The Idaho Work Verification Form is a short employer questionnaire the Idaho Department of Health and Welfare sends out to confirm an employee’s wages, hours, health insurance offer, and job status. If it landed on your desk, a caseworker needs the information to decide a benefits application (SNAP, Medicaid, or TAFI) or to set or enforce a child support order. Fill it in from your payroll records, sign it, and return it by the method printed on the cover letter. A payroll clerk can usually finish it in about ten minutes.
Who Sends the Form and Why It Matters
The Department of Health and Welfare and Idaho Child Support Services send this form directly to employers when they need current, verified income data on a specific worker. The agency uses your answers to determine eligibility for public assistance and to calculate child support obligations. Because Idaho’s child support guidelines pull from gross income before taxes, the figures you write down feed almost directly into the support calculation. An estimate or rounded guess can produce a benefit overpayment the employee later has to repay, or a support order that doesn’t match real earnings.

What to Gather Before You Start
Pull these items from your payroll system first:
- The employee’s full name, Social Security number, and hire date
- Current pay rate (hourly wage or salary) and pay frequency
- Recent pay stubs or a payroll report showing gross earnings by pay period, including any overtime, commissions, or bonuses
- Your business name, address, phone number, and federal tax ID
- The name and premium cost of any group health plan the employee is offered
Having everything in front of you prevents the back-and-forth that stalls the employee’s case.
Filling Out Each Section
Employee and Employer Identification
The top block asks for the employee’s full name, Social Security number, and date of hire, plus your business name, address, phone number, and the name of the person completing the form. Write legibly. A transposed digit in the SSN or an illegible phone number sends the form back for correction, and the employee’s application waits until it clears.
Earnings and Pay Details
Report the hourly wage or salary, the pay frequency (weekly, biweekly, semimonthly, or monthly), and gross earnings for each of the recent pay periods listed on the form. If pay varies week to week because of overtime, tips, commissions, or bonuses, put in the actual amount for each period rather than an average. Health Insurance
The insurance section asks whether group coverage is available to the employee, whether the employee is enrolled, and what the employee would pay for individual and family coverage. This is not a throwaway section. In child support cases, Idaho law directs the department to enforce medical support when private coverage is available at reasonable cost, and your answers determine what happens next.2Idaho State Legislature. If the employee was offered coverage and declined, note the date they first became eligible. Do not leave any insurance line blank. An incomplete insurance response is one of the most common reasons the form comes back.
Employment Status
The last fields cover whether the employee is still actively working for you, the date of any recent separation, whether a termination is pending, and whether hours have recently been reduced. This is how the caseworker distinguishes stable employment from a situation where income is about to change.

For benefits verifications tied to SNAP, TAFI, or Medicaid, the local Health and Welfare office that issued the request will list a return address or fax number on the form itself. If nothing is listed, call the office named on the cover letter. Employees can also upload documents through idalink at idalink.idaho.gov.
Keep a copy of what you send. If the agency follows up about an inconsistency, your copy makes it easy to answer. For online submissions, save or screenshot the confirmation page as proof you responded on time.
Confirming the Request Is Legitimate
The completed form contains a Social Security number, wage data, and health insurance details, so treat it accordingly. Limit access to payroll and HR staff who need to see it, and send it only through a secure channel: the agency’s designated fax line, certified mail, or an encrypted portal. Do not email a completed form with an unencrypted attachment.

What Happens if You Ignore It
The form is administrative, but the agencies behind it have enforcement tools. When employers refuse to respond, the department can subpoena payroll records, and courts in child support proceedings routinely order the production of employment records. An employer that stonewalls the process can face contempt sanctions. Responding is faster and cheaper than the alternative.
Related Employer Obligations That Are Not This Form
Two adjacent duties come up often enough to be worth flagging, because they are separate from returning a Work Verification Form:
New hire reporting. Under Idaho Code ยง 72-1604, every employer must report each new hire or rehire to the Idaho Department of Labor within 20 calendar days of the start date, with the employee’s name, address, SSN, the employer’s federal tax ID, the Idaho unemployment insurance account number, and the hire date.4Idaho State Legislature. This is how the state often learns where a noncustodial parent works in the first place.
Income withholding. If a child support income withholding order follows the verification, federal limits under the Consumer Credit Protection Act cap what you can take from a paycheck: 50 percent of disposable earnings if the employee is supporting another spouse or child, 60 percent if not, with an added 5 percent when the employee is more than 12 weeks behind.6U.S. Department of Labor.
