How to Fill Out and Submit Missouri Form 2519: Fees and Filing

Missouri Form 2519 is the one-page request you file with the Department of Revenue to obtain a certified receipt of your vehicle’s title or registration when the original document has been lost, destroyed, mutilated, or stolen. It costs $8.50 per receipt plus a $9 processing fee, must be notarized before you submit it, and can be filed by mail or in person at any Missouri license office.¹

A title receipt is not the same as a duplicate certificate of title. It is a certified record of what the state has on file, useful as proof of ownership or for verifying lien and registration details. If you actually need a replacement title (the document you sign to transfer ownership), you want the Application for Missouri Title and License, Form 108, instead.¹

When Form 2519 Is the Right Form

The form asks you to check one of four reasons for the request: destroyed, lost, mutilated, or stolen.¹ Typical situations include proving ownership when the physical title is unavailable, giving a lender verification of lien information, or confirming registration details after a records issue. You can request a title receipt, a registration receipt, or both, but each receipt carries its own $8.50 fee.

One boundary worth noting: if you moved to Missouri and your out-of-state lender is holding your title, Form 2519 will not help you. That situation runs through Form 5834 and Form 108 and has its own set of requirements.²

Information to Gather Before You Start

Have the following ready before you fill in the form:

Vehicle identification number (VIN). For watercraft, the hull identification number (HIN); for outboard motors, the outboard motor identification number (OBIN).
Year and make as recorded on your previous title or registration.
Kind of vehicle, meaning the body type or classification (sedan, truck, motorcycle, and so on).
Plate number and expiration year from your current or most recent Missouri registration.
Title number if you have it. Including it speeds up the search on the Department’s end.
Owner’s legal name, phone number, and current address exactly as they appear on the Missouri title record.

If you plan to pay by credit or debit card, keep the card number and expiration date within reach as well.¹

Filling Out the Form

Download the form at dor.mo.gov/forms/2519.pdf, or pick up a blank copy at any Missouri license office. It fits on a single page.

Vehicle and Owner Information

Enter the vehicle details first: year, make, kind of vehicle, plate number, expiration year, title number, and VIN (or HIN or OBIN for watercraft and outboard motors). Below that, write the owner’s legal name exactly as it appears on the existing Missouri title record, with a phone number and current mailing address. Mismatches between what you write and what the Department has on file slow the request down, so cross-check the name against any older paperwork you still have.

Type of Request and Reason

Check the box for a Title Receipt or a Registration Receipt (or both, if you need both), then mark the reason: destroyed, lost, mutilated, or stolen. The form does not ask for a written explanation. The one-word reason is all it wants.¹

Alternate Delivery Address

By default, the Department sends the receipt to the record holder’s address. To have it go somewhere else, such as a lender, an attorney, or a different personal address, mark “Yes” in the alternate address section and choose your delivery method. The options are mail to a different address, fax (add $0.50 per page), or email.¹ You can also request a certified record if you need one with an official stamp for legal or financial purposes.

Signature and Notarization

Sign and date the form, then have it notarized. This is not optional. The Department will not process an un-notarized Form 2519.¹ The notary section calls for the notary’s signature, printed name, commission expiration date, and the state and county where the notarization takes place. If you file at a license office, notary service is available on site for $2.

What It Costs

The base fee is $8.50 for each receipt plus a flat $9 processing fee. Requesting both a title receipt and a registration receipt means $8.50 twice plus the single $9 processing fee, for a total of $26.¹

Add-ons where they apply:

Credit or debit card payments carry a convenience fee of 2% plus $0.25 per transaction.
Notary service at a license office is $2.
Fax delivery is $0.50 per page.

Where to File

Once the form is completed and notarized, you have two options:

By mail. Send the form and payment to Motor Vehicle Bureau, PO Box 2046, Jefferson City, MO 65105-0100. Include a check or money order payable to the Missouri Department of Revenue, or fill in the credit or debit card section on the form itself.³
In person. Bring the form to any Missouri license office. Staff can notarize it on site for $2 if you have not already had it notarized, and you can pay by card or check at the counter.¹

The Department does not process vehicle titling transactions over the phone, so requesting a receipt by calling in is not an option.³

After You Submit

Once the Department validates the request, the receipt goes to the address on the form (or the alternate address you specified) by mail, fax, or email, based on what you selected. Mail submissions to Jefferson City generally take longer than in-person filings, where staff can often process the request during your visit if the record is accessible in the system. If a check is returned for insufficient funds, the Department may electronically resubmit it, so make sure the payment clears.¹
