What to Gather Before You Fill It Out
Have these ready:
- Your eight-digit employee number, which usually begins with two leading zeros (for example, 00723671).3Los Angeles Unified School District. IT Helpdesk – Activating Your LAUSD Account
- Your bank’s nine-digit routing number, found on a check, in your bank’s mobile app, or by calling the bank.
- The account number for the specific checking or savings account where you want your pay to land.
- A voided check for a checking account, or a letter from your bank on its letterhead confirming the routing number, account number, and your name for a savings account.
Don’t use a deposit slip to verify the account. Deposit slips sometimes carry a different routing number than the one banks use for electronic transfers.
Filling Out the Form
Print your name and enter your eight-digit employee number at the top. Add your bank’s routing number and your account number exactly as they appear on the check or bank letter. One transposed digit sends your pay to the wrong place or triggers a rejection.
Mark whether the account is checking or savings, since those route through different channels at the bank. Then select what you’re doing: starting a new deposit, changing your bank information, or canceling direct deposit. Sign and date the bottom. An unsigned or undated form comes back to you, which pushes the whole process out at least another pay cycle.
How to Submit It
You have two ways to get the completed form to Payroll Administration:
- Mail or hand-deliver it, with your voided check or bank letter, to Payroll Administration, 333 S. Beaudry Avenue, 27th Floor, Los Angeles, CA 90017.1Los Angeles Unified School District. Payroll Administration
- Upload a scanned copy through the Employee Self-Service portal at selfservice.lausd.net, using your Single Sign-On credentials. Include the verification document with your upload.4Payroll Administration. LAUSD MyPay

What Happens After You Submit
The district runs a pre-note verification once your form arrives. That’s a zero-dollar test transaction confirming the routing and account numbers connect to a real, active account in your name. It usually takes at least one full pay period. During that window you keep getting a paper check, so there’s no gap in your pay.
Your pay stub tells you when the switch happens. Paper checks show a warrant number; direct deposits show a deposit confirmation instead. Once the pre-note clears, your next paycheck lands in your bank account on the designated payday. If the test transaction fails because a number was wrong or the account isn’t active yet, Payroll Administration will contact you, and you’ll submit a corrected form.
Changing Your Bank Account
Switching banks uses the same form. Download a fresh one, mark the “change” option, enter the new routing and account numbers, attach a voided check or bank letter for the new account, and submit it the same way.2Los Angeles Unified School District. Payroll Forms
Keep the old account open until you see the first deposit land in the new one. If you close it too early, the district’s payment has nowhere to go. The bank rejects the transfer, and Payroll Administration has to reissue your pay as a paper check after the returned funds clear, which can take several business days.
Canceling Direct Deposit
To go back to paper checks, fill out the same form and select the cancellation option. Submit by mail, in person, or through the portal. The change can take up to two pay cycles to process, so expect at least one more electronic deposit before paper warrants resume. Paper checks are either mailed to the address on file or distributed at your work site.
If You Don’t Have a Bank Account
The district offers a California Credit Union Pay Card as an alternative to both direct deposit and paper checks.5Los Angeles Unified School District. Payroll The card works like a prepaid debit card: your wages load onto it each payday, and you can use it for purchases or ATM withdrawals. Enrollment details are at ccupaycard.org or through Payroll Administration.
If a Deposit Is Wrong or Missing
If a deposit doesn’t arrive, comes in short, or overpays you, contact Payroll Administration at (213) 241-2570 or payrollsupport@lausd.net as soon as you notice.1Los Angeles Unified School District. Payroll Administration California law requires that wages be paid on time, and the district has its own internal deadlines to correct payroll errors, so flagging problems early leaves more room to fix them.6Department of Industrial Relations. Paydays, Pay Periods, and the Final Wages For overpayments greater than fifty dollars, the district’s collective bargaining agreements require notice before any deduction from future paychecks, and you can ask for a printed breakdown of the affected pay periods before agreeing to a repayment plan.
