How to Fill Out and Schedule the Massachusetts RMV Road Test Application

The Massachusetts RMV road test application is a one-page paper form, called the Class D or M Road Test Application, that you download from mass.gov or pick up at any RMV Service Center, fill out at home, and hand to the examiner on test day.1Mass.gov. RMV Forms and Applications The same form covers both the passenger car (Class D) and motorcycle (Class M) tests; you check the box for the one you’re taking. You do not submit it online.2Mass.gov. Where to Get the Form

The application lives on the RMV forms page at mass.gov as a downloadable PDF. Printed copies sit at every RMV Service Center counter.1Mass.gov. RMV Forms and Applications Either version is the same document. Print it, fill it in with a pen, and keep it with your other test-day paperwork. There is no electronic submission step.2Mass.gov. What You Need to Fill It In

Have these in front of you before you start writing:

  • Your learner’s permit number. Current Massachusetts permits use a two-letter, seven-digit code beginning with “SA.”
  • Your Social Security number.3Mass.gov. Passenger (Class D) Road Tests
  • Your full legal name and date of birth, written exactly as they appear on your permit.
  • Your current Massachusetts residential address on file with the RMV.

Check the Class D or Class M box to tell the examiner which evaluation to run. The form also asks a short set of medical and driving-history questions. If you answer yes to any of them, an authorized RMV employee has to review and approve the application before you can take the test, so don’t leave that review for the morning of your appointment.2Mass.gov. If You’re Under 18: Signature and Prerequisites

The application has a parental consent section. A parent or legal guardian has to sign it. Without that signature, the RMV will not let you take the test.4Mass.gov. Schedule Your Road Test If the person signing isn’t your parent (a legal guardian, a boarding school headmaster), they need to bring documentation showing their authority to sign for you.2Mass.gov. Class D and M Road Test Information Sheet

A clean application is only part of it. Under Chapter 90, Section 8, a minor applicant must also:

  • Be at least 16½ years old.
  • Have held a learner’s permit for at least six consecutive months with a clean record during that time: no surchargeable incidents, no permit suspensions.
  • Have completed an approved driver education program (at least 30 hours of classroom instruction and 18 hours of on-road instruction with a certified instructor).
  • Have logged at least 40 hours of supervised driving certified by a parent or guardian, reduced to 30 hours after completing a closed-course driver skills development program.5General Court of Massachusetts. Massachusetts Code Chapter 90 Section 8 – Operators Licenses, Applications, Examinations

If any of those pieces are missing, the application is rejected no matter how neatly it’s filled in. The supervised hours and driver education requirements are the ones people underestimate. Start logging early.

Scheduling the Test and Paying the Fee

Once the form is complete, book your appointment through the RMV’s online service center at mass.gov using your name, date of birth, Social Security number, and permit number.3Mass.gov. Passenger (Class D) Road Tests Road tests run by appointment only, Monday through Friday, 9:00 a.m. to 5:00 p.m., at Service Centers across the state.6Mass.gov. Massachusetts Registry of Motor Vehicles Locations

The road test fee is $35, paid online when you schedule (or afterward). You can prepay the $50 five-year license fee at the same time for either Class D or Class M.7Mass.gov. Massachusetts Registry of Motor Vehicles Fees The $35 is nonrefundable in most situations: you lose it if you fail, show up late or unprepared, don’t show at all, bring a vehicle that isn’t properly registered, or cancel with less than 72 hours’ notice.4Mass.gov. Schedule Your Road Test Rescheduling uses the same portal; selecting “schedule a road test” cancels the current appointment and lets you pick a new slot, as long as you’re more than 72 hours out.

What to Bring to the Test

Walk in with all of the following or the examiner will cancel the test:

  • The completed paper application, signed by a parent or guardian if you’re under 18.
  • Your valid learner’s permit. The physical paper permit has to be legible; if it’s damaged, you’ll need a duplicate first.2Mass.gov. Class D and M Road Test Information Sheet
  • A qualified sponsor for Class D tests. The sponsor must be at least 21, have at least one year of driving experience, hold a valid license from their home state or U.S. territory, and present the physical license to the examiner. A phone image won’t be accepted.3Mass.gov. Passenger (Class D) Road Tests2Mass.gov. Class D and M Road Test Information Sheet
  • A properly equipped, legally registered vehicle with a current Massachusetts inspection sticker, in safe working condition, with a parking brake the examiner can reach and operate from the passenger seat.3Mass.gov. Passenger (Class D) Road Tests

The parking brake catches people. Many newer cars put a small electronic button on the center console that the examiner physically cannot operate from the right seat. Check yours before you drive to the appointment. If the examiner can’t use it, the test is canceled and the $35 is gone.

Out-of-state vehicle? Bring proof of insurance meeting Massachusetts minimums: $25,000/$50,000 bodily injury and $30,000 property damage.2Mass.gov. Class D and M Road Test Information Sheet Rental? Bring the rental agreement plus written permission on the rental company’s letterhead, signed by an authorized representative, allowing use of the vehicle for a road test.

If You’re Applying for Class M Instead

The form is the same, but a few of the rules above don’t apply. Class M applicants don’t need a sponsor.4Mass.gov. Schedule Your Road Test The vehicle you bring is your motorcycle, which must have a valid registration and inspection sticker; the examiner can cancel if they believe the motorcycle is unsafe.8Mass.gov. Motorcycle (Class M) Road Tests Taking the test on a limited-use vehicle or a three-wheeled motorcycle gets you a Class M license with a “U” restriction, which limits you to that vehicle type. Everything else on the application, scheduling, and under-18 requirements works the same way.
