How to Fill Out and Submit the BCBS Texas ABA Authorization Form

The BCBS Texas ABA authorization form is actually two forms: the ABA Clinical Service Request Form and the ABA Initial Assessment Request Form. Blue Cross and Blue Shield of Texas needs both, along with the full supporting packet, at least two weeks before the requested start date for Applied Behavior Analysis therapy. Submit anything less, or submit late, and claims get denied and pushed into a retroactive review that stalls reimbursement and leaves families in billing limbo.

What to Gather Before You Open the Form

Going back for missing information is the fastest way to miss the two-week window. Pull everything together first.

  • Member insurance details: Patient’s full legal name, date of birth, subscriber name, subscriber ID, and group number from the BCBSTX card.
  • Provider credentials: The National Provider Identifier for both the rendering clinician and the billing practice. The NPI is a 10-digit number assigned to every covered healthcare provider in the United States.1Centers for Medicare & Medicaid Services. National Provider Identifier Standard
  • Diagnostic evaluation report: A formal evaluation signed by a qualified diagnostician, typically a neurologist, developmental pediatrician, or licensed psychologist. The diagnosis cannot be older than 36 months at the time of submission.2Blue Cross and Blue Shield of Texas. Applied Behavior Analysis Clinical Service Request Form
  • ICD-10 diagnosis code: The primary code for the patient’s condition. For autistic disorder, that is F84.0.3World Health Organization. International Statistical Classification of Diseases and Related Health Problems 10th Revision
  • Baseline assessment data: The treating Board Certified Behavior Analyst’s skills assessment and baseline data showing current functioning levels.
  • Treatment plan: A written plan listing targeted behaviors, functional skill goals, and requested weekly hours for direct therapy, supervision, and caregiver training.

Diagnostic reports commonly cite standardized tools like the ADOS-2 (Autism Diagnostic Observation Schedule) or the CARS2 (Childhood Autism Rating Scale). BCBSTX does not mandate a specific instrument, but the report must clearly support the autism spectrum disorder diagnosis with standardized measures.

The Two Forms You Need to Submit Together

Sending only the Clinical Service Request Form is the most common reason packets get held up. Both documents go in together.4Blue Cross and Blue Shield of Texas. Management of Applied Behavior Analysis – Section: ABA Prior Authorization

  • ABA Clinical Service Request Form (pages 1–5): The main document, covering member information, provider details, clinical summary, treatment plan, and requested service hours. The last page carries a certification statement where the supervising clinician attests that the member can actively participate in treatment and demonstrates the capacity to learn generalized skills.2Blue Cross and Blue Shield of Texas. Applied Behavior Analysis Clinical Service Request Form
  • ABA Initial Assessment Request Form: A shorter form capturing the diagnostic practitioner’s information, the rendering provider’s credentials and state licensure, and the assessment CPT codes (97151 for the qualified healthcare provider’s assessment, 97152 for technician-administered assessment). It must be received within 30 days of the assessment start date.5Blue Cross and Blue Shield of Texas. ABA Initial Assessment Request

Both forms live on the BCBSTX provider website under behavioral health forms. For questions about which version to use, call BCBSTX at 800-851-7498.2Blue Cross and Blue Shield of Texas. Applied Behavior Analysis Clinical Service Request Form

Working Through the Clinical Service Request Form

The form runs five pages. Fill it out in order; the sections build on each other.

Member Information

Patient’s full legal name, date of birth, subscriber name, subscriber ID, group number, and the state where the patient resides. If the patient lives in one state but receives services in another, note that clearly. BCBSTX may apply different review criteria depending on the service location.

Provider Information

Billing practice name and NPI, rendering qualified healthcare provider’s name and NPI, credential type (BCBA, BCBA-D, or other state-recognized certification), state license number, practice address, and separate contact details for both the clinical supervisor and the billing contact. BCBA-D is not a separate certification from BCBA; it indicates doctoral-level training and carries the same clinical privileges and supervision authority.6Behavior Analyst Certification Board. Board Certified Behavior Analyst

Clinical Information

This is where most requests run into trouble. Enter the requested start date, keeping the two-week lead time in mind. Include a concise treatment plan summary listing the behaviors targeted for reduction, the functional skills being built, and the baseline data behind those targets. Specify the treatment model (focused or comprehensive ABA) and the exact weekly hours requested for direct one-on-one therapy, clinical supervision, and caregiver training. Each hour figure should tie directly to a clinical need identified in the diagnostic report.

