How to Fill Out and Submit the Idaho Duplicate Title Application (ITD 3367)

To replace a lost, damaged, or unreadable Idaho vehicle or vessel title, file the Idaho duplicate title application, ITD 3367, with your county assessor’s motor vehicle office. You can submit it in person or by mail, and the typical total cost runs between $14 and $33 depending on the county. The one catch that stops most applications is the signature step: it must be notarized or witnessed by a county assessor or deputy assessor before the form will be processed.1Idaho Transportation Department. When ITD 3367 Is the Right Form

Idaho issues a duplicate title in three situations: the original is lost, mutilated, or illegible. If you still have the damaged or unreadable title, you attach it to the application. If it’s simply gone, no attachment is needed and the form presumes it lost.1Idaho Transportation Department. Duplicate Idaho Title Application

One situation does not call for this form. If your vehicle has a paperless electronic title because of an active lien, you don’t file ITD 3367. The Idaho Transportation Department automatically prints and mails a paper title the business day after your lienholder sends an electronic lien release.2Idaho Transportation Department. And a bigger boundary worth stating up front: ITD 3367 does not transfer ownership. The form says so explicitly. If your real goal is to sell the vehicle, see the last section below.1Idaho Transportation Department. Duplicate Idaho Title Application

Who Can File the Application

Only three parties may file ITD 3367: the owner of record, the lienholder of record, or an authorized agent of either.1Idaho Transportation Department. Duplicate Idaho Title Application A buyer who never completed a title transfer cannot use this form. The application must come from whoever Idaho’s records currently show as the owner.

A lienholder filing the form provides the business name along with an authorized representative’s signature and job title. An agent acting for the owner must attach the original or a verified copy of a power of attorney. ITD 3368 works for this, and the power of attorney must be notarized when used for a duplicate title application.3Idaho Transportation Department. Limited Power of Attorney

Filling Out the Form

You can download ITD 3367 from the Idaho Transportation Department website or pick one up at any county assessor’s motor vehicle office. The form has six sections. Sections 1, 2, and 6 are required; the others apply only in certain situations.1Idaho Transportation Department. Duplicate Idaho Title Application

Vehicle or Vessel Information (Section 1)

Enter the full VIN (or hull identification number for a watercraft) along with the year, make, model, and a brief description. Pull these from your registration, insurance card, or the VIN plate itself. A mismatched VIN is the fastest way to get the application rejected.

Owner Information (Section 2)

List every owner exactly as they appear on the current title record. For individuals, provide an Idaho driver’s license number or Social Security number. For a business or other entity, use the Employer Identification Number. If two owners are listed, check “OR” or “AND” to show how ownership is held, because that choice decides who has to sign in Section 6.1Idaho Transportation Department. Use a permanent physical home address, not a P.O. box. If your legal name has changed since the original title was issued, attach a signed “One and the Same Statement” (ITD 3125) to the application.

Lienholder and Alternate Mailing Address (Sections 3 and 4)

List a lienholder in Section 3 only if the original title showed an active, unreleased lien. When a lienholder is listed, the replacement title gets mailed directly to that lienholder — unless the lienholder also signs Section 6 and you give an alternate address in Section 4.1Idaho Transportation Department. Fees (Section 5)

Credit cards are accepted but carry a service fee. If you’re mailing the form, make your check payable to “DMV.” Don’t put cash in the mail; pay cash in person at a county office.1Idaho Transportation Department. Duplicate Idaho Title Application

Reason and Signature (Section 6)

Check the box for lost, illegible, or mutilated. The form presumes “lost” unless you check otherwise. If you check illegible or mutilated, attach the old title. Then sign — but see the next section first.

The Signature Step

Most applications stall here. Your signature on ITD 3367 must be either notarized by a notary public or witnessed by an assessor or deputy assessor at the county office. Without one or the other, the form will not be processed.1Idaho Transportation Department. Duplicate Idaho Title Application

If you plan to mail the application, get it notarized before you send it. If you bring it in person, the assessor or deputy at the counter can witness the signature on the spot, which saves a trip to a notary.

When two owners are joined by “OR,” either one can sign alone. When they’re joined by “AND,” both must sign. If one “AND” owner wants off the replacement title entirely, that owner files a Release of Interest (ITD 3377) instead of signing the application. By signing, you certify under penalty of law that everything on the form is true.1Idaho Transportation Department. Duplicate Idaho Title Application

Where to Send It and What It Costs

Submit the completed form in person or by mail to your local county assessor’s motor vehicle office. The form itself lists mailing addresses for every Idaho county.1Idaho Transportation Department. Duplicate Idaho Title Application

The statutory title fee is $14.00. Every county adds its own administrative fee on top. County admin fees range from $0 in counties like Bannock and Caribou to $18.75 in Blaine County, putting the total somewhere between $14.00 and $32.75. Most counties land between $19 and $25.4Idaho Transportation Department. County Registration and Title Fees

Rush processing is available in some counties for an extra fee. Ada County, for example, charges $33.00 for rush service on top of the standard title fee.5Ada County. Vehicle Registration, Renewals and Titles Call your county office for its specific rush fee and turnaround. Under standard processing, titles are mailed the business day after the title is issued.

If You Actually Need to Sell the Vehicle

ITD 3367 alone won’t let you transfer the vehicle to a buyer. You have two paths depending on what you’re selling.

For vehicles at least ten years old, vehicles without an odometer (such as trailers or certain off-highway vehicles), vehicles with a gross weight over 16,000 pounds, and vessels, Idaho offers ITD 3369, a combined duplicate title application and ownership transfer. It replaces the lost title and transfers ownership in a single step.6Idaho Transportation Department. Duplicate Idaho Title Application With Ownership Transfer – ITD 3369

For newer vehicles that don’t qualify for ITD 3369, file ITD 3367 first, wait for the replacement to arrive, and then sign that duplicate over to the buyer through the standard title transfer process. The buyer must file the signed title and supporting documents at a county assessor’s motor vehicle office within 30 days of the transfer to avoid a $20.00 late-filing penalty.2Idaho Transportation Department.
