon doesn’t require a professional server. Under ORCP 7, any competent person at least 18, who lives in Oregon or the state where service happens and is not a party to the case, can serve the papers. A sheriff’s deputy, a friend, or a hired process server all qualify. Service by mail, using first-class mail plus certified or registered mail with return receipt requested, is also allowed in many situations.13Oregon Rules of Civil Procedure. Forms to Modify an Existing Order
Oregon has two tracks for changing an existing order. Through the DOJ, you can request an administrative review if at least 35 months have passed since the order was entered, last reviewed, or last modified. You can request a review sooner by showing proof of a substantial change in circumstances, such as a significant shift in either parent’s income, a change in custody, or a change in the children’s needs.14Oregon Department of Justice. Through the court, either parent can file a motion to modify under ORS 107.135. The statute requires a substantial change in economic circumstances, which can include changes in either parent’s income or a significant shift in reasonable and necessary expenses.15Oregon Public Law. Either route requires a new Uniform Support Declaration with current income and fresh supporting documents. The old declaration from the original case will not do the work, because the point of the modification is to show the numbers have changed.
