Ohio BMV 4856 is the form carriers use to prove they have an established place of business in Ohio so the state can serve as the base jurisdiction for their International Registration Plan account. It is not the vehicle registration application itself — that is BMV 4890. Form 4856 does one job: it documents that your business has a real, physical presence in Ohio. You file it when opening a new IRP account, when your Ohio physical address changes, or when your Federal Employer Identification Number changes.
When You File BMV 4856
Three situations trigger it. Opening a brand-new IRP account: 4856 goes in with the BMV 4890 packet and the vehicle support documents. Moving your Ohio physical address: 4856 goes in with updated documentation so the IRP Central Processing Center can complete a fleet update supplement and issue a revised invoice. Changing your Federal Tax Identification Number, usually because you restructured the business: a new IRP account application is required, and 4856 is part of it.1Ohio Bureau of Motor Vehicles. International Registration Plan (IRP)
If you are only updating a physical address, you do not need to resubmit the full BMV 4890 packet. 4856 and the supporting documentation are enough for Ohio to process the change.
The form does not register individual vehicles and does not record mileage. Those are handled by BMV 4839 (Distance and Weight Schedule) and BMV 4825 (Fleet Management Application). Think of 4856 as the gatekeeping document. Until Ohio accepts your base jurisdiction proof, nothing else moves.
Section One: Entity Information
Section One asks for your legal entity name, your Ohio physical address (this becomes your established place of business address on file), and your Federal Identification Number. You must attach a copy of IRS correspondence confirming the issuance or confirmation of the FEIN assigned to the entity.2Ohio Bureau of Motor Vehicles. International Registration Plan Establishing Base Jurisdiction This means the letter the IRS sent when it assigned your EIN, not a printout from an online lookup. If you have lost it, request a replacement confirmation from the IRS before you file.
Section Two: Proving Your Established Place of Business
Section Two is where most 4856 submissions stall. Ohio’s EPOB documentation requirements go well beyond a utility bill, and every item has to be present.
- Property ownership or lease proof. If you own the location, Ohio may confirm it through the County Auditor’s Office. If you lease, attach a copy of the commercial lease between your entity and the property owner.
- Hours of operation and staffing. List the hours the EPOB is open during regular business hours and the number of employees present. The form shows the expected format: “5 employees M-F 8:00am–4:00 PM.”
- Ohio IT4 certificates and withholding registration. Provide a copy of the Ohio IT4 Employee’s Withholding Exemption Certificate for each employee listed, plus proof of your entity’s Ohio Withholding Registration number. Employees cannot be independent contractors. They must perform substantial duties connected to the trucking or commercial motor vehicle business, not only handle registration paperwork, and they must be accessible to the public during the posted hours.
- Photographs. Submit photos of the office directory, the interior offices, a street view showing your on-site truck yard, maintenance facilities, and permanent exterior signage. Temporary vinyl or paper signs may not be accepted. The photos need to show commercial vehicle activity actually taking place at the location.
Ohio is looking for evidence of a functioning trucking operation, not a mailbox service or a shared office rented for registration purposes.2Ohio Bureau of Motor Vehicles. Section Four: Records Affirmation
Section Four is a signed affirmation that the operational, employment, and maintenance records for your trucking business are kept at the physical address you listed, and that those records will be made available at your Ohio EPOB or Ohio residency address for audit purposes.2Ohio Bureau of Motor Vehicles. International Registration Plan Establishing Base Jurisdiction IRP auditors do request records on-site. Signing creates an enforceable commitment, so make sure the address you list is the address where the records actually live.
What You Submit Alongside BMV 4856 for a New Account
For a new IRP account, 4856 is one document in a larger packet. Ohio also expects the following, depending on your situation:1Ohio Bureau of Motor Vehicles. International Registration Plan (IRP)
- BMV 4890, the IRP New Account Application Packet.
- PUC 3422, Declaration of Knowledge for Commercial Vehicle Registration.
- Vehicle titles, front and back, for all owned and leased vehicles.
- IRS Form 2290, Schedule 1, for any vehicle with a combined gross weight of 55,000 pounds or more that you have owned for more than 60 days, showing the Heavy Highway Vehicle Use Tax has been paid.4Internal Revenue Service. Instructions for Form 2290
- BMV 4885, Motor Carrier Responsible for Safety Statement, if the vehicle operates under another entity’s interstate authority.
- BMV 4845, Owner’s Authorization to Lessee, or a Power of Attorney, for all leased vehicles.
- Proof of liability insurance that meets federal minimums. Most for-hire property carriers over 10,001 pounds need $750,000, rising to $5,000,000 for carriers of explosives or certain hazardous materials.5Federal Motor Carrier Safety Administration. Insurance Filing Requirements
- A USDOT number, which every interstate commercial motor carrier needs as its unique identifier for safety monitoring, audits, and inspections.6Federal Motor Carrier Safety Administration. Do I Need a USDOT Number?
Where to Send It
Ohio accepts BMV 4856 by mail or online. The online system is OHCORS 2.0, the Ohio Commercial Registration Online System, at irp.bmv.dps.ohio.gov. Electronic submission flags basic clerical errors in real time and generally moves faster than paper.
For paper submissions, Ohio uses two addresses depending on how you ship:
- U.S. Mail: IRP Processing Center, P.O. Box 18320, Columbus, OH 43218-0320.
- Express mail or courier: IRP Processing Center, 1970 W Broad St, Columbus, OH 43223.
The IRP Processing Center is open Monday through Friday, 8:30 a.m. to 3:30 p.m. Phone: 1-800-IRP-0007 or 614-777-8400. Email: OHIRP@dps.ohio.gov. Fax: 614-974-2118.1Ohio Bureau of Motor Vehicles. International Registration Plan (IRP)
What Happens Once Ohio Accepts Your 4856
Once your base jurisdiction documentation is accepted and the rest of the packet is in order, Ohio generates an invoice covering the apportioned registration fees owed across every jurisdiction your vehicles will operate in. After payment, Ohio issues one apportioned license plate and a cab card for each registered vehicle. The cab card lists every jurisdiction the vehicle is authorized to operate in and must be carried in the vehicle at all times.7International Registration Plan, Inc. International Registration Plan
If a vehicle needs to be on the road before the plate arrives, you can request a 45-Day Temporary Apportionment Authorization as part of your invoice. The TA lists the jurisdictions you are authorized to travel in without a physical plate installed, and a legible copy, physical or electronic, must be available to enforcement personnel. For a vehicle that needs immediate travel in Ohio only and has no apportionment yet, a 72-hour trip permit is also available.1Ohio Bureau of Motor Vehicles. International Registration Plan (IRP)
Any later change that affects your Ohio physical address or your FEIN sends you back to 4856. Relocate the office, or restructure into a new entity with a new tax ID, and the base jurisdiction form has to be refiled with fresh EPOB documentation before anything else on the fleet can update.
