How to Fill Out and Submit AK Form 385-1-E: Testing and OF 346

AK Form 385-1-E is the Eighth Army worksheet you complete to request a U.S. Government Motor Vehicle Operator’s Identification Card (OF 346) in Korea. You fill out your personal information in Section I, report to the installation licensing facility for the testing and briefings that make up Sections II through IV, and then route the completed worksheet through your unit’s Master Driver and commander for endorsement. Once everything clears, the installation issues your OF 346 listing the specific vehicle types you are cleared to operate.1U.S. Army. What You Need Before You Start

A missing prerequisite stops the process cold, so gather these first.

Glasses or contacts are fine as long as your corrected vision meets the standard. The form asks whether you wear glasses and whether you are colorblind, so know your answers before you sit down.

Section I: What You Fill Out

Section I is the only part of the worksheet you complete yourself. It runs 17 fields and must be legible, because a clerk who can’t read your handwriting will return the form.

  • Fields 1 through 5: last name, first name, and middle initial; grade or civilian equivalent; sex; date of birth; and DoD ID number.
  • Fields 6 through 8: state driver’s license number, issuing state, and expiration date.
  • Field 9: your DEROS (Date Eligible for Return from Overseas), which ties the license to your remaining time on assignment.
  • Fields 10 through 15: whether you wear glasses, colorblind status, height, weight, hair color, and eye color.
  • Fields 16 and 17: unit designation and duty phone number.

Check the unit designation against your current assignment. An outdated unit listing creates confusion when the licensing facility routes your paperwork for command endorsement.1U.S. Army. AK Form 385-1-E Request for GOV Vehicle License (OF346) Worksheet

Section II: Testing at the Licensing Facility

You do not fill out Section II. Staff at the licensing facility complete it during your in-person testing appointment and record three results: visual acuity, reaction time, and your test score. Testing follows a three-part structure of written exam, Pre-Operational Maintenance Checks and Services (PMCS) evaluation, and road test, scaled to the vehicle types you’re requesting.3United States Army Reserve. USAR Regulation 600-3 – The Army Driver and Operator Standardization Program

If you already hold an OF 346 and are adding a new vehicle type, you take a performance test on that vehicle rather than repeating the full sequence. Bring your DA Form 348 so the examiner can record results directly into your qualification record.

Sections III and IV: The Briefings

Sections III and IV are not fill-in-the-blank. They are structured briefings the licensing facility walks you through, and you acknowledge each topic as it’s covered.

Section III addresses operational requirements: the USFK Driver Licensing Course, driving risks specific to Korea, post-accident reporting using SF Form 91 and DD Form 518, PMCS responsibilities, and required safety equipment (warning triangles, first aid kit, fire extinguisher). It also covers the USFK Highway Condition Code system, which governs when you can operate off-installation:1U.S. Army. AK Form 385-1-E Request for GOV Vehicle License (OF346) Worksheet

  • Green: normal conditions, standard rules.
  • Amber: only government vehicles on essential official business may leave the installation, authorized by an O-3 (company or detachment commander), CW3 or above, or a civilian supervisor at GS-10/KGS-10 or above.
  • Red: essential and emergency missions only, authorized by an O-5 or above, or a civilian supervisor at GS-13/KGS-13 or above.
  • Black: roads are not passable; only a colonel (O-6) or above may personally authorize emergency vehicle use.

Section IV covers government vehicle misuse. The briefing official reviews the penalties under AR 58-1 and walks through scenarios that constitute fraud, waste, or abuse. The signed acknowledgment creates a documented record that you were warned.

Routing: Master Driver, Commander, and Licensing Facility

Once Section I is complete and the briefings are done, the worksheet goes to your unit’s Master Driver. The master driver is the subject-matter expert on driver training within your organization and keeps records for everyone operating government equipment.4The United States Army. Safety at Forefront of Garrison Master Driver Training, Qualifications Program

The master driver checks your DA Form 348 against the vehicle types you’ve requested, confirms your training records are current, and verifies USFK Driver Licensing Course completion. Any mismatch sends the form back to you. When the master driver signs off, the form moves to the installation Transportation Motor Pool or licensing facility for final processing.

A commanding officer or designated representative then endorses the form to confirm you have a legitimate mission-based need for the license. Make sure that signature is legible and dated. Small clerical issues like an unreadable signature are common reasons a form gets kicked back a step.

Receiving and Keeping Your OF 346

After everything clears, the installation issues your OF 346, the U.S. Government Motor Vehicle Operator’s Identification Card. The card lists the specific vehicle types you are authorized to operate.5General Services Administration. U.S. Government Motor Vehicle Operator’s Identification Card

Carry the OF 346 on your person at all times while driving a government vehicle. Federal regulations also require you to carry a valid agency identification card or credential, such as your CAC or building pass, while operating the vehicle.6eCFR. 5 CFR Part 930 Subpart A – Motor Vehicle Operators

Authorization is not permanent. Federal regulations require agencies to review each operator’s authorization at least once every four years, and renewal depends on continued medical fitness and a safe driving record. If you qualify on a new vehicle type during your assignment, you’ll run through the testing and worksheet process again for that category, and the licensing facility will update both your OF 346 and your DA Form 348.6eCFR. 5 CFR Part 930 Subpart A – Motor Vehicle Operators

Penalties for Misuse

Driving a government vehicle without an OF 346 or without proper dispatch paperwork is treated as a violation, and the consequences differ by status.7U.S. Indo-Pacific Command. AK Form 385-1-E Request for GOV License

Military personnel who willfully use or authorize the use of a government vehicle for anything other than official purposes under 31 USC 1344 face discipline under the Uniform Code of Military Justice or other administrative action. Civilian employees face a mandatory minimum 30-day suspension without pay under 31 USC 1349(b), with longer suspensions or removal where circumstances warrant. Both military and civilian personnel may also be assessed financial liability for loss, damage, or destruction resulting from negligent use. The Section III and IV briefings exist in part to remove any “I didn’t know” defense if an allegation comes up later.
