How to Fill Out and Submit FDNY Form A-433: Certificate of Fitness

To get an FDNY Certificate of Fitness, you complete the application online through the FDNY Business portal, pay the $25 fee, and then walk in to FDNY Headquarters in Brooklyn to sit for the exam. Most applicants also need a recommendation letter from their employer before the certificate card can be issued. The FDNY Certificate of Fitness application is handled entirely online at the front end — the agency no longer accepts new requests by mail, email, or in person at the counter.1Fire Department. What to Gather Before You Apply

A Certificate of Fitness (COF) authorizes you to handle specific hazardous materials, operate certain building systems, or supervise fire-safety work in New York City.2Fire Department of New York. Certificates of Fitness Each certificate has its own code (for example, F-60 for fire guard torch operations, S-12 for citywide sprinkler supervision, P-15 for motor fuel-dispensing stations). Your employer or the job posting will tell you which code you need. Entering the wrong code means studying for and sitting for the wrong test, so confirm it before you start the application.

Before you log in, pull together:

  • Your Social Security number, legal name, date of birth, height, weight, phone, email, and mailing address
  • Your employer’s company name and the worksite address where the certificate will be used
  • Years and months of related practical experience and any school training
  • An employer recommendation letter on company letterhead (details below)
  • A credit card, debit card, check, or money order for the fee — cash is not accepted2Fire Department of New York. Certificates of Fitness
  • A government-issued photo ID (driver’s license, non-driver ID, or passport) for exam day3Fire Department of New York. General Notice of Exam – Certificate of Fitness

Each COF type has its own Notice of Examination and, in most cases, a study guide, both available on the FDNY website. Read them before you pay.

Step 1: Create Your NYC.ID and Open the Application

All COF applications and payments go through the FDNY Business portal, and the portal uses a NYC.ID login. If you don’t already have a NYC.ID from another city service, create one through the portal. Once you’re in, choose the Certificate of Fitness application option to begin.2Fire Department of New York. Certificates of Fitness

Step 2: Fill Out the Application

The online form mirrors the agency’s paper application. Enter your personal information, your employer’s name and worksite address, your related experience, and the specific COF type code for the exam you plan to take.

Everything you put on the form must be accurate. Knowingly submitting false information on this kind of written statement is a class A misdemeanor under New York Penal Law, carrying up to 364 days of imprisonment.4New York State Senate. Step 3: Get the Employer Recommendation Letter

Most original COF applications require a recommendation letter from your employer. It has to be printed on the employer’s official letterhead and signed by an officer or supervisor who includes their title. The letter must contain:

  • Your full name
  • How long the employer has known you
  • Your employment and training details, including duration
  • The building address where the certificate will be used
  • A character attestation covering your relevant work experience, training, and physical ability to perform the required duties6Fire Department, City of New York. Certificate of Fitness Application

If you don’t have an employer yet, you can still apply and take the exam. Passing gets you a temporary letter with your photo that you can show to prospective employers. The FDNY will not issue the actual Certificate of Fitness card, though, until an employer submits the required recommendation.6Fire Department, City of New York. Certificate of Fitness Application

Step 5: Take the Exam

Most COF exams are walk-in. FDNY Headquarters at 9 MetroTech Center, 1st Floor, Brooklyn, NY 11201 is open to the public Monday through Friday (except legal holidays) from 8:00 AM to 2:30 PM.2Fire Department of New York. A few certificate categories require a scheduled appointment instead of a walk-in: F-80, F-89, Q-01, S-56, and S-98. For these, you need your COF application ID before you can book an exam slot through the online scheduler.2Fire Department of New York. Certificates of Fitness

If you don’t pass, you’ll get a failure report. There is no formal waiting period before you can try again, but each attempt requires a brand-new application and the full fee.

If Your Application Is Incomplete

The FDNY will not accept an application that is missing required documentation or the fee. If your submission is deficient, you’ll receive notice and have 30 days to fix it. Original applications not corrected within that window are considered abandoned, and you’d have to start over with a new application and fee.7NYC Rules. 3 RCNY 113-01 – Certificates of Fitness and Certificates of Qualification

After You Pass

Applicants who pass the exam and have a valid employer recommendation on file receive their Certificate of Fitness card. The FDNY photographs you on exam day for the card.7NYC Rules. 3 RCNY 113-01 – Certificates of Fitness and Certificates of Qualification Most certificates are valid for three years from the date of issuance. You can track your application and card status through the FDNY Business portal using the same NYC.ID.

Why It’s Worth Getting Right the First Time

Working in a role that requires a COF without having one, or with an expired certificate, carries real financial consequences. Under the FDNY’s penalty schedule, a first violation draws a standard penalty of $750, rising to $1,000 if you fail to appear at the hearing. A second or subsequent violation within 18 months jumps to $1,875, with a maximum of $5,000 for a default.8NYC Rules. 3 RCNY 109-03 – Penalty Schedule for FDNY Summonses Those penalties can fall on both the individual and the business that allowed uncertified work to proceed, so apply well before your start date and keep the card on you once you have it.
