Make Sure AHCA Is the Right Agency
AHCA regulates healthcare facilities: the building, the operation, and the care delivered inside it. It does not handle complaints against individual doctors, nurses, dentists, or other licensed practitioners acting in their personal capacity. Those complaints go to the Florida Department of Health through its portal at complaint-portal.mqa.flhealthsource.gov/.2FL HealthSource. Florida Health Care Complaint Portal If a nurse at a hospital gave your mother the wrong medication, you can file with AHCA about the hospital’s procedures and separately with the Department of Health about the nurse.
What to Have Ready Before You Start
The form moves faster, and your complaint lands harder, if you gather everything first.
- The facility’s legal name, street address, city, and ZIP code. Use the name on the license, not a nickname, so AHCA can match it to the right record. FloridaHealthFinder.gov lets you search licensed facilities if you’re unsure of the exact name.1Agency for Health Care Administration. Health Care Facility Complaint Form
- The patient or resident’s name and date of birth, plus their diagnosis and insurance type if you know them.3Agency for Health Care Administration. Health Care Facility Complaint Form
- Full names of any staff involved. Partial names or job titles still help investigators figure out who to interview.
- Specific dates and times. Incidents more than twelve months old generally do not result in an on-site inspection, though AHCA keeps the information on file.1Agency for Health Care Administration. Health Care Facility Complaint Form
- Supporting documents: medical records, photos, discharge papers, written witness statements. You can request copies of your own medical records from any provider under HIPAA, with a modest fee cap for electronic patient-directed requests.
Walking Through the Form
The form sits at apps.ahca.myflorida.com/hcfc/. No login or account is needed.
The first section asks for the facility’s identifying information. Enter the legal name, street address, city, and ZIP code.
The next section collects the patient or resident’s details: name, date of birth, insurance type. If your complaint is about general conditions rather than one person’s care, you can skip this part.
The narrative section is where the complaint lives or dies. Write a clear, factual account of what you saw or experienced, who was involved, when it happened, and what the facility did or failed to do. Stick to facts rather than conclusions. “The call light went unanswered for 45 minutes while my mother called out in pain” is far more useful to an inspector than “the staff was neglectful.” Include names, dates, room numbers, and any specific policy or promise the facility broke.3Agency for Health Care Administration. The final section asks for your contact information. It’s optional. You can file anonymously by leaving it blank.3Agency for Health Care Administration. Health Care Facility Complaint Form Providing your name and email does two things: it lets investigators follow up for clarification, and it’s the only way you’ll be notified about the outcome. Keep in mind that information submitted to Florida state agencies can become subject to public records requests under the Florida Constitution unless a specific statutory exemption applies.

What Happens Next
AHCA’s Complaint Administration Unit reviews every submission to decide whether the allegations fall within the agency’s regulatory authority. If they do, AHCA opens a complaint inspection. If the complaint is really about an individual practitioner or otherwise outside AHCA’s jurisdiction, the agency’s response will say so.1Agency for Health Care Administration. When an inspection is triggered, a surveyor visits the facility, typically without advance notice. The inspector reviews the laws relevant to your complaint, examines facility records, observes how current patients and residents are being cared for, and interviews staff and other people receiving care.3Agency for Health Care Administration. Health Care Facility Complaint Form The surveyor’s job is to determine whether the facility is violating any law at the time of the inspection, not just to confirm your specific experience.
If you gave contact information, AHCA will email you the disposition of your complaint and a separate notification with any inspection results.1Agency for Health Care Administration. The False Claims Act protects anyone who reports fraud against the federal government, which reaches many Medicare and Medicaid billing issues. OSHA’s general anti-retaliation provision covers workplace safety hazards. HIPAA includes a safe harbor allowing healthcare workers to disclose protected health information to public health authorities or their own attorney when they reasonably believe the employer has engaged in unlawful conduct or that patient care is at risk.8Whistleblower.house.gov.
