How to Fill Out and Submit Georgia Form T-200: License Plate Affidavit

Georgia Form T-200 is the Affidavit of Non-Receipt of an Original License Plate or Renewal Decal, and you use it when the Department of Revenue or your County Tag Office mailed you a plate or decal that never arrived. File the notarized affidavit along with a police report, a completed Form MV-7, and proof the item was issued to you, and under O.C.G.A. § 40-2-44 the replacement is free so long as you report the non-receipt within 90 days.1Justia Law. When Form T-200 Is the Right Form

T-200 covers one narrow situation: you registered a vehicle or renewed registration, a plate or decal was mailed to you, and it never showed up. Your signature on the form is a sworn statement that the item was lost in transit and never came into your possession.3Georgia Department of Revenue. It does not apply if the plate was stolen off your vehicle, if you lost it after receiving it, or if it was damaged while in your possession. Those situations go through the standard replacement process on Form MV-7 alone, which carries an $8.00 fee for standard plates.2Georgia Department of Revenue. Replace License Plate T-200 also has nothing to do with correcting a vehicle title; Georgia uses the separate T-11 affidavit for that.

What to Gather Before You Start

Three documents have to be in hand before the submission is complete. Missing any one of them will cause the package to be rejected.

A Police Report

Georgia law requires you to report the non-receipt to a law enforcement agency. Your local municipal or county police department, the sheriff, or the Department of Public Safety all qualify.1Justia Law. Georgia Code 40-2-44 – Reporting of Theft, Loss Ask the officer to record the plate number or decal number on the report. You will attach a copy to the T-200, and the agency cannot charge you for that copy.

A Completed Form MV-7

Form MV-7 is the Application for a Replacement License Plate (Tag) and/or Decal. Every T-200 submission has to include a signed MV-7.3Georgia Department of Revenue. Proof of Issuance

You also need a record showing the plate or decal was issued and mailed to you. Whichever office processed your registration — the County Tag Office or the state Motor Vehicle Division — can produce this. It confirms that the item existed and was sent to your address.

Filling Out the Form

The form is a single page. You can download it from the Georgia Department of Revenue or request a blank copy at any County Tag Office.4Georgia Department of Revenue. T-200 Affidavit of Non-Receipt of an Original License Plate/Renewal Decal

Vehicle Information

Enter the year and make of the vehicle and its full 17-character Vehicle Identification Number, found on the dashboard near the windshield or on the driver’s side door jamb. Below that, fill in the tag number or decal number you were assigned and the year it was issued. Match these entries to your registration paperwork.

Issuing Authority

Check one box to indicate whether the plate or decal was issued by your County Tag Office or by the state Motor Vehicle Division. If you registered in person at the tag office, that is the box. If the state MVD handled it directly, check the other. This routes the replacement to the right office.

The Sworn Statement

The body of the form is pre-printed language affirming that you applied for the plate or decal and never received it. Read it carefully before signing. The statement specifically references the penalty provisions under O.C.G.A. § 40-2-114 for filing a false report with the Revenue Commissioner. A first conviction carries a fine of $25 to $100. A second or subsequent conviction carries a fine of $25 to $500, up to 30 days in jail, or both.3Georgia Department of Revenue. Owner Signature and Address

Print your full legal name as the registered owner, then sign and date the form in front of the notary (not before). Enter your current street address, city, state, and zip code. The replacement will be mailed to this address, so check it twice.

Getting the Form Notarized

Form T-200 must be signed in the presence of a notary public or a justice of the peace. The notary will ask for a valid photo ID, watch you sign, and then apply their own signature, seal, and commission expiration date in the designated section at the bottom.3Georgia Department of Revenue. Many County Tag Offices keep a notary on staff, so notarization and submission can often happen in the same visit. Banks, UPS stores, and law offices also provide notary services, sometimes for a small fee.

Where to Submit the Package

Take the notarized T-200 to the office that originally issued the plate or decal, matching the box you checked on the form. Bring the full package:

  • The completed, notarized Form T-200
  • The completed, signed Form MV-7
  • A copy of the police report listing the plate or decal number
  • Proof the plate or decal was issued and mailed to you

In-person submission at the County Tag Office is the fastest route because staff can verify the documents on the spot. If you mail the package, send copies of the police report and issuance record rather than originals, and consider certified mail for delivery confirmation.

Fees and What to Expect After You Submit

When the plate or decal was genuinely lost in the mail before you ever received it, the replacement is issued free under O.C.G.A. § 40-2-44(c).1Justia Law. Georgia Code 40-2-44 – Reporting of Theft, Loss The no-fee replacement applies as long as the non-receipt is reported within 90 days of the issue date.2Georgia Department of Revenue. Replace License Plate Past 90 days, the standard replacement fee of $8.00 for regular plates (or $43.00 for special prestige plates) may apply. The police report copy is free, and the only other possible cost is a notary fee.

Once the issuing office accepts the package, the replacement is mailed to the address on the form. The Department of Revenue does not publish a guaranteed processing timeline, though standard replacements are typically fulfilled within a few weeks. If you submit in person, ask the clerk whether the office keeps plate inventory and can issue the replacement over the counter.

While you wait, you may be driving without a visible plate or current decal. Keep a copy of your T-200 submission and the police report in the vehicle. If you are pulled over, those documents show that the problem has been reported and a replacement is in progress.

Penalties for a False Affidavit

T-200 is a sworn statement, and Georgia treats false filings seriously. The form itself references O.C.G.A. § 40-2-114 for false reports to the Revenue Commissioner. Beyond that, knowingly submitting false information to a state agency can trigger prosecution under the broader false-statements law, O.C.G.A. § 16-10-20, which carries a fine of up to $1,000, one to five years of imprisonment, or both.5Justia Law. Georgia Code 16-10-20 – False Statements and Writings, Concealment of Facts, and Fraudulent Documents in Matters Within Jurisdiction of State or Political Subdivisions Filing a T-200 to avoid the $8.00 replacement fee on a plate you actually received is not worth the exposure.
