Ohio BMV Form 4809 is a mail-in application vehicle owners use to request a duplicate registration card, transfer an existing registration to a different vehicle they own, or replace lost or damaged license plates and validation stickers. One thing the form does not do: it does not tell the BMV you sold a vehicle. Under Ohio law, when you sell a car its registration expires automatically and you remove your plates.2Ohio Legislative Service Commission. What Form 4809 Covers

  • Duplicate registration with validation sticker, for a lost or damaged card. You can keep your current registration number or take a new one.
  • Duplicate registration card only, without a new validation sticker.
  • Transfer, which moves the unexpired portion of your existing registration from a vehicle you no longer own to a replacement vehicle titled in your name.
  • Replacement plates, when your current plates are lost, stolen, or damaged.

The transfer option is the one most people reach for after selling or trading in a car and buying another. It does not transfer ownership to a buyer. It transfers your registration to your next vehicle.

Information to Gather Before You Start

Have these ready before you sit down with the form:

  • Your full legal name exactly as it appears on the certificate of title.
  • Your Social Security number, Ohio driver license number, or federal tax ID (whichever applies).
  • Your current Ohio street address, city, and zip code.
  • Vehicle details: year, make, model, and the seventeen-digit VIN. For a transfer, you need these for both the old vehicle and the new one.
  • The current Ohio license plate number.

Filling Out the Form

Type or print every entry in ink. The form’s own instructions require it, and handwritten cursive can slow processing or get the application returned.1Ohio Department of Public Safety Bureau of Motor Vehicles. Check the box for the transaction you need. If you want to keep your existing plate number on a duplicate registration, mark the “Retain Current Number” option. Fill in your owner information at the top: name, identification number, and Ohio address. Enter the vehicle information (year, make, model, VIN, plate number) in the designated section, and if you are transferring registration, add the details of the new vehicle receiving it. Sign the form in the owner block.

The signature has to be that of the owner or owners as named on the certificate of title. Fees

Enclose payment with the form. Make your check or money order payable to the Ohio Treasurer of State. The BMV does not accept cash by mail.   • Duplicate registration, with or without validation sticker: $9.00
  • Registration transfer: $9.00
  • Replacement plates: $15.00
  • Second plate (optional): add $1.25 for two plates instead of one
  • Retain current plate number: add $10.00

The same schedule applies across passenger cars, motorcycles, commercial trucks, non-commercial trucks, and trailers. When you transfer registration from a passenger vehicle to a non-commercial truck, additional fees beyond the base $9.00 may apply. Postage is charged at current rates.

Processing Time

Allow roughly four weeks from the date the BMV receives your application.3Ohio Department of Public Safety Bureau of Motor Vehicles. Keep a copy of the completed form and note the check or money order number so you can follow up if four weeks pass with no response.

Using the Transfer Option After Selling a Car

When you sell or otherwise transfer ownership of a motor vehicle in Ohio, the vehicle’s registration expires immediately and you are required to remove your license plates.2Ohio Legislative Service Commission. If you buy a replacement vehicle within the same registration period, Form 4809’s transfer option moves the unexpired portion of your old registration onto the new vehicle. The BMV calculates the difference in registration tax between the two vehicles and collects or credits accordingly. You have up to thirty days to complete the transfer, and during that window you can legally display your old plates on the new vehicle on Ohio roads.2Ohio Legislative Service Commission. Extra Documents for Special Situations

Leased Vehicles

If the vehicle in your 4809 transaction is leased, include two extra documents: a Power of Attorney form and a copy of the lease agreement signed by the leasing company. Emissions Testing Counties

If you live in Cuyahoga, Geauga, Lake, Lorain, Medina, Portage, or Summit county, there is an added step. Gasoline vehicles between six and twenty-five years old, and hybrid vehicles between seven and twenty-five years old, have to pass an emissions test before the registration transaction can go through. First-Time Non-Passenger Registrations
