How to Fill Out and Submit the Maine Road Test Request Form

To submit the Maine road test request form, fill out the road test request card printed on the back of your learner’s permit and mail it to the Bureau of Motor Vehicles main office at 29 State House Station, Augusta, ME 04333-0029. The BMV processes the card centrally and mails you back an appointment notice with your date, time, and testing location. There is no online option, and local branch offices do not schedule road tests.1Maine Secretary of State. Drivers License Exam

Confirm You’re Eligible Before You Mail

The request card only works if you’ve met the permit-holding and practice requirements for your age. Sending it in too early will not speed anything up.

  • Under 18: complete a state-approved driver education course before applying for the permit, hold the permit at least six months, and log 70 hours of supervised practice driving with 10 of those hours after dark.2Maine State Legislature. Applicants under 21 must complete and certify the BMV’s Permittee Driving Log (Form MVE-21) documenting the 70 practice hours, including the 10 hours after dark. A parent, step-parent, guardian, or spouse signs the form to certify the hours; if none of those are available, an employer can sign instead.4Maine Bureau of Motor Vehicles. Permittee Driving Log
