How to Fill Out and Submit Alabama Form DL1-93: Enrollment and Exclusion

The Alabama DL1-93 form is the school enrollment and exclusion form that any driver license applicant under 19 must submit to the Alabama Law Enforcement Agency (ALEA). You fill in your personal details in Part I, your school’s attendance officer completes Part II to verify enrollment, and Part III is used only if a physician or employer needs to certify an exclusion. Then you bring the signed original to your ALEA appointment.

Who Needs to Submit a DL1-93

Alabama law requires ALEA to deny a learner’s permit or driver’s license to anyone under 19 who cannot show proof of graduation or current enrollment in a secondary school, GED program, nontraditional high-school diploma option program, or an approved job training program. You need the DL1-93 for a first-time permit or license, and again for renewal or reinstatement as long as you’re still under 19.2Alabama Law Enforcement Agency. Driver License Forms Most school front offices, guidance departments, and attendance offices also keep blank copies. All information must be printed or typed. Unclear handwriting can hold up processing at the ALEA counter.

How to Complete Each Part

The form has three parts. You handle Part I. A school official handles Part II. Part III is only for applicants who qualify for an exclusion.

Part I: Your Information

Enter your full legal name, Social Security number, sex, date of birth, driver’s license number if you already have one, and current street address including city, state, and zip code.5Alabama Law Enforcement Agency. A middle initial on one document and a full middle name on another is the kind of small mismatch that causes delays.

Part II: Enrollment

Part II lists the name and full address of your school, GED program, nontraditional high-school diploma option program, or job training program, with a checkbox for your situation.5Alabama Law Enforcement Agency. Part III: Exclusion

If you’re under 19 but not currently a student, you can qualify for one of several exclusions.2Alabama Law Enforcement Agency. Alabama Code 16-28-40 – Enrollment / Exclusion Form

  • Parent with care and custody of a minor or unborn child, verified by a physician or health department.
  • Sole source of transportation for your parents, certified by a physician.
  • Gainful and substantial employment, confirmed by your employer’s signature.
  • Circumstances beyond your control: physical or mental inability to attend, legal employment under child labor laws, or living more than two miles from the nearest public school with no public transportation available.

Suspension, expulsion, and imprisonment don’t count as “circumstances beyond your control.”2Alabama Law Enforcement Agency. Alabama Code 16-28-40 – Enrollment / Exclusion Form If you’ve been expelled and don’t fit another exclusion, you’ll need to enroll in a GED program or another qualifying option to get Part II signed.

Who Signs the Form

By law, the attendance officer or chief attendance administrator at your school completes and signs the enrollment portion.3Alabama Law Enforcement Agency. Enrollment / Exclusion Form In practice, many schools route requests through a guidance counselor or the principal’s office. The official provides a signature, title, and telephone number. There is no signature line for the student. The only other signatures are the physician or employer signatures used in Part III.5Alabama Law Enforcement Agency. The instructions call for printed or typed information and ink signatures. ALEA doesn’t reference electronic or digital signatures anywhere on the form, so plan on a wet signature. Request the form during the school year when office staff are available. Summer break and holidays can make finding the right official harder than expected.

Taylor’s Law Points on the Form

Part II includes a field for total disciplinary points under Taylor’s Law, established by Act 2009-713. Points accumulate starting in the school year you turn 13, and each point delays your eligibility to apply by one week.6Alabama Legislature. Alabama Code 32-6-7.4 – Disciplinary Point System

  • In-school suspension: 1 point per day
  • Out-of-school suspension: 2 points per day
  • Alternative school placement: 6 points
  • Expulsion: 20 points

The school official enters your total on the form.7Alabama State Department of Education. Alabama Code 32-6-7.4 – Disciplinary Point System Even so, an expulsion in ninth grade (20 points) could push a permit application back by roughly five months.

What to Bring to Your ALEA Appointment

The signed DL1-93 is one piece of a larger document package for a learner’s permit.8Alabama Law Enforcement Agency. Document Requirements and Fees You’ll also need:

  • A certified U.S. birth certificate from a Bureau of Vital Statistics (no photocopies).
  • Your original Social Security card, or an accepted alternative such as a W-2 or DD-214.
  • Two proofs of principal residence, such as a utility bill less than 90 days old, a vehicle registration in your name, a voter registration card, or school enrollment documentation.
  • Proof of enrollment or graduation, which is the DL1-93 itself, a diploma, or a GED certificate.
  • A $5 testing fee and $36.25 to purchase the license. ALEA does not accept personal checks.

If You Stop Attending School

Alabama treats accumulating more than 10 consecutive or 15 total unexcused absences in a single semester as withdrawal. You don’t have to formally drop out. Once a student 16 or older crosses that threshold, the attendance officer notifies ALEA, and ALEA sends a letter within five days stating the license or permit will be suspended on the 30th day after the letter is mailed.1Alabama Legislature. Getting back into compliance and submitting documentation before the 30 days runs out prevents the suspension.

Using the DL1-93 to Reinstate a Suspended License

To reinstate a license suspended for school noncompliance, get a new DL1-93 showing you’re back in compliance, whether that’s re-enrollment in school, a GED program, a job training program, or meeting one of the exclusion categories.3Alabama Law Enforcement Agency.
