How to Fill Out and Submit LATTC Dynamic Forms: Student Petitions and Appeals

LATTC Dynamic Forms is the online system Los Angeles Trade-Technical College uses for petitions, appeals, and other administrative requests. You open the form from the Admissions Forms page or the Financial Aid office page, sign in through the LACCD student portal, fill in the fields, upload your supporting documents, sign electronically, and submit. Processing typically takes about five working days once everything is in correctly.1Los Angeles Trade-Technical College. Admissions Forms

Logging In Before You Open a Form

Every Dynamic Form routes through LACCD single sign-on before it opens. Your username is your nine-digit LACCD student ID (formatted like 881234567 or 901234567). If you have never changed your password, the default is 88@ followed by the first letter of your last name capitalized and your four-digit birthday in MMDD format.2Los Angeles Community College District. Reset your password at csprd.laccd.edu before you open a form. A failed login after you’ve started entering data can wipe what you’ve typed.

Picking the Right Form

Most of the Dynamic Forms students file at LATTC fall into one of four categories. Pick the one that matches your situation, because the supporting documents are different for each.

Residency Reclassification

If you were classified as a nonresident and believe you qualify for in-state tuition, submit a Supplementary Residency Questionnaire through the Admissions Forms page. You need to show you’ve been physically present in California for more than one year and one day and that you intend to make California your permanent home. Within LACCD, the request must be filed within 30 calendar days of receiving your nonresident notification and before your first semester of attendance.3Los Angeles City College. Residency

California College Promise Grant Appeal

The Promise Grant waives the $46-per-unit enrollment fee for eligible California residents. If you lost the waiver because your GPA dropped or your completion rate fell below the threshold, submit the Loss of California College Promise Grant Appeal form from the Financial Aid page. The appeal has to explain verifiable circumstances that caused the academic trouble, such as illness, an accident, or another situation beyond your control.4Los Angeles Trade-Technical College. Financial Aid

Satisfactory Academic Progress Appeal

To keep federal financial aid, LATTC requires you to hold at least a 2.0 cumulative GPA and complete at least 66.5% of all units you attempt. Withdrawals, incompletes, and failing grades count as non-completions.5Los Angeles Trade-Technical College. Financial Aid Appeals

If you’re on financial aid suspension, there’s an extra step most students don’t expect: before you can access the SAP appeal form, you have to complete the online GetSAP Workshop and pass it with a score of at least 70%. Save the completion certificate as a PDF, because you will upload it with the appeal.6Los Angeles Trade-Technical College. SAP Online Appeal Form

Course Repetition Petition

If you have received three substandard grades (D, F, NP, NCR, or W) in the same course across LACCD, you need to file an academic petition to enroll a fourth time. The petition asks you to describe extenuating circumstances — verified accidents, illness, or situations beyond your control — that justify another attempt. Approval is not automatic.7Los Angeles Trade-Technical College. LATTC Academic Petition

What to Have Ready Before You Open the Form

The single biggest cause of a stalled submission is opening the form before you have everything. Dynamic Forms sessions time out, and an idle form can wipe your entries. Pull the following together first:

  • Your nine-digit LACCD student ID and portal password.
  • Supporting evidence for your form type. Residency reclassification usually needs IRS tax transcripts and California identification documents. Appeals based on illness or injury need medical records or provider notes. SAP appeals need the GetSAP Workshop certificate.
  • A personal statement explaining why you’re filing. Draft it in a word processor or notes app first, then paste it in. Writing directly in the form risks losing the text if the session refreshes.
  • Digital copies of every document, saved as clear PDFs or image files. Blurry uploads are a common reason processing drags on.

For any financial aid form, confirm your current FAFSA is on file with LATTC (school code 001227) before you begin.

Filling Out and Signing the Form

Once the form opens, the fields are grouped into sections. Every required field is flagged if you try to advance without it. For anything that asks about your academic history, check course numbers and semester dates against your unofficial transcript in the SIS portal instead of relying on memory.

After the data sections, a review screen shows everything you entered. This is where you catch a mistyped student ID, a wrong course name, or an attachment you forgot to upload. Errors here can slow your petition or get it denied as incomplete.

The last step is the electronic signature. You click a signature box and type your name exactly as it appears in college records. The signature carries the same legal weight as a handwritten one and is a declaration that what you provided is truthful. When you click submit, the form and its attachments go to the relevant department.

When a Parent or Guardian Has to Co-Sign

Some forms, particularly supplemental enrollment applications for students under 18, need a parent or guardian signature too. Dynamic Forms calls that person a “co-signer.” When you finish your part, the system emails the address you entered. The parent creates their own Dynamic Forms account, reviews the form, and signs electronically.8Los Angeles City College. Dual Enrollment Application Process

Enter your parent’s email address, not your own. If the notification goes to the wrong place or never arrives, log back into your Dynamic Forms dashboard, select “Forms Awaiting Other Signatures,” and update the co-signer’s name or email. The system will resend the notification.

After You Submit

A confirmation email arrives at your LACCD student account (@student.laccd.edu, not a personal address) within a few minutes. If you don’t see it, check spam. The Dynamic Forms dashboard also shows every form you’ve submitted and its current status.

LATTC’s Admissions Forms page lists approximately five working days as the standard processing time.1Los Angeles Trade-Technical College. Admissions Forms Expect longer during the first two weeks of a semester, when volume spikes. SAP appeals can run longer still, because financial aid counselors sometimes come back with a request for additional documentation. Check the dashboard yourself rather than waiting on email; some status updates post there before a formal notification goes out.

If Your Petition Is Denied

A denial notification will state the reason. The most common ones are missing or insufficient documentation, or circumstances that don’t meet the criteria for that specific petition. In many cases you can resubmit with stronger evidence or a fuller explanation. For a SAP appeal, a denial means your financial aid stays suspended until you either bring your GPA and completion rate back into compliance on your own or appeal successfully in a later review period. For residency reclassification, a denial means you pay nonresident tuition for that semester, but you can refile for the following term with additional proof of California domicile.

Avoiding Lost Work

Dynamic Forms runs in your browser, and session timeouts cause most of the frustration students report. A few habits prevent it:

  • Use a current version of Chrome or Edge. Safari and older Firefox versions sometimes cause display or submission errors.
  • Don’t leave the form idle. Walking away for 15 or 20 minutes can expire the session and clear your entries.
  • Draft long text answers in a separate document and paste them in. A page refresh will not take your statement with it.
  • Disable pop-up blockers for the LACCD domain. Some forms open in new windows that aggressive blockers will stop from loading.
