Start by contacting the DUI program where you were enrolled. You will either need to resume treatment or confirm that a prior completion is on record. If the evaluation referred you to a separate substance abuse treatment provider licensed by the Department of Children and Families, you must complete that treatment before the DUI program can certify completion on HSMV 77096. Submitting the Form and Paying Reinstatement Fees
Take the completed HSMV 77096 to your local county tax collector’s office, which handles driver license services for FLHSMV. The office scans the form and forwards it for verification.
You will owe reinstatement fees at the counter. FLHSMV charges a $130 administrative fee for alcohol- and drug-related offenses. Reinstatement following a DUI also requires an FR-44 certificate of financial responsibility from your auto insurer, which carries higher liability limits than a standard policy. Common Mistakes That Delay Reinstatement
