To visit someone in an Iowa state prison, you need to submit an Iowa DOC visitor application by mail to the Centralized Visiting office in Mt. Pleasant. The Iowa Department of Corrections does not take electronic submissions, so the form has to be printed, signed, and sent in on paper. The same application covers both in-person visits and video calls, and both are free once you are approved.
Where to Get the Form
The application (form OP-MTV-04 F-1) is posted in English and Spanish on the department’s “How Do I Visit an Offender” page at doc.iowa.gov.1Iowa Department of Corrections. Visit an Offender If you cannot download it, call Centralized Visiting at (319) 385-9511 to request a paper copy, or pick one up at any Iowa correctional facility.
What the Application Asks
The form has four parts: information about the incarcerated person, your own personal information, any children who will visit with you, and your legal and background history.
Offender and Personal Details
You need the incarcerated person’s full name and IDOC offender number. If you don’t know the number, look it up on the Iowa DOC’s online offender search before you start filling out the form.
Your section asks for your legal name (including maiden name if applicable), date of birth, sex, marital status, spouse’s name, Social Security number, phone number, full mailing address including county, your relationship to the incarcerated person, and how long you have known them.2Drake University. The Yes-or-No Background Questions
The second half of the form is a series of yes-or-no questions the department uses to screen for security concerns. Answer every one, and answer honestly. The department runs a background check against state and national databases, and any discrepancy between what you wrote and what the check turns up will likely result in denial. The questions cover:
- Any pending criminal charges, including what they are and where.
- All adult and juvenile arrests, including misdemeanors, felonies, deferred judgments, and any jail time served.
- Whether you are or have ever been incarcerated, on probation, or on parole.
- Any history of illegal drug use.
- Whether you are or have ever been a DOC employee, volunteer, contractor, or private-sector employer working for the department.
- Whether you are or have been on any other incarcerated person’s visiting list in Iowa.
- Whether your visiting privileges have ever been denied, suspended, or terminated.
- Whether you have been the victim of someone supervised or incarcerated by the IDOC.
- Whether you are a legal U.S. citizen. You will need a picture ID at the visit regardless of your answer.
If Children Will Visit With You
Minors under 18 do not file their own applications. A parent or legal guardian lists each child on their own visitor application.1Iowa Department of Corrections. Choose one of two options: the child may visit only when accompanied by you, or the child may visit with any approved adult on the incarcerated person’s visiting list. During a visit, a minor must be under the immediate supervision of their parent or legal guardian; the incarcerated person’s own minor children may be supervised by any adult on the approved list.4Cornell Law Institute. Iowa Admin Code r 201-20.3 – Visits to Incarcerated Individuals When the incarcerated person is a sex offender whose victim was a minor, no children may be added until that person completes the department’s sex offender treatment program. Once a listed child turns 18, they need to file their own adult application.

Do not email or fax it. Sign the form before you mail it; an unsigned application will not be processed.
What Happens After You Mail It
The Centralized Visiting Authority at Mt. Pleasant receives the application and runs the background check. If the application is denied, the department has to send written notice of the denial and the reasons for it to both you and the incarcerated person within 30 days of receiving the form.4Cornell Law Institute. Who Will Not Be Approved
Under Iowa’s administrative code, several categories of people cannot be approved:
- Anyone convicted of a felony.
- Anyone currently under criminal indictment.
- Anyone currently on probation, work release, or parole.
- Anyone who intentionally provides false information on the application.
- Anyone convicted of or involved in aiding an escape or smuggling contraband into any correctional facility.
- Anyone whose behavior represents a control problem, or whose background shows a pattern of carrying concealed weapons, controlled substance use, or prior violations of institutional rules.

The felony bar and the community-supervision bar catch a lot of applicants. An old felony conviction makes denial essentially automatic under the current rule. Volunteers who work at the facility also cannot appear on a visiting list without specific permission from the warden.
Appealing a Denial
A denial has two levels of appeal. First, write to the warden or the warden’s designee at the facility where the incarcerated person is housed, explaining why you believe the decision should be reconsidered. If the warden upholds the denial, you can appeal to the Director of the Iowa Department of Corrections. The director’s decision is final agency action.4Cornell Law Institute. After You Are Approved
Approved visitors schedule both in-person and video visits through the Ameelio app. The Iowa DOC website has download instructions and a setup guide, and neither type of visit costs anything.1Iowa Department of Corrections. Visit an Offender Visiting hours vary by facility, so check the specific institution’s page before booking.
Bring a valid government-issued photo ID, such as a driver’s license or state ID. Leave phones, bags, food, and tobacco in your car. Under the administrative code, all visitors must wear shoes, and the following are not allowed: miniskirts or shorts above the knee; muscle shirts, halter tops, or see-through clothing; and clothing with slogans, pictures, or words that deprecate race, sex, or cultural values. Staff may ask you to remove outerwear, hats, gloves, and sunglasses during the visit; bring a prescription if you need sunglasses for a medical reason.4Cornell Law Institute.
