To complete a Georgia CNA renewal, submit the Nurse Aide Registry renewal form with proof that you worked at least eight hours as a paid nursing assistant during the 24 months before your expiration date. There is no fee. Alliant Health Solutions processes the applications, and you can file online through its portal or mail the form to its Atlanta office.
The Eight-Hour Work Requirement
Federal regulations require that you performed paid CNA work under the supervision of a licensed nurse for at least eight hours within the 24 consecutive months leading up to your expiration date.1eCFR. 42 CFR 483.156 – Registry of Nurse Aides If you cannot document those hours, you are not eligible for standard renewal and will need to test again to get back on the registry.
You need at least one of the following to prove the work:
- A pay stub showing you were paid for CNA duties.
- A W-2 from the employer where you performed nursing aide work.
- A letter on the employer’s letterhead confirming your dates and duties, if you no longer work there.
Private duty CNAs have an extra step. If your work was for a private client rather than a facility, you must submit a notarized statement that lists your job duties, the timeframe you worked, and the signature and license number of your supervising LPN or RN, along with a pay stub or W-2. Both the employer and the supervising nurse sign the statement.
Filling Out the Renewal Form
The renewal application has two employment sections. Complete the one that fits your situation, not both. Then sign and date the form yourself.
Section A: Currently Working as a CNA
Use Section A if you are employed as a nursing assistant right now. Enter your current employer’s name, phone number, address, and employer type, then list the dates you worked there. Your employer signs and dates this section. Attach a pay stub or W-2 that shows paid CNA work during the renewal period.
Section B: Not Currently Working as a CNA
Use Section B if you are not working as a CNA now but did at some point during the past 24 months. Enter your most recent CNA employer’s name, phone, address, and type, plus your hire date and end date. Attach a pay stub, W-2, or a letter from that employer on their letterhead confirming your CNA work.

Submitting the Form
You have two options. Uploading through the Alliant Health Solutions portal at nurseaide.allianthealth.org is faster. Log in, go to the renewal section, and upload scanned copies of the signed form and your proof of employment. Legible scans matter; anything blurry or cut off will slow the review.

Renewal is free. If You Missed the Window or the Deadline
The standard form will not help you if you did not work at least eight hours during the 24-month window, or if your certification has already expired. What you do next depends on how long you have been off the registry.
Less Than Three Years Since Your Last Recertification
You can regain your status by passing the state competency examination, which has a written or oral knowledge portion and a hands-on clinical skills evaluation. Credentia administers the Georgia exam. A full training program is not required at this stage; you only need to pass both parts of the test.
Three Years or More Since Your Last Recertification
Georgia requires you to complete a new state-approved nurse aide training program before you can sit for the exam again. Approved programs must include at least 85 hours of classroom, lab, and clinical instruction.3Georgia Nurse Aide Registry. If you work at a Medicare- or Medicaid-certified nursing facility, your employer may be required to cover the cost of training and testing. Federal rules bar these facilities from charging nurse aides for training, materials, testing, or registry fees when the aide is employed there or has an offer of employment.
Keep Your Address and Name Current
Notify the Georgia Nurse Aide Registry within 10 business days any time your legal name or mailing address changes. If your address is out of date, you may never receive your renewal notice.4Georgia Nurse Aide. The Registry
Your written notification must include your previous name, current name, mailing address, phone number, and Social Security number. For a name change, add official documentation: either a signed Social Security card showing the new name plus a government-issued photo ID, or a court-issued document such as a marriage certificate or divorce decree paired with a government photo ID. Coming From or Moving to Another State
If you are moving to Georgia with an active certification from another state, you do not necessarily start over. Most states offer a reciprocity process for CNAs in good standing. You generally need an active certification with no disciplinary findings, and you may need to show that your training met Georgia’s 85-hour requirement, which is higher than the federal minimum of 75 hours.3Georgia Nurse Aide Registry. Transfer applications go through the same Alliant Health Solutions portal used for renewals. Expect to submit verification of your current certification from your home state and to complete a background check. After Georgia accepts your transfer, you follow the two-year renewal cycle described above.
