How to Fill Out and File the Tennessee Unemployment Form LB-0489

Tennessee unemployment Form LB-0489 is the Separation Notice your employer must hand you within 24 hours of your last day, and you use the details on it to file your unemployment claim online at Jobs4TN.gov.1Tennessee Department of Labor and Workforce Development. Separation Notice LB-0489 The form itself is not your application for benefits. It’s a record the employer creates about why and when you left, and the information on it feeds the state’s decision about whether you qualify.

What LB-0489 Is and When You Should Get It

LB-0489 is a state-issued Separation Notice completed by the employer, not the worker. Under Rule 0800-09-01-.02 of the Tennessee Employment Security Law, every employer must fill it out and give it to a separated employee within 24 hours of the separation.2Tennessee Secretary of State. Rules of Tennessee Department of Labor and Workforce Development – Rule 0800-09-01-.02 Two narrow exceptions apply: the employer does not have to furnish the notice if you worked less than one full week, or if you will be recalled within seven days.

Keep your copy. You’ll need to read from it when you file, and you may need it later if the department questions why you left.

What Information Is on the Form

Your employer fills in the whole form. Check each section against what you know to be true before you rely on it to file.

  • Your full name, Social Security number, occupation, and the location where you worked.
  • The start and end dates of your employment.
  • The reason for separation: lack of work (permanent, or temporary with a recall date), discharge, or voluntary quit. If the reason is anything other than lack of work, the employer must explain the circumstances.
  • Any vacation pay, severance pay, or wages in lieu of notice, with the amounts and the periods they cover.
  • The business name, address, phone, email, and the employer’s state account number.1Tennessee Department of Labor and Workforce Development. Separation Notice LB-0489

The separation-reason box is the single most important entry on the page. It drives the Tennessee Department of Labor and Workforce Development’s initial decision on your claim. If the employer marks “discharge” or “quit,” the department will investigate before approving benefits. What to Do With the Form

Use it to file. You do not submit LB-0489 as your application. You file at Jobs4TN.gov, select “File for Unemployment,” and create an Unemployment Claimant e-Services account if you don’t already have one.3Tennessee Department of Labor & Workforce Development. Apply for Unemployment Benefits You’ll need that account to file, certify each week, and manage the claim.

Have the separation notice open while you work through the application. The online form asks for the same facts your employer wrote down: employment dates, the employer’s name and address, and the reason you separated. When your answers match the employer’s record, the department doesn’t have to open a conflict investigation that holds up payment.

Pull these together before you start:

  • Your Social Security number.
  • Work authorization documentation if you are not a U.S. citizen or permanent resident.1Tennessee Department of Labor and Workforce Development. Separation Notice LB-0489

  • Your employment history for roughly the last 18 months, including each employer’s name, address, and dates.

If Your Employer Didn’t Give You One, or the Reason Is Wrong

You can still file without LB-0489 in hand. The 24-hour rule binds the employer, not you, and your eligibility doesn’t depend on possessing the paper. Go to Jobs4TN.gov and file with the information you have: dates, employer address, and your own account of why you left. The department will contact the employer for its side.

If the form says you quit when you were laid off, or marks “discharge” over something you dispute, file your claim with the accurate reason and be prepared to explain. Tennessee denies benefits for voluntarily quitting without good work-related cause, discharge for work-related misconduct, participating in a labor dispute other than a lockout, and refusing suitable work.4Tennessee Department of Labor & Workforce Development. Do I Qualify? Quitting over unsafe conditions or a significant change to your job terms may still qualify, though the department will look closely. Keep documents, emails, and names of witnesses that support your version of events.

What Happens After You File

It shows your weekly benefit amount, your maximum total benefit for the benefit year, the wages each employer reported during your base period, and the start of your benefit year.5Tennessee Department of Labor and Workforce Development.
