How to Fill Out and Submit Missouri Form 5805: Address Change Request

Missouri Form 5805 is the one-page Address Change Request you send to the Missouri Department of Revenue to update your address on individual income tax and motor vehicle records.1Missouri Department of Revenue. It asks for your name, date of birth, the last four digits of your Social Security number, and your new address, and you sign it under penalty of perjury.

What the Form Covers

One submission can update individual income tax records, motor vehicle records, or both. Form 5805 is not for businesses. How to Fill It Out

  • Enter your first, middle, and last name exactly as they appear on your Missouri tax account.
  • Put your date of birth in MM/DD/YYYY format.
  • Give only the last four digits of your Social Security number. The form does not ask for the full number.
  • Include a daytime phone number and email so the Department can reach you if something doesn’t match its records.
  • The old address field is optional. Filling it in can help the Department find your existing records faster; leaving it blank won’t hold up processing.
  • Provide the full new mailing address, including city, state, ZIP code, and county.
  • Check whether you’re updating individual income tax records, motor vehicle records, or both.
  • If you checked motor vehicle records, list every plate, disabled placard, and boat or outboard motor title number you want moved to the new address. This covers passenger cars, trucks, RVs, motorcycles, and trailers.
  • Sign and date the form. The signature line carries a perjury declaration; you’re affirming the information is true and correct.

If you use the paper form, complete it in black or blue ink. The Department scans submissions, and lighter ink can cause processing errors.

Keep a copy before you drop it in the mail. The Department does not typically send a confirmation letter after processing an address change, so your copy is your proof of submission.

After You Submit

Once the change is processed, future tax forms, refund checks, and assessment notices go to the new address. You can confirm the update by logging into the state’s online tax system or by calling the Department after a reasonable processing window.

Timing matters. If the Department mails a notice of deficiency or other official correspondence before your change takes effect, that notice is still legally valid even when it goes to your old address. Form 5805 Does Not Update the IRS

Changing your address with Missouri has no effect on your federal tax records. To update your address with the IRS, file federal Form 8822 (Change of Address), which the IRS says generally takes four to six weeks to process.6Internal Revenue Service.
