To file California SDI Form DE 2501, you complete Part A as the claimant, have a licensed health care provider complete Part B as the medical certification, and submit both to the Employment Development Department either through SDI Online or by mail. EDD will not begin processing until both parts arrive, and the complete claim must be filed no later than 49 days after your disability begins. Most complete claims are processed within 14 days of receipt, with weekly benefits ranging from $50 to $1,765 for claims starting in 2026.1Employment Development Department. Before You Start Part A
Pull these items together first. Each one has a line on the form, and a missing piece is the usual reason a claim stalls.
- Your Social Security number, which EDD uses to pull your wage records.
- A photo ID (driver’s license, state ID, or U.S. passport) plus a second identity document such as a W-2, utility bill, or birth certificate. Your name should match what the DMV or Social Security Administration has on file.3Employment Development Department. Step 1 – Get Your Information In Order
- Your most recent employer’s name, phone number, and mailing address as shown on your W-2 or paystub, along with the last date you worked your normal hours.3Employment Development Department. Step 1 – Get Your Information In Order
- Any other names you have used for work, including a maiden name, so EDD can locate all your taxable wages.
- The specific date your condition first prevented you from doing your regular job.
Fill the form out carefully. Willfully making a false statement or using a false identity to obtain benefits is a crime under California Unemployment Insurance Code Section 2101, prosecutable as a misdemeanor or felony depending on the amount, with fines up to $20,000 and mandatory repayment.4California Legislative Information. California Code Unemp Ins Code 2101 – False Statements
One eligibility figure to confirm before you file: you need to have earned at least $300 in wages that had SDI deductions withheld during your base period, roughly 5 to 18 months before your claim start date.5Employment Development Department. California Disability Insurance Application Form DE 2501 You also cannot be working or receiving other wage-replacement benefits for the period you claim.
Getting Part B Completed by Your Provider
Part B is the medical certification, and your claim goes nowhere without it. EDD will not process the application until both your claimant statement and the medical certification arrive.6Employment Development Department. Step 3 – Have a Medical Certification Completed Your provider can submit Part B electronically through SDI Online or complete the paper section of the DE 2501, which you then mail together with Part A.
The range of professionals who can sign is broader than many claimants expect. Under Unemployment Insurance Code Section 2708, “physician” incorporates the definition in Labor Code Section 3209.3 and covers doctors of medicine, doctors of osteopathy, psychologists, acupuncturists, optometrists, dentists, podiatrists, and chiropractors licensed in California and practicing within their scope.7California Legislative Information. California Labor Code 3209.3 – Physician Definition Nurse practitioners can certify after performing a physical exam and collaborating with a physician. Physician assistants can certify if they examined you under a supervising physician’s direction. For pregnancy and childbirth, licensed midwives and nurse-midwives can complete the certification.8California Legislative Information. California Code UIC 2708 – Filing, Determination and Payment of Disability Benefit Claims
The provider supplies a diagnosis, a description of how your condition prevents you from working, and an estimated date you can return to work. That return-to-work date drives how long your benefits run, so ask for a realistic estimate rather than an optimistic one you will need to extend later.
If You Rely on Prayer Instead of Medical Treatment
Claimants who rely entirely on prayer or spiritual means for healing can substitute Form DE 2502, a religious practitioner’s certificate, for Part B. The practitioner must be accredited in writing by EDD and provide a detailed statement of symptoms, the date spiritual treatment began, how frequently prayer is given, and a specific estimated recovery date. “Unknown” and “indefinite” are not acceptable.9Employment Development Department. Submitting the Claim: Online or by Mail
Filing through SDI Online is faster and gives you more control. You log in through your myEDD account, complete Part A on screen, and your provider logs in separately to submit Part B. SDI Online is also the only filing method that offers direct deposit, and it lets you track your claim status and respond to EDD requests without waiting for postal mail.10Employment Development Department. If you prefer paper, you can get a blank DE 2501 by ordering one through EDD’s website, picking one up at an SDI office, asking your doctor or employer, or calling 1-800-480-3287 and selecting option 3.11Employment Development Department. Paper claims take longer because of mail transit and manual data entry. Most complete claims are processed within 14 days of receipt regardless of method.12Employment Development Department. What You Need to Know about SDI Online
The 49-Day Deadline
EDD recommends filing no earlier than nine days after your disability begins and no later than 49 days after it begins.10Employment Development Department. Disability Insurance Claim Process The outer window aligns with Unemployment Insurance Code Section 2706.1, which requires the completed claim, including the medical certification, to be filed no later than 41 days after the first compensable day of your disability.13California Legislative Information. California Code Unemp Ins Code 2706.1 – Filing, Determination and Payment of Disability Benefit Claims Because the first compensable day is day eight (after the mandatory seven-day waiting period), the statutory 41 days from that point translates to roughly 49 days from when the disability started.
