How to Fill Out and Submit the BCBSIL Claim Review Form

The BCBSIL Claim Review Form is a one-page document providers use to ask Blue Cross and Blue Shield of Illinois to reconsider a claim that has already been finalized. Download it from the BCBSIL provider website, fill in the claim and member details, explain why the original adjudication was wrong, attach your supporting records, and send the packet to P.O. Box 660603, Dallas, TX 75266-0603 for commercial plans or P.O. Box 4555, Scranton, PA 18505 for Medicare Advantage.1Blue Cross and Blue Shield of Illinois. Claim Review Form

When to Use This Form

The form is only for claims that have already run through adjudication, meaning BCBSIL has processed the claim, issued a payment or denial, and generated an Explanation of Benefits. Pending claims cannot be reviewed this way, and the form itself instructs you not to attach the original claim.1Blue Cross and Blue Shield of Illinois. Claim Review Form If the problem is that your original submission was incomplete or unclear, submit a corrected or replacement claim through the normal channel instead.

A claim review (also called a reconsideration) is not the same as a formal appeal. A review asks BCBSIL to look again at how the claim was adjudicated, typically a coding dispute, a missed authorization, or an incorrect denial reason. A formal appeal involves a physician or clinical peer reviewing a medical-necessity determination and carries specific rights under ERISA and state law. If your dispute turns on whether the service was medically necessary, you likely need the appeal process rather than this form.

The Reason for Review section lists the categories the form covers. Check every box that applies:

  • Authorization — the claim was denied or reduced over an authorization issue.
  • Code Editing — automated edits bundled, denied, or reduced codes you believe should have paid separately.
  • Pre-Pay — the claim was flagged or held in pre-payment review.
  • Experimental/Investigational — the service was denied as experimental.
  • NSA — disputes related to the No Surprises Act.
  • Timely Filing — the claim was denied as late but you believe it was timely.
  • Other — anything outside the categories above.

How to Fill Out the Form

The form warns at the top that inquiries missing required information will not be reviewed, so completeness matters. Pull the Explanation of Benefits, the patient’s insurance card, and your provider details before you start.1Blue Cross and Blue Shield of Illinois. Claim Review Form

  • Claim Number(s): the Document Control Number (DCN) from the Explanation of Benefits. Multiple related claims can go on a single form if they share the same dispute reason.2Blue Cross and Blue Shield of Illinois. Claim Review and Appeal
  • Group Number: the employer group number from the insurance card.
  • Prefix: the three-character alpha or alphanumeric prefix in front of the Member ID.
  • Member Identification Number: the member’s ID from the card.
  • Patient Name: last name, then first.
  • Provider Name: the billing provider as it appeared on the original claim.
  • NPI: the provider’s National Provider Identifier.
  • Contact Person and Phone Number: a direct line BCBSIL can call with questions.
  • Reason for Review: every applicable box from the list above.
  • Detailed Information: the open text area explaining why the adjudication was wrong.

The detailed information section is where most reviews succeed or fail. A note that just says “please reprocess” gives the reviewer nothing to work with. Cite the specific code that was incorrectly bundled, reference the authorization number that was on file, or identify the policy provision covering the denied service. If you are requesting a second review of the same claim, you must provide information that was not part of the first review. BCBSIL will not reconsider a claim on the same evidence twice.1Blue Cross and Blue Shield of Illinois. Claim Review Form

For medical-necessity questions that come up during review, BCBSIL draws on MCG Care Guidelines (though not all of them). Provider correspondence and Clinical Payment and Coding Policies indicate when a specific guideline applies, and the guidelines themselves are available through the Availity Essentials portal under the Resources tab in the BCBSIL Payer Space.3Blue Cross and Blue Shield of Illinois. What to Attach

The records you include depend on the dispute:

  • Code editing: operative reports showing procedures were distinct, modifier documentation, or published coding guidelines supporting separate reporting.
  • Authorization: a copy of the authorization confirmation with the authorization number and the date issued.
  • Experimental/investigational: peer-reviewed literature, FDA approvals, or a letter of medical necessity from the treating physician.
  • Timely filing: proof of original submission such as a clearinghouse acceptance report, fax confirmation, or certified mail receipt showing the claim went in within the filing window.

Write the Member ID and Claim Number (DCN) on every page of your attachments. Large packets get separated during scanning, and unlabeled pages can end up disconnected from the request. If you are adding diagnostic codes that were missing from the original claim, call them out in both the detailed-information section and the attached records so the reviewer can match them to the right line items.

Filing Deadlines

For ERISA-governed group health plans, federal regulations give claimants at least 180 days after receiving a denial notice to appeal an adverse benefit determination.5eCFR. 29 CFR 2560.503-1 – Claims Procedure Your Explanation of Benefits or denial letter will state the deadline that applies to your specific plan. Some plan types carry shorter windows: BCBSIL’s community health plans, for example, require appeals within 60 calendar days from the date on the Notice of Action letter. Don’t wait until the last week. Gathering documentation takes time, and a form submitted without the right records won’t accomplish much even if it arrives on time.

After You Submit

Once BCBSIL receives the review, the claim goes back through adjudication with the new information. A successful review produces a revised Explanation of Benefits reflecting the corrected payment or adjustment. If the original decision is upheld, you receive a written explanation telling you why. Formal appeals involving physician or clinical peer review take 30 days and conclude with a written determination.2Blue Cross and Blue Shield of Illinois. Claim Review and Appeal Reconsiderations that don’t need clinical peer review can resolve faster, but BCBSIL does not publish a guaranteed turnaround for them. Watch the provider portal for status changes; updates there usually show before a letter arrives.
