Indiana’s Form 54706 supervised driving log is the one-page BMV document where a learner’s permit holder records at least 50 hours of behind-the-wheel practice, including 10 hours at night, with a qualified supervising driver. You hand it in at your driving skills test appointment, and without it the BMV will not issue a probationary license.1Bureau of Motor Vehicles. Driver’s License – Section: Log of Supervised Driving Download it from the BMV website or pick one up at any license branch. The requirement applies to every applicant, regardless of age.
Who Can Sit in the Passenger Seat
Hours only count if the person riding with you qualifies under Indiana law, and the rules differ by age.2Indiana General Assembly. Indiana Code Title 9 Motor Vehicles 9-24-3-2.5
If you are under 18, your supervisor must be one of these:
- A relative at least 25 years old with a valid license (blood, marriage, legal guardian, or stepparent)
- Your spouse, at least 21, with a valid license
- A licensed driving instructor working through an approved driver training school, licensed under Indiana Code 9-27-6-8
- A certified driver rehabilitation specialist recognized by the BMV

Use blue or black ink and print legibly.3Indiana Bureau of Motor Vehicles. Indiana State Form 54706 Supervised Driving Log The form has three parts.
Header
At the top, enter the driver’s full name (last, first, middle initial) and the driver’s license number printed on the learner’s permit.3Indiana Bureau of Motor Vehicles. Indiana State Form 54706 Supervised Driving Log
The Driving Log Table
Each row records one practice session with three pieces of information: the date in month/day/year format, the total time driven in hours and minutes, and a mark in either the day or night column. The form does not have a space for the supervisor’s name or license number, so you only need those three entries per row.
If you run out of rows, the form tells you to attach additional sheets. Keep the same format on any extra pages so the totals are easy to add up.
Certification and Signature
The bottom of the form carries a certification that the driver completed 50 hours of supervised practice with at least 10 hours at night. If the driver is under 18, a parent or legal guardian signs. If the driver is 18 or older, only the driver signs.3Indiana Bureau of Motor Vehicles. Indiana State Form 54706 Supervised Driving Log The signature is a legal attestation that the entries are accurate, so review the log before signing.
If Your Permit Has a Daylight-Only Restriction
Some learner’s permits restrict driving to daylight hours. If yours does, you do not have to log the 10 nighttime hours, but you still need all 50 hours during the day.1Bureau of Motor Vehicles. Turning the Log In at Your Skills Test
You submit Form 54706 at the BMV branch when you arrive for your driving skills test. The skills test is by appointment, scheduled at least 48 hours and no more than three weeks ahead, and you should arrive at least 15 minutes early.4Bureau of Motor Vehicles. Driving (Skills) Test
Bring the following along with the completed log:
- Your valid Indiana learner’s permit
- Driver’s education proof of completion, if applicable
- A parent or guardian to sign for financial responsibility if you are under 18, or a licensed driver if you are 18 or older
- An insured, registered vehicle that passes inspection: working brake lights and turn signals, no cracked windshield, no spare tires, and all driver-assistance technology turned off4Bureau of Motor Vehicles. Driving (Skills) Test
A BMV representative checks the log for the 50-hour total, the 10 nighttime hours (unless you have the daylight-only exception), and the required signature. If anything is missing or the hours fall short, the application is denied until you fix the form. Once the examiner is ready for the test itself, no one else is allowed in the vehicle.
Falsifying Hours Is a Felony
Fabricating entries on the log is treated as application fraud. Making a false statement or committing fraud on a driver’s license or permit application, which includes the documents filed with it, is a Level 6 felony under Indiana law.5Indiana General Assembly. Indiana Code 9-24-18-2 – Misuse of Licenses and Permits A parent or guardian who knowingly signs off on hours that were not driven is attesting to false information too.
Practical Tips
Fifty hours works out to about 45 minutes a day over three months, or a handful of longer weekend sessions. A few habits make the paperwork easier:
- Log each session the day it happens. Reconstructing months of driving from memory the night before your BMV appointment leads to errors and rejected totals.
- Spread the nighttime hours across the practice period instead of saving them for the last week.
- Vary your routes. Highway merges, parking lots, residential streets, and unprotected left turns all build different skills, and the skills test will expose gaps.
- Photocopy or photograph the completed form before your appointment in case the original is lost or damaged.
