To replace a lost or destroyed Oklahoma vehicle title, you file Oklahoma Form 701-7, the Application for Replacement Certificate of Title for Vehicle, Boat, or Motor, with Service Oklahoma or a licensed operator. The fee is $11 for a vehicle and $2.25 for a boat or outboard motor, the form must be notarized, and mail processing typically takes four to eight weeks.
When Form 701-7 Is the Right Form
Use Form 701-7 whenever your original Oklahoma certificate of title has been lost, misplaced, or destroyed and you need a new one issued. Under 47 O.S. ยง 1108, Service Oklahoma or one of its licensed operators may issue a duplicate once the loss is “accounted for to the satisfaction of” the agency.1New York Codes, Rules and Regulations. Oklahoma Code 47-1108 Once the duplicate is issued, every earlier certificate for that vehicle becomes invalid.
One boundary to know before you apply: if there is an active lien on record, the duplicate title is mailed to the lienholder, not to you. Check with your lender or Service Oklahoma first if you are unsure whether a lien is still recorded.
What to Have Ready
Pull these together before you sit down with the form:
- The VIN for a vehicle or the HIN for a boat. This is the single most important entry on the application.
- Your current tag number, registration decal number, and decal expiration month and year. Current Oklahoma registration is required unless you have moved out of state.2Service Oklahoma. Application for Replacement Certificate of Title for Vehicle/Boat/Motor
- Model year, make, and color. The form gives you a fixed list of 24 colors to choose from.
- The title number if you have it. The form treats this field as optional.
- A notary public. Your signature has to be notarized, so banks, shipping stores, and tag agencies are common stops.
If your Oklahoma registration has lapsed and you still live in the state, you have to pay all delinquent registration fees and penalties and submit an Oklahoma insurance verification form before the duplicate title will be issued.3Service Oklahoma. How Do I Apply for a Duplicate Title?
Filling Out the Form
Form 701-7 is a single page, downloadable as a PDF from Service Oklahoma. It has three parts.
Vehicle, Boat, or Motor Details
Enter the model year and make, then the VIN or HIN, followed by the tag or vessel/motor number, registration decal number, decal expiration date, and color from the approved list. Add the title number if you know it. Leaving that blank will not cause a rejection.
Owner Information
Enter the record owner’s full legal name exactly as it appears on the existing title record, along with your mailing address, daytime phone number, and email. If you have moved out of Oklahoma, list your new state of residency in the field at the top of this section.2Service Oklahoma. Application for Replacement Certificate of Title for Vehicle/Boat/Motor The form also asks whether you want the title held electronically by the state. Choose “Yes” and Service Oklahoma stores it digitally instead of mailing you a paper document.
Affidavit and Notarization
By signing, you affirm under oath that you are the lawful owner and that your certificate of title has been lost or destroyed. The form warns that a false statement may subject you to prosecution.4Service Oklahoma. Application for Replacement Certificate of Title for Vehicle/Boat/Motor Do not sign the form beforehand. The notary has to watch you sign, then complete the notary block (state, county, date, commission expiration) and apply their seal.
If someone else is filing for you, a notarized power of attorney, either the original or a certified copy, must accompany the application.3Service Oklahoma. How Do I Apply for a Duplicate Title?

Service Oklahoma notes that mailing straight to the agency is not faster than going through a licensed operator, because of the volume of mail the office handles.
How Long It Takes
Mail submissions generally take four to eight weeks when the form is filled out correctly. In-person submissions through a licensed operator run about three to eight weeks and can vary with the office and the condition of the vehicle’s record in the system. The most common causes of delay are avoidable: a missing notary seal, a blank VIN, or a lapsed Oklahoma registration the applicant did not realize needed to be cleared first.
If You No Longer Live in Oklahoma
You can still apply for a replacement Oklahoma title from out of state. List your current state of residency on the form and include proof of that residency: an out-of-state vehicle registration, an out-of-state driver license in the record owner’s name, a utility bill, or a rental agreement.3Service Oklahoma.
