Handle Two Registrations Before You Download the Form

The first is your New Jersey Division of Revenue registration. File Form NJ-REG with the Division of Revenue in the Department of the Treasury, then request a Business Registration Certificate once the filing is processed. The second is a Federal Employer Identification Number. You need a certified IRS document showing your entity’s name, address, and FEIN. If you don’t already have one, apply through the IRS. The FEIN is nine digits; the MVC EIN you’re applying for is fifteen. They are two different numbers on two different records.
Documents to Attach

- A certified IRS document showing the entity’s legal name, current address, and FEIN. The CP 575 notice issued when the FEIN was assigned works, as does an IRS 147C verification letter.
- Your formation document: an NJ Certificate of Formation for an LLC, an NJ Certificate of Incorporation for a corporation, or an NJ Business Registration/Trade Name Certificate for a trade name. These come from the NJ Division of Commercial Recording.
- The Business Registration Certificate from the Division of Revenue.
- A copy of any Certificate of Authority your entity holds from another state, showing that state’s tax identification number.
- An original or certified authorization letter or power of attorney if the person signing the form isn’t already named on the corporate documents. Licensed motor vehicle dealers may use an authorized signatory card in place of that letter.
- A color photocopy of the signer’s driver’s license or ID if it was issued outside New Jersey.
Lienholders have a shorter list. Banks submit their Banking Commission documents and credit unions submit their charter papers, along with the IRS document and any authorization or ID materials. Filling Out the Form

The single most important field is the legal business name. Enter it exactly as it appears on the IRS document and the formation certificate. A mismatch as small as “LLC” on one paper and “L.L.C.” on another, or a missing comma before the entity suffix, is one of the most common reasons an application comes back. If the two documents don’t already match, fix them with the issuing agencies before you apply.

Check the box for the entity type that applies to your business, whether corporation, LLC, trade name, or other, so the MVC records the EIN under the right classification.
An authorized representative signs and dates the form at the bottom. That signer has to be someone already on the corporate documents or the person named in the authorization letter or power of attorney you’re including. The signature certifies the information is accurate.
How to Submit It
You have two ways to send the completed form and documents to the Business Licensing Services Bureau.

What Happens After You Send It
Initial EIN requests take three to five business days. Updates to an existing EIN, like an address or name change, are faster, generally one to two business days.2New Jersey Motor Vehicle Commission. When the MVC approves the request, you get a 15-digit Entity Identification Number. Updating an Address or Legal Name
If the business moves or changes its legal name, you use the same BA-8 to update the record. The form has separate sections for each type of change.
For an address change, include an updated certified IRS document showing the new address and the FEIN, a power of attorney if the applicant isn’t named on the corporate documents, and a color photocopy of the requestor’s photo ID.
