The Louisiana DPSMV 1799 form is the Office of Motor Vehicles’ vehicle application, used to title and register a car, truck, or motorcycle whenever ownership changes, a title is lost, or an out-of-state title is being converted. You can download it from the OMV’s Express Lane website or pick one up at any OMV field office or public tag agent.1Louisiana Office of Motor Vehicles. When You Need to File One
You use the DPSMV 1799 any time the state’s title record needs to change. The common triggers:
- Buying a new vehicle from a dealer.
- Buying a used vehicle from a private seller or dealer.
- Moving to Louisiana and converting an out-of-state title.
- Replacing a title that was lost, stolen, or destroyed.
- Correcting a misspelled name or wrong VIN on an existing title.
- Recording a lien when the vehicle is financed.
Louisiana law requires the seller to hand over the certificate of title, properly endorsed with the sale and assignment, at the time of sale.2Louisiana State Legislature. Owner Information
Enter your full legal name exactly as it appears on your Louisiana driver’s license or state ID. The OMV defines “full legal name” in line with the Real ID Act, based on your birth certificate or a court-ordered name change. Add your license or ID number, residence address, city, parish, state, and ZIP. If the vehicle is titled to a business, use the business address and the EIN in place of a driver’s license number.
Vehicle Description
This is where most applications go wrong. You need the full 17-character VIN plus make, model, year, body style, and current mileage. Model is required for passenger vehicles and automobiles. Copy the VIN off the dashboard plate or door-jamb sticker, not off an older title. One transposed digit and the package comes back.
Odometer Disclosure
Federal law requires an odometer disclosure on every ownership transfer during the first 20 model years for vehicles of model year 2011 or newer. Model year 2010 and older vehicles remain under the earlier 10-year rule and may already be exempt.5National Highway Traffic Safety Administration. If the odometer has rolled over or is known to be inaccurate, mark that on the form; do not enter a false reading.
Purchase Price and Lien
Enter the full purchase price and any trade-in credit. The OMV uses these to calculate the tax you owe. The state rate on vehicle sales is 4.45%, and local parish and municipal taxes add roughly 1.85% to 7% on top.6Louisiana Department of Revenue. What to Attach

New Vehicle From a Dealer
- Manufacturer’s Statement of Origin (MSO), assigned from the dealer to you.
- Original itemized dealer invoice showing make, year, model, VIN, any trade-in, and the dealer’s current permit number.
- Signed odometer disclosure statement.
- UCC-1 or other security agreement if a lien is being recorded.
- Copy of each owner’s current, valid driver’s license or state ID (required if you also want a plate).
- Proof of Louisiana liability insurance, unless the dealer is submitting on your behalf.
Used Vehicle
Instead of an MSO, you submit the previous certificate of title, signed over to you by the seller with the sale-and-assignment section completed. Out-of-State Title Conversion
Submit the out-of-state certificate of title, the DPSMV 1799, a bill of sale, photo ID, and proof of Louisiana liability insurance. The OMV surrenders the old title and issues a Louisiana one.
Duplicate Title

Someone Handling It for You
If an agent is filing on your behalf, include the original power of attorney. If executed in Louisiana, it must be notarized. The mandate must name the specific agent and, for vehicle transfers, include the make, year, and VIN. The agent cannot sign as both buyer and seller on the same transaction, and cannot transfer the vehicle into their own name unless the mandate specifically authorizes it.10Louisiana Department of Public Safety. Insurance
You cannot register a vehicle in Louisiana without proof of liability coverage meeting the state minimum: $15,000 bodily injury per person, $30,000 per accident, and $25,000 property damage.11Louisiana State Legislature. Fees and Payment
All fees are due when you file. The OMV does not accept cash. You can pay by credit or debit card, check, cashier’s check, or money order. Checks cannot be used for reinstatement transactions.
- Title fee: $68.50 for each certificate, including duplicates and salvage titles.
- Lien recordation: $15.00 for a UCC-1 financing statement, $10.00 for other security agreement documents.
- Handling fee: $8.00 per transaction.
- Security interest notation: $5.00 per notation on the certificate.

The 40-Day Sales Tax Deadline
Sales tax is due by the 40th day after the date of sale. - Penalty: 5% of the unpaid tax for each 30 days delinquent, up to 25%.
- Interest: 1.25% per month on state tax, 1% per month on parish and municipal tax.

Where to File
You have two ways to submit the completed form and attachments:
- In person at an OMV field office or public tag agent. Staff review the package on the spot and flag problems immediately, and most in-person filings are handled the same day.

If a mailed application is rejected, the OMV grants one 30-day extension from the date the rejected package reaches you, with no penalty or interest during that window as long as you resubmit inside it.3Louisiana Department of Public Safety. Office of Motor Vehicles Sales Tax Policy That safety net is thin. One rejection can eat weeks, and a second mistake pushes you past the 40-day sales-tax deadline.
Why Applications Get Rejected
Run through this list before you send anything:
- Missing or mismatched VIN. One wrong character kills the application. Check against the physical vehicle.
- Unsigned or improperly endorsed title. The seller’s signature on the assignment must match the name printed on the title; a marriage or legal name change needs supporting documentation.
- No proof of insurance meeting the 15/30/25 minimums.
- Missing bill of sale or dealer invoice. New vehicles need the original itemized invoice; used vehicles need a notarized bill of sale.

- Wrong payment. No cash. No checks on reinstatements.
For mailed packages especially, a single missing item can put you on the wrong side of the tax deadline. In person is slower to get to but faster to resolve.
