How to Fill Out and Submit the Illinois Instruction Permit Application

To get an Illinois instruction permit, you apply in person at a Secretary of State Driver Services facility. The application form is not online — you pick it up at the counter, fill it out there, and submit it during the same visit that includes your vision screening, the 35-question written exam, and payment of the fee. Anyone 15 or older can apply, but the requirements shift depending on whether you are under or over 17 years and 3 months old.

Who Can Apply

Eligibility turns on your age and driver education status. Applicants between 15 and 17 must be enrolled in an approved driver education course, or within 30 days of starting one. Anyone under 18 who has been certified as a chronic or habitual truant cannot get a permit until they can show regular school attendance has resumed.2Illinois General Assembly. Documents to Bring

Illinois requires original documents from four groups: one each from the first three, and two from the fourth.   • Group A, written signature: a credit or debit card, Social Security card, U.S. passport, or canceled check.
  • Group B, date of birth: a certified birth certificate, U.S. passport, or naturalization certificate.
  • Group C, Social Security number: a Social Security card, W-2, SSA-1099, or pay stub showing your name and full SSN.
  • Group D, residency: two separate documents showing your current Illinois address, each dated within 90 days. A bank statement, utility bill, or insurance policy works.

Photocopies and digital printouts are not accepted for identity verification. If your legal name is different from the name on your birth certificate or passport, bring proof of the change, such as a marriage certificate or court order.4Illinois Secretary of State. If you want a REAL ID compliant permit — needed for boarding domestic flights or entering federal buildings — only documents marked with an asterisk on the DSD X 173 checklist qualify, and your Group B document also has to prove lawful U.S. presence. A birth certificate or passport covers both. If you do not specifically ask for REAL ID, you will get a standard permit.

Booking the Visit

What Happens at the Facility

Plan for about an hour. The visit has four parts: document review, vision screening, written exam, and payment.

Document Review and the Application Form

An examiner checks your identity documents against the four-group requirements and hands you the official application. The form asks for your full legal name, height, weight, eye color, and medical history, including any conditions that could cause loss of consciousness or impaired motor control. You sign a declaration that your answers are accurate.

Vision Screening

You need binocular visual acuity of 20/40 or better. If you wear glasses or contacts to reach that mark, your permit will carry a corrective-lenses restriction, meaning you must wear them any time you drive. If you fail without corrective lenses on hand, you can return another day and retake the screening, or submit a favorable report from a vision specialist instead.6Legal Information Institute. Written Knowledge Exam

The test has 35 questions: 15 on traffic sign identification and 20 multiple-choice or true-false questions on Illinois traffic laws.7Illinois Secretary of State. Sign shapes and colors are worth serious study since they account for nearly half the questions.

If you fail, you can retake the test a second time the same day during normal business hours. Your fee entitles you to three attempts within one year of your first try, with no additional charge for retakes in that window.8Legal Information Institute. Fees

  • First-time applicant under age 69: $20.
  • Applicant age 69 or older: $5.
  • Applicant under 69 who previously held an Illinois license or permit: $10.
  • Current license holder seeking a classification change: $5.

The Secretary of State accepts checks, cashier’s checks, and money orders. Credit cards work but carry a convenience fee.

What You Leave With and What Comes in the Mail

Once you pass and pay, the facility gives you a temporary paper permit valid for up to 90 days. It is only valid while you are physically carrying it, so leaving it at home means you cannot legally drive that day. No extensions are given past 90 days.10Justia. Your permanent plastic permit arrives by mail at the address on your application. If it has not shown up after 15 business days, check the Secretary of State’s mailing-status tool.11Illinois Secretary of State. Restrictions Once You Have the Permit

A permit is not a license. Every time you drive, a supervising adult must sit in the front passenger seat. That person must be at least 21, hold a valid license for the type of vehicle you are driving, and have at least one year of driving experience.2Illinois General Assembly.   • No driving from 10 p.m. to 6 a.m. Sunday through Thursday, or 11 p.m. to 6 a.m. Friday and Saturday. Local curfews may be stricter.
  • One passenger in the front seat, and only as many in the back as there are seatbelts.
  • Every occupant must wear a seatbelt.
  • Drivers under 19 cannot use a phone while driving, including hands-free, except in an emergency.
  • Texting is prohibited for all permit holders regardless of age.

How Long the Permit Lasts

Under 18, your permit is valid for 24 months, and you must hold it for at least nine months before taking the road test for a full license. At 18 or older, the permit is valid for 12 months.1Illinois Secretary of State.
