What to Gather Before You Fill Anything Out

Set these four things on the table in front of you:

  • Your winning ticket, signed in ink on the back. An unsigned ticket is a bearer instrument, meaning whoever holds it can claim it. Sign it now.
  • A valid, unexpired government-issued photo ID. A Michigan driver’s license, state ID card, or U.S. passport all work.
  • Your Social Security card. In person, the original is required. By mail, a legible copy is acceptable.
  • The completed Ticket Receipt Form itself.

Before anything leaves your hands, photograph or photocopy the front and back of the signed ticket and the completed form. If the envelope disappears in the mail, those images are your only proof that the ticket existed.2Michigan Lottery. How to Claim Prizes by Mail

Some authorized retailers keep copies on hand, but availability varies.

How to Fill Out the Form

The form asks for straightforward personal and ticket information. What matters is that every field matches your ID documents exactly. Mismatches are the usual reason claims get held up.

  • Full legal name, exactly as it appears on your government-issued ID. If your Social Security card shows a different name — a maiden name, for example — you may be asked for documentation connecting the two.
  • Current residential address. This is where correspondence and, if applicable, your check will be sent.
  • A daytime phone number lottery staff can use if a question comes up.
  • Your Social Security Number. This is used for tax reporting. Leaving it off can trigger backup withholding at a higher rate.3Internal Revenue Service. Instructions for Forms W-2G and 5754
  • The ticket serial number, copied exactly from your ticket.
  • The tax declarations section, covering your citizenship status and whether you are subject to backup withholding. Answer truthfully; the lottery uses your answers to set your withholding rate.

Sign and date the bottom. An unsigned form can result in nonpayment. Print legibly throughout. A misread digit in your Social Security Number or ticket serial number turns a two-week claim into a two-month one.

Use certified or registered mail with tracking. You are dropping an irreplaceable winning ticket into an envelope, so treat it that way. For prizes of $600 and below, a written request with your name, address, and phone number attached to the signed ticket is enough on its own, though including the form still speeds processing.2Michigan Lottery. Submitting in Person at a Claim Center

The advantage of filing in person is immediate verification. If something is wrong with your paperwork, staff can catch it while you are still there instead of mailing a rejection weeks later.

If You Are Claiming as a Group

Pool tickets follow a different track. The group has to be established as a “lottery club” with the Bureau of State Lottery, and the Bureau recommends drafting bylaws and appointing a representative before any prize needs to be claimed. Setting a club up after a big win, when tempers and lawyers appear at the same time, is much harder than doing it in advance.5State of Michigan. Starting a Lottery Club

For a group prize, the club representative files a Substitute 5754 Form instead of the individual Ticket Receipt Form. The Substitute 5754 requires the name, address, date of birth, phone number, Social Security Number, and signature of every member receiving a share. The lottery then issues separate payments and W-2G tax forms to each member.5State of Michigan. What Happens After You Submit

Staff verify the ticket and match your identity to the form. For prizes of $1,000 or more, Michigan law requires the lottery to check state databases for outstanding debts before releasing any money. The Bureau looks for liabilities owed to the Department of Treasury (including delinquent state taxes and past-due court debts assigned to the state for collection), support arrearages, unemployment insurance restitution obligations, and debts owed to the Department of Human Services.6Michigan Legislature. If a debt turns up, the Bureau deducts what is owed from your prize and forwards that portion to the relevant state agency. You receive the balance. Approved prizes are paid by state check sent through the mail. Processing time varies with prize size and claim volume. If several weeks pass with no update, call Player Relations at (517) 373-1237.

Why Your Check Will Be Smaller Than the Prize

The lottery withholds 24% in federal income tax from any prize where winnings minus your wager exceed $5,000.7Internal Revenue Service. Instructions for Forms W-2G and 5754 Michigan taxes lottery winnings as ordinary income at the state’s flat individual income tax rate, which is 4.25% for 2026, and typically withholds that amount from larger prizes as well.8Michigan Department of Treasury. Anonymity When You File

Michigan grants automatic anonymity to winners of more than $10,000 on state-only lottery games. You do not need to request it. You can waive it voluntarily if you want to go public.9Michigan Lottery. Winner Resources

Multi-state games work differently. For Powerball, Mega Millions, and Millionaire For Life, Michigan law defers to those games’ own rules, which allow winner names to be released.9Michigan Lottery.
