How to Fill Out and Submit the Missouri Medicaid Prior Authorization Form

Pick the Right Form for the Service

MO HealthNet uses different forms and different intake channels depending on what’s being authorized.

  • Prescription drugs. Use the Drug Prior Authorization form (886-3003), available as a fillable PDF from the MO HealthNet Division. This is required for non-preferred formulary drugs, brand-name drugs when a generic equivalent exists, and specialty medications such as growth hormones and hepatitis C treatments.3Missouri Department of Social Services.   • Durable medical equipment. DME items that require authorization are listed in Section 19 of the DME provider manual. How to Complete the Drug Prior Authorization Form

The Drug Prior Authorization form has three working sections. The clinical middle section is where most denials originate, so it deserves the most care.

Patient Information

Enter the participant’s MO HealthNet number, full name, and date of birth. The MO HealthNet number is the identifier that links your request to the participant’s active coverage file. Mark whether this is an initial request or a renewal; renewals use the same form, but the reviewer will look for evidence that the medication continues to be necessary.1Missouri Hospital Association. Clinical and Medication Details

Provide:

  • The diagnosis and its ICD-10 code. The diagnosis must be consistent with a medically accepted use of the requested drug. A mismatch triggers a denial.
  • The date the diagnosis was established.
  • The requested drug name, dosage form, strength, and dosing schedule.
  • Whether the patient is currently taking the drug and, if so, the date it was first used.
  • The duration of need.
  • The patient’s full current drug regimen, with dosing schedules for every medication.
  • Every previously tried alternative, including dose, schedule, and how long the patient used it, plus a specific clinical explanation of why each was discontinued or not used.
  • If you are requesting a brand-name drug, when the generic was tried, for how long, and the results. If the generic was never tried, explain why.

The alternative-medication section is where the reviewer spends the most time. A vague explanation such as “patient preferred brand” almost always gets denied. Give specific clinical reasons: adverse reactions, documented treatment failure, contraindications with other medications in the regimen.1Missouri Hospital Association. Prescriber Information and Signature

The requesting physician or advanced practice nurse must fill in name, title, telephone, fax, address, specialty, and National Provider Identifier (NPI). Confirmation and Decision Timelines

Missouri law requires the reviewer to send back a unique confirmation number when it receives your request, using the same method you used to submit unless you asked in writing for something different.5Missouri Revisor of Statutes. For managed care plans, federal regulations set the outer limits on how long a decision can take. Starting with rating periods on or after January 1, 2026:

Either timeline can be extended by up to 14 additional calendar days if the patient or provider asks for the extension, or if the plan needs more information and can show the extension serves the patient’s interest.6eCFR. 42 CFR 438.210 – Coverage and Authorization of Services In practice the extension usually means the reviewer needs more clinical records, and the clock pauses until you send them.

The decision notice goes to both the provider and the participant. An approval will state when or under what circumstances the authorization expires.7CMS.gov. CMS Interoperability and Prior Authorization Final Rule

If the Request Is Denied

A denial notice must state the specific reason. The usual reasons are insufficient documentation of medical necessity, a lower-cost alternative the patient hasn’t tried, or a diagnosis that doesn’t match a medically accepted use of the requested drug or service. Fixing the underlying gap and resubmitting is often faster than appealing, but the participant also has a formal appeal right.

Requesting a State Hearing

Participants can request a state administrative hearing through the Missouri Department of Social Services.8Missouri Department of Social Services. Keeping Benefits During the Appeal

A participant already receiving the service or medication can keep it going during the appeal, but only by filing the hearing request within 10 calendar days of the adverse action notice.10DSS Manuals. At the hearing, the participant can represent themselves or bring a lawyer, family member, or friend to speak for them. Broader Coverage for Children Under 21

Children on MO HealthNet have a wider coverage guarantee than adults through the federal Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) benefit. Under EPSDT, states must cover any Medicaid-coverable service, in any amount, that is medically necessary for a child, even if the service is not part of the state’s standard benefit plan.12MACPAC. The practical effect: a denial based on “service not covered” has a much higher bar for a child. If a screening identifies a medical need, treatment must follow, and screenings can happen outside the routine schedule whenever a need is identified. One limit worth knowing: a family that goes to a non-network provider or a provider who doesn’t participate in MO HealthNet, without prior approval, will generally be responsible for the bill except in emergencies.