Certification Statement

The supervising clinician signs to confirm the member has a reasonable expectation of benefiting from ABA and that the requested hours align with the plan’s medical necessity criteria. The clinician also acknowledges that the approved length of stay may differ from what was requested, based on the reviewer’s assessment.2Blue Cross and Blue Shield of Texas. How to Submit

Submit the completed packet through the Availity electronic portal or by fax. Availity is the better option. It generates a confirmation receipt and lets you track status in real time.

To submit through Availity:

  • Log in at availity.com.
  • Select Patient Registration, then Authorizations & Referrals, then Authorizations.
  • Choose BCBSTX as the payer and select your organization.
  • Select the appropriate authorization type (outpatient for most ABA cases).
  • Complete the required fields, attach supporting documents, and submit.7Blue Cross and Blue Shield of Texas. New Prior Authorization and Referral Submission Tool via Availity

If faxing, send the full packet (both forms plus all supporting documents) to the behavioral health fax line at 1-888-530-9809.8Blue Cross and Blue Shield of Texas. Utilization Management (Prior Authorizations) Keep the transmission confirmation page. If BCBSTX later says they never received the submission, that page is your proof of date and time.

What Happens After You Submit

Once BCBSTX has a complete packet with all information needed to review, Texas law requires the insurer to transmit its determination no later than the second working day after receipt.9State of Texas. Texas Insurance Code INS 4201.302 The clock does not start until the insurer has everything it needs. A missing document resets the timeline.

If the determination is adverse (a denial or partial approval) and the patient is not hospitalized, the insurer must provide written notice to both the provider and the patient within three working days.10Texas Public Law. Texas Insurance Code Section 4201.304 The notice must explain the specific clinical reasons the services were found not medically necessary and describe how to appeal, including how to request independent review.

Three outcomes are typical: full approval, partial approval with fewer hours than requested, or denial. If the reviewer finds the submitted information insufficient, they may request additional documentation, and the decision clock pauses until it arrives. This is where incomplete initial packets cost families weeks.

If the Request Is Denied

A denial is not the end. BCBSTX offers an internal appeal, and after that an independent external review.

You have 60 days from the date you receive the denial notice to file the internal appeal. The provider, the family, or an authorized representative can file. Send it by mail to Blue Cross and Blue Shield of Texas, Attn: Complaints and Appeals Department, P.O. Box 660717, Dallas, TX 75266-0717, or call 1-888-657-6061.11Blue Cross and Blue Shield of Texas. Complaints and Appeals If you want services to continue while the appeal is pending, you must request that within 10 days of receiving the denial letter, not 60.

If waiting for a standard decision could seriously harm the patient’s health, you or the treating provider can request an emergency appeal. BCBSTX must decide emergency appeals within 72 hours.11Blue Cross and Blue Shield of Texas. Complaints and Appeals

If the internal appeal is denied, you can request an independent external review. Federal rules give you four months from the date you receive the final internal denial to file a written request. The review is conducted by an independent organization with no relationship to BCBSTX, and its decision is binding on the insurer.12HealthCare.gov. External Review

Reauthorization for Ongoing Services

Initial authorization does not last indefinitely. BCBSTX requires concurrent review, where the treating BCBA submits updated clinical data showing progress and continued need. Concurrent review uses the same Clinical Service Request Form, refreshed with current assessment data, progress toward treatment goals, and any adjustments to service hours.4Blue Cross and Blue Shield of Texas. Management of Applied Behavior Analysis – Section: ABA Prior Authorization

Reviews commonly fall every four to six months, though the exact interval varies by plan. Missing a reauthorization deadline can mean retroactive denials, with financial exposure for both the provider and the family. Track the expiration date of each authorization period and begin the next submission well before it lapses. The same two-week lead time applies.
