NYC HRA Form W-700D, the FIA School/Training Enrollment Letter, is the four-page document Cash Assistance and SNAP recipients use to verify enrollment in an approved school or training program. You fill out the first section with your HRA caseworker, your school’s authorized representative fills out the rest, and the completed form goes back to HRA so your participation is on file and your carfare, childcare, and other training-related reimbursements can be processed.1NYC Human Resources Administration. FIA School/Training Enrollment Letter
Without a completed W-700D on file, HRA has no way to verify that you are attending an approved program, which puts both your benefits and any expense reimbursements at risk. The form also authorizes your school to share attendance, academic progress, and employment information with your caseworker.
Who Needs To File It
You need Form W-700D if you receive Cash Assistance or SNAP and are enrolling in or already attending an HRA-approved school or training program. That includes college courses, vocational training, and other structured programs HRA recognizes as qualifying work activities. The form covers both in-person and distance learning, and there is a specific field asking which one applies.1NYC Human Resources Administration. FIA School/Training Enrollment Letter
Two people complete different parts. You handle Section I at your Job Center or Benefits Access Center with your HRA representative. Your school’s registrar, program coordinator, or financial aid officer handles Section II. Neither part is optional.
Section I: What You Fill Out
Section I is the student portion. Start with your name, case number, and case name, exactly as they appear on your HRA correspondence.
The form then asks about out-of-pocket costs. Write in your daily carfare, meaning what you spend on transportation each day you attend class. HRA uses that number to calculate your reimbursement. If you need a specific item to participate in the program, like a uniform, safety equipment, or tools, mark “Yes,” describe it, give the cost, and attach a receipt or bill. If nothing applies, mark “No.”1NYC Human Resources Administration. FIA School/Training Enrollment Letter
Overpayment Recovery Agreement
You have to sign a mandatory agreement choosing how HRA recovers any overpayment of engagement expenses. The two options are deduction from your Cash Assistance grant, or recovery from future carfare and childcare payments. Pick one, sign, and date. Skipping this holds up the whole form.1NYC Human Resources Administration. FIA School/Training Enrollment Letter
Release Authorization
Section I ends with a notice about how educational grants and scholarships affect your benefits, followed by your signature authorizing your school to release attendance and progress information to HRA. The release is what lets your caseworker confirm you are actively participating.
Section II: What Your School Fills Out
Once Section I is done, bring the form to your school. The authorized representative completes the rest.
Enrollment Details
The representative enters your student ID, the school or program name, your course of study or major, semester start and end dates, your enrollment start date, and whether the program is graduate level. They also provide internal vendor and skill codes HRA uses to track approved programs. If you are re-enrolling, they must indicate whether you maintained at least a “C” average.1NYC Human Resources Administration. FIA School/Training Enrollment Letter
Weekly Activity Schedule
This is the most detailed part of Section II. The school fills in a grid showing your hours for every day of the week using these codes:
- CL for classroom hours
- LAB for laboratory hours
- FWS for Federal Work Study hours
- INT for internship or externship hours
- SH for supervised homework hours
The representative then totals your weekly classroom and lab hours, internship hours, Federal Work Study hours, and supervised and unsupervised homework time. HRA uses these totals to confirm you are meeting the activity-hour requirements for your Cash Assistance case. A school stamp is required in this section.1NYC Human Resources Administration. School-Paid Expenses and Educational Income
The representative records whether the school directly pays you for carfare or childcare, including the weekly amount and funding source. They also break down all non–Title IV educational grants, loans, and scholarships, itemized across categories including tuition, loan origination fees, books, meals, transportation, supplies, living expenses, and any funding from the SEEK Program or College Discovery Program. HRA uses this breakdown to figure out which portions count toward your SNAP budget and which are excluded.1NYC Human Resources Administration. FIA School/Training Enrollment Letter
Finally, the school representative prints their name, signs, dates the form, and provides a phone number and email address.
How To Submit the Completed Form
Once both sections are filled out, get the form back to HRA. You have three options:
- In person at your assigned Job Center or Benefits Access Center. Ask for a date-stamped copy as proof.
- Through your ACCESS HRA account online, using the document submission feature to upload a scan or clear photos.2NYC Human Resources Administration. Access HRA Frequently Asked Questions
- Through the ACCESS HRA mobile app, which lets you photograph and upload from your phone.3NYC Human Resources Administration. ACCESS HRA Mobile
If you upload digitally, make sure every page is legible. The form runs four pages, and a blurry signature or a cut-off schedule grid will trigger a resubmission request. What Typically Delays Processing
Most problems with the W-700D come from missing pieces rather than wrong answers. Common ones:
- The school representative forgets the stamp in the weekly activity section.
- You skip the overpayment recovery agreement signature.
- Part of the weekly schedule grid is left blank.
- You submit Section I before the school has completed Section II. HRA cannot process a half-done form; both parts need to arrive together.
- The semester start date on the form doesn’t match what your school has on file.
Any one of these will bounce the form back and delay reimbursements.
When To File a New One
You generally need to submit a new W-700D each semester or whenever your enrollment status changes meaningfully. That includes switching programs, moving between full-time and part-time, or adding an internship. Keeping the form current prevents gaps in your carfare and childcare reimbursements and avoids compliance issues on your case.
If Your Benefits Change Based on the Form
If HRA reduces or terminates your benefits based on information from your W-700D, or denies a reimbursement you expected, you can request a fair hearing through the New York State Office of Temporary and Disability Assistance.
