How to Fill Out and File the Connecticut Caseflow Request (JD-FM-292)

The Connecticut Caseflow Request, Form JD-FM-292, is how you ask a family court judge to schedule a status conference, a pretrial, or a hearing on a pendente lite motion you have already filed. It is a one-page form available as a PDF from the Connecticut Judicial Branch, and it applies to divorce, custody, child support, and other family division matters.1Connecticut Judicial Branch. What the Form Does

JD-FM-292 gives the court four options you can check: a status conference on a proposed date, a pretrial on a proposed date, a hearing on one or more pendente lite motions on the earliest available motion docket, or an “Other” request you write in. Two things this form does not do. It is not an appearance. If you need to enter the case formally as a party or attorney, that is Form JD-CL-12, and you need an appearance on file before submitting a caseflow request.2Connecticut Judicial Branch. What to Gather Before You Start

  • Case name, docket number, and judicial district.
  • Any court events already scheduled in the case, with dates and the judge’s name.
  • The number and title of each pendente lite motion you want heard.
  • Names and full mailing addresses for every attorney and self-represented party in the case.
  • Your juris number and firm name, if you are an attorney.

How to Complete Each Section

Case Information and Type of Request

Enter the caption, docket number, and judicial district at the top. If a judge has already scheduled something related to the request, add that date and the judge’s name. Then check the box or boxes for what you are asking the court to do. For a status conference or pretrial, write in your proposed date. Reason for the Request

State your reasons. This field is required for every type of request. A blank reason line gives the judge nothing to evaluate. Be specific about why you need the conference, pretrial, or hearing and why your proposed timing makes sense.

Existing Scheduled Events

List any trial dates, specially assigned hearings, or other court events already on the calendar. The court uses this to avoid conflicts and to see where the case stands.

Consent of Other Parties

Indicate whether all other counsel and self-represented parties consent to the request. Before filing, you must confirm on the form that you told all counsel and self-represented parties of record that you would be asking for the action.1Connecticut Judicial Branch. Your Identification and Signature

Check whether you are the plaintiff, defendant, or the attorney for either. Print your name, juris number if applicable, firm name, address, and phone number, then sign and date. By signing, you also agree to notify your client and the other parties of the outcome once the court rules.

Certifying Service on the Other Parties

The certification at the bottom of the form is your signed statement that a copy was or will immediately be mailed or delivered, electronically or on paper, to all counsel and self-represented parties of record. If you deliver electronically to someone who is exempt from e-filing or to a self-represented party, you need their written consent for electronic delivery.1Connecticut Judicial Branch. List the name and full mailing address of every person you served, then sign, print your name, add your phone number, and date the certification. Skip this step and your request can stall.

How to File It

Attorneys generally file through the Judicial Branch’s E-Services portal. Self-represented parties are not required to e-file in trial court matters and can file on paper.3Connecticut Judicial Branch. Request an Exemption from E-Services Requirements To file on paper, deliver or mail the completed form to the clerk’s office in the judicial district where the case is pending. Keep a copy and confirm the filing appears on the docket.

What Happens After You File

A judge reviews the request and either grants or denies it using the Order section at the bottom of the form. If granted, the court schedules the event for your proposed date when that date is available, or the next available date if it is not.1Connecticut Judicial Branch. Once the court rules, you must notify your client and all other parties whether the request was granted or denied and what the court ordered. The form puts that duty on the person who filed it, not the clerk.

When to Use This Form Under Pathways

Connecticut family cases are managed under Pathways, which replaced the older Short Calendar model in 2021. Under Pathways, pendente lite motions are generally argued on the case’s next scheduled date under Practice Book Section 25-34A.4American Academy of Matrimonial Lawyers. The form is also used in pre-2021 cases that were never part of Pathways but still need something put on the calendar.
