The ASU Parent Review Form is a financial aid verification document that a parent completes through My ASU Guest Access so Arizona State University can confirm household income and recalculate the student’s aid eligibility.1Arizona State University. 2026-2027 Parent Review Form If it has appeared in the student’s My ASU Priority Tasks, the financial aid package is on hold until both the student and the parent finish their parts and upload supporting documents.
Why ASU Is Asking for It
Federal rules require schools to verify FAFSA information when the Department of Education flags an application, and ASU can also select a student for verification on its own.2eCFR. 34 CFR 668.54 – Selection of an Applicants FAFSA Information for Verification The Parent Review Form is how ASU collects the parent-side information needed to complete that check.
Common triggers are a recent divorce or legal separation, a significant drop in income from job loss or reduced hours, or a one-time financial event such as a large medical expense. The form may also appear when figures brought in automatically through the FAFSA Data Direct Exchange don’t match what the parent reported manually. Knowing which trigger applies to your situation matters, because the form branches from there and the documents ASU wants depend on it.
What to Gather Before You Open the Form
Pulling documents together first prevents incomplete submissions and a second round of requests. ASU’s review form supplement lists examples by scenario.3Arizona State University. What is the Review Form Supplement?
For a divorce or separation, have a copy of the divorce decree or separation agreement (or a written statement with the separation date if there is no formal agreement), the signed federal tax return and W-2s for the applicable tax year, and a detailed letter listing everyone currently in the household along with their relationships and ages.4Arizona State University. For a drop in income, have the parent’s last pay stub, any unemployment or severance documentation, and a detailed letter that gives the date the income changed, the prior employer’s name and contact information, and an estimate of total income for the current calendar year.4Arizona State University. ASU Parent Review Form ASU treats a reduction as significant when it is roughly 25 percent or more of total income and lasts at least eight weeks. Smaller or shorter drops can still require the form, but the explanatory letter does more of the work.
Other situations call for whatever documents fit: paid receipts, a death certificate, survivor benefits documentation, a signed letter from a childcare provider, or a signed personal letter.3Arizona State University. What is the Review Form Supplement?
How to Get Into the Form
The student starts the process. In My ASU, open Priority Tasks and click the Parent Review link to submit the initial eForm.1Arizona State University. 2026-2027 Parent Review Form Only after that does the parent’s matching task appear in their Guest Access account. If the parent’s task isn’t showing, check that the student’s portion has actually been submitted.
If the parent doesn’t have Guest Access yet, set it up before starting. The student logs in to My ASU, opens the Profile tab, selects Parent Guest Access, and enters the parent’s email address.5Arizona State University. Parent Guest Access The parent gets an email invitation to create a guest account. Once it’s active, the student has to enable sharing for financial aid specifically; without that, the parent cannot see the form. Students can change or revoke access at any time from the same page.
Current-year forms are also posted on ASU’s financial aid forms page if you want to confirm you have the right document.6Arizona State University. Financial Aid Forms
Filling It Out
The form opens with the student’s identifying information: full legal name, 10-digit ASU ID, local address, and daytime phone number.4Arizona State University. ASU Parent Review Form These are the student’s details, not the parent’s. Mixing them up is a common error that delays processing.
From there, the form branches by reason. A divorce or separation review asks for the current household members (names, relationships, ages), the assets reported on the FAFSA, and the date of separation. An income-reduction review asks for an estimate of total income for the current calendar year, broken into earned income from work, alimony received, child support received, taxable Social Security benefits, and other taxable income such as rental income or capital gains. You provide those figures inside the detailed letter rather than typing them into dedicated fields on the form.
The form also asks whether the parent filed a federal tax return for the applicable tax year or was not required to file. That answer feeds the Student Aid Index calculation; a dependent student whose parents were not required to file can be assigned an SAI as low as negative 1,500.7Federal Student Aid. Student Aid Index (SAI) and Pell Grant Eligibility Answer it accurately, with documentation to match.
The parent signs electronically through Guest Access by completing a checklist and signing the eForm.8Arizona State University. 2024-2025 Parent Social Security Number Verification
Submitting and Processing Time
The preferred route is to complete the eForm online and upload supporting documents directly through the Priority Tasks list in My ASU.1Arizona State University. 2026-2027 Parent Review Form If you need to send documents outside the portal, ASU accepts them by fax at each campus and by phone through the financial aid office at 1-855-278-5080; the Tempe fax line is 480-965-9242.9Arizona State University. Contact Financial Aid and Student Accounts Faxed or mailed paperwork takes longer because staff have to digitize it.
Submissions can take up to two business days to show as received in My ASU.6Arizona State University. Financial Aid Forms Review doesn’t start until the full financial aid file is complete, so if ASU is still waiting on other documents, the Parent Review Form alone won’t move the clock forward.1Arizona State University. 2026-2027 Parent Review Form
Deadline and What Happens If You Miss It
There is no single date printed on the form. The functional deadline is the student’s last day of attendance for the semester: ASU needs a valid FAFSA and all verification documents in by then for the student to be considered for campus-based funds, institutional grants, Pell Grants, and Direct Loans.10Arizona State University. What Are the Deadlines to Apply for Financial Aid?
Miss that, and the student has 120 days after their last day of attendance, or until the close of the aid year (typically September), whichever comes first, to finish verification and still qualify for the Federal Pell Grant. Every other form of aid, including institutional grants, is forfeited. If federal aid was already disbursed before verification was complete and the student never finishes, that money has to be returned.10Arizona State University. What Are the Deadlines to Apply for Financial Aid?
If the review isn’t finished before the semester starts, aid won’t disburse until eligibility is confirmed, which can mean paying tuition out of pocket and waiting for reimbursement.11Arizona State University. Financial Aid Disbursement Submit the form the week it appears, not the week classes begin.
When the Form Isn’t the Right Tool
The Parent Review Form verifies what was already reported. If the family’s finances have changed in a way the FAFSA no longer reflects, ASU has a separate Special Circumstances process where an administrator can adjust the aid offer using professional judgment.12Arizona State University. Special Circumstances When both apply, the Parent Review Form usually has to be completed first, because verification has to be done before any adjustment can be finalized.
If a parent refuses to complete the form and has ended all financial support, the student may be able to borrow a limited Federal Direct Unsubsidized Loan without parent data. The student stays classified as dependent and remains at dependent borrowing limits, and no grants, subsidized loans, or institutional need-based aid are available through that path. If there is any realistic chance of getting the form completed, that produces a far better outcome.