Miss the window and you risk losing benefits or having the claim disqualified entirely. EDD can grant an extension for good cause, but “I didn’t know” is a hard sell. The medical certification carries the same deadline, so do not assume you can submit Part A on time and let Part B trickle in weeks later. If Part B is the piece slowing you down, file Part A and press your provider in parallel rather than waiting to send everything together.
After You File
Every new SDI claim begins with a mandatory seven-calendar-day waiting period during which no benefits are paid. The first payable day is day eight.10Employment Development Department. Disability Insurance Claim Process If you file a second claim for the same or a related condition within 60 days of your original benefit period, you do not have to serve the waiting period again.14California Legislative Information. California Code Unemp Ins Code 2627 – Waiting Period
Once your claim is approved, EDD sends a Notice of Computation showing your weekly benefit amount. Benefits can continue for up to 52 weeks as long as your medical provider certifies you remain unable to work.5Employment Development Department. California Disability Insurance Application Form DE 2501
Payment Method
When you file, you choose how to receive payments:
- Direct deposit, available only to SDI Online filers. Set it up through your myEDD profile by editing the benefit payment option and entering your bank routing and account numbers.15Employment Development Department. Direct Deposit
- A Money Network Prepaid Debit Card mailed by EDD, which works anywhere Debit Mastercard is accepted. Activate it by calling 1-800-684-7051 or through the Money Network app.
- A paper check mailed to your address on file.
If you chose direct deposit but entered incorrect bank details, EDD falls back to a debit card or mailed check and you will need to re-enroll with corrected information.15Employment Development Department. Direct Deposit
Keeping Benefits Flowing
Benefits do not run automatically for 52 weeks. If you are on automatic payments, EDD mails a Continuing Eligibility Certification (Form DE 2593) after 10 weeks, and you must return it to confirm your disability continues. If you are not on automatic payments, EDD sends a Claim for Continued Disability Benefits (Form DE 2500A) every two weeks. Failing to return either form stops your payments.10Employment Development Department. Disability Insurance Claim Process
One note for pregnancy claims: SDI covers the time you are physically unable to work, typically up to four weeks before your estimated delivery date and six weeks after (eight weeks for a cesarean), with longer periods if complications are certified by your doctor.16Employment Development Department. Disability Insurance – Pregnancy FAQs Bonding time with a newborn falls under Paid Family Leave, a separate program with a separate claim.
If Your Claim Is Denied
If EDD finds you ineligible, it mails a Notice of Determination (Form DE 2517) along with an Appeal Form (Form DE 1000A or DE 1000M).10Employment Development Department. Disability Insurance Claim Process You have 30 days from the mail date on the notice to file a written appeal. If you miss the 30-day window, you must explain the delay, and an administrative law judge decides whether you had good cause; without it, the appeal is dismissed.17Employment Development Department. Appeal Form
Appeals that are not resolved internally go to the California Unemployment Insurance Appeals Board’s local Office of Appeals. That office mails a hearing notice with the date, time, and location. An impartial administrative law judge hears testimony from you and an EDD representative and decides based on the facts. You must attend; failing to appear results in automatic dismissal.18Employment Development Department. State Disability Insurance Appeals
The most common denial reasons are filing after the 49-day deadline, an incomplete or missing medical certification, and insufficient base-period earnings below the $300 minimum. The medical certification issue is often the most fixable: if your provider’s submission was unclear about how your condition prevents you from working, getting a more detailed certification for the appeal can change the outcome.
