What the Form Asks and What to Gather First
The renewal form is a signed legal declaration that you still qualify for the program. It gives you two ways to check the box: enrollment in a qualifying assistance program, or household income at or below the California LifeLine limit.1California Public Utilities Commission. Before you start, pull together:
- The renewal notice itself. Your deadline and account references are printed on it.
- The last four digits of your Social Security Number or Tribal Identification Number, used for identity verification.
- Proof of eligibility dated within the last 12 months.
If you are qualifying through a program, acceptable proof is a benefit award letter, a statement of benefits, or a screenshot from your benefits portal that shows your name, the program name, and a recent date. Qualifying programs include Medi-Cal, CalFresh/SNAP, SSI, Federal Public Housing Assistance or Section 8, WIC, the National School Lunch Program, LIHEAP, CalWORKs and other TANF programs, Tribal TANF, Bureau of Indian Affairs General Assistance, Head Start Income Eligible (Tribal only), the Food Distribution Program on Indian Reservations, and the Federal Veterans and Survivors Pension Benefit Program.1California Public Utilities Commission. If the Administrator can verify your enrollment electronically through state databases, you may not have to send documents at all; the form or online system will tell you.
If you are qualifying by income, California LifeLine uses 150% of the Federal Poverty Guidelines. The limits in effect from June 1, 2025 through May 31, 2026 are $24,200 for one person, $32,600 for two, $41,100 for three, $49,600 for four, and add $8,500 for each additional household member.3California LifeLine. California LifeLine Household income means all pretax money received by everyone at your address who shares income and expenses: wages, Social Security, pensions, unemployment, child support, and any other source. Common proof documents are your most recent federal tax return, pay stubs covering at least three consecutive months, or a Social Security statement of benefits.
Submitting Online
Go to CaliforniaLifeLine.com and click “My Account” at the top right. The old PIN system is gone. Since December 2023, the portal uses an email address as your username with a password you create, so first-time users need to select “Register” and set up credentials.4California LifeLine. Every login uses multi-factor authentication. After your email and password, the system sends a one-time code by email or text that is valid for about 10 minutes.4California LifeLine. California LifeLine Customer Portal FAQ Once inside, follow the prompts to confirm your qualifying program or income, upload any documents the system asks for, and submit. You should see an immediate on-screen confirmation of receipt.
Submitting by Mail
Fill out the paper form from the pink envelope. Check the box for either the specific assistance program you or a household member is enrolled in, or the income option with your household size and income range. Sign and date it. Because the form is a legal declaration, the information has to be accurate.

Send it early enough to arrive before your deadline. The date the Administrator receives your form is what counts, not the postmark.
After You Submit
The Administrator reviews your form and checks the information against state databases. If everything matches, your discount continues without interruption, and you get confirmation by mail or text to the phone number on file. Your enrollment status also appears in the online portal once processing is complete.
If something is missing or unclear, you will receive a notice explaining what to send. Respond quickly, because a slow reply can push you past your renewal window.
If You Miss the Deadline
A signed form that arrives after the due date ends the discount. Your phone service is not disconnected, but the monthly bill goes up by as much as $19.00, and getting the benefit back requires a brand-new application rather than a renewal.6California Public Utilities Commission. If Your Notice Never Arrived
The Administrator mails renewal notices in pink envelopes to the address on file, and your carrier is required to send yours at least 30 days before your due date.6California Public Utilities Commission.
