The Maryland ICD-071 certified statement is the form you send the Motor Vehicle Administration to swear, under penalty of perjury, that your registered vehicle sat parked and undriven during a period when its insurance lapsed. If the MVA has assessed an insurance violation penalty against you and the lapse genuinely happened but the car never moved, a properly completed ICD-071 with witness signatures and supporting proof can get that penalty reduced or eliminated.1Cloudfront. One boundary first. If your insurance was actually in force during the dates the MVA flagged, ICD-071 is the wrong form. Section 1: Vehicle and Owner Information

- Tag number (license plate)
- Title number from your Maryland certificate of title
- Vehicle identification number, the 17-character VIN on the dashboard or door jamb
- Year and make of the vehicle
- Vehicle owner’s name, as printed on the registration
- Owner’s Maryland driver’s license number
- Case number from the MVA violation notice
- Insurance cancellation date reported by your insurer
- Name of the insurance company
Copy each entry character-for-character. A wrong digit in the tag number, VIN, or case number can send your form back or delay the review while staff try to match it to a case.
Section 2: The Certified Statement
This is where you state the substance of your defense. In Section 2 you declare that the vehicle was not driven, was not involved in an accident, and was not issued a citation at any point during the lapse.1Cloudfront. - The start date and end date of the lapse period
- The street address, city, state, and zip code of where the vehicle was parked during those dates
- The reason the vehicle was uninsured
Be specific about the reason. The car was in a shop for engine repairs. It was in storage while you were overseas for work. It sat in a family member’s driveway after your policy nonrenewed. Vague answers leave the reviewer guessing.
Owner Signature
Under the statement, the owner or co-owner signs and dates the form and provides a daytime phone number. The signature block certifies, under penalty of perjury pursuant to Section 12-109(b)(2) of the Maryland Vehicle Law, that everything on the form is true and correct.1Cloudfront. Witnesses
The form has two witness lines. Witness A may be either an individual or a repair facility; a repair facility must supply its business license number. Witness B is a second individual. Witnesses sign under penalty of perjury too, certifying the owner’s statements are true to the best of their knowledge. Pick people with firsthand knowledge: a neighbor who saw the car in your driveway daily, a relative who can confirm you were out of state, the mechanic whose shop held the vehicle. Someone who only heard you say the car wasn’t driven does not qualify.
What to Attach
Section 2 references “supporting documentation attached.” The form itself does not spell out required items, but the purpose is to back up both the reason for the lapse and the claim that the vehicle was parked. Useful attachments include:
- A repair invoice showing dates the vehicle was in the shop
- Dated photographs of the car in a garage or storage lot
- Travel records such as boarding passes or hotel receipts covering the lapse dates
- A signed statement from a storage facility
A form submitted with nothing attached rests entirely on the owner’s sworn word and the witnesses’ credibility. Documentation strengthens the case for adjustment.

Watch the deadline on your violation notice. If it’s close, mail is faster than trying to reach a branch. Keep a full copy of what you sent and a proof of mailing, such as a certified mail receipt, in case the MVA later says it never arrived.
The Penalties at Stake
For the first 30 days without coverage, the MVA assesses a flat $200 penalty per vehicle. Beginning on day 31, the penalty grows by $7 for each additional day the lapse continues, up to $3,500 per violation within a 12-month period. The registration is also suspended as of the lapse date, effective no later than 60 days after the MVA is notified.3Maryland General Assembly. The ICD-071 does not remove the underlying obligation to carry insurance on a registered vehicle. It gives the MVA a basis to lower the money penalty when the facts show the car sat still.
After You Submit
MVA staff review your certified statement, the attachments, and the witness attestations. The “MVA Use Only” portion of the form includes fields for the reviewer to check whether you had any moving violations or accidents during the lapse period and whether you have a prior compliance case on file. If records turn up a citation or crash tied to the vehicle within the dates you swore it was parked, the statement is contradicted and the adjustment will likely be denied. The MVA does not publish a set turnaround for ICD-071 review, so keep watching the deadline on your original notice.
