How to Fill Out and Submit the Nevada DMV-301 Certification of Attendance

To complete the Nevada DMV 301 Certification of Attendance, a teen applicant fills in the student information block at the top, then either takes the form to their school principal to sign Section 1 or has a parent sign Section 2 if the student is home-schooled, exempt from compulsory attendance, or already holds a diploma or equivalency. The signed original goes to the DMV with the rest of the permit or license application, and it must reach the DMV within 60 days of the signature date.¹

Every applicant between 14 and 17 who wants an instruction permit, restricted license, or full driver’s license in Nevada has to submit this form, whatever type of school they attend.¹ You can download it as a fillable PDF from dmv.nv.gov/pdfforms/dmv301.pdf, or pick up a paper copy at your school’s front office or any DMV location. Print it before signing. The DMV wants original ink signatures.²

Which Section You Fill Out

The form has one top block that everyone completes and two numbered sections below it. You use one section, not both.

Section 1 is for students currently enrolled in a public or private school. Section 2 is for students who are home-schooled, legally exempt from compulsory attendance, or who have already earned a high school diploma or a Certificate of High School Equivalency such as a GED, HiSET, or TASC.² If a school official signs Section 1, leave Section 2 blank.

Filling In the Student Information Block

At the top of the form, enter the applicant’s last, first, and middle name, date of birth, driver’s license or instruction permit number if one already exists, and the student ID number assigned by the school. The name must match the applicant’s birth certificate or other identity document exactly. Any mismatch will get the form kicked back.¹

The form asks for a student ID number, not a Social Security number. You will still need proof of your Social Security number when you go to the DMV, but that is a separate document.²

Section 1: Getting the School to Sign

Section 1 is labeled “School Use Only.” Take the partially completed form to your school and give it to the principal or another designated school official. The official is certifying that you meet the minimum attendance requirements set by your school district under NRS 392.122. They sign, print their name, write the school name, and date the form.

If you don’t currently meet the attendance minimums, the principal still has room to help. Under NRS 392.123, they can sign the form anyway if they determine a hardship exists and driving would be in your or your family’s best interest.³ That determination is the principal’s call, not the DMV’s.

Private school students use Section 1 the same way public school students do. The private school’s principal or designated official signs off on attendance.

Section 2: Home-Schooled, Exempt, or Already Graduated

Section 2 is for applicants who don’t have a school principal to sign Section 1. A parent or legal guardian fills out and signs the first part of Section 2, certifying that the student is exempt from Nevada’s compulsory attendance laws under NRS 392.050 through 392.070.¹

For a home-schooled teen, the parent’s signature only works if the family has already filed a Notice of Intent to Homeschool with the superintendent of schools for the district where the child lives, as required by NRS 388D.020.⁴ That notice is a one-time filing. It only needs to be refiled if the parent’s or child’s name or address changes, or if the child returns to homeschooling after a stretch in public school.⁵ After you file, the district sends back a written acknowledgment. Keep it. If a DMV technician questions the parent’s authority to sign Section 2, that letter clears it up.

If the applicant has already graduated or earned a GED, HiSET, or TASC, check the appropriate box in Section 2 and bring a copy of the diploma or equivalency certificate to the DMV. The form alone is not enough in that case, and neither is the certificate alone. You need both.²

The 60-Day Clock

The DMV 301 expires 60 days after it is signed and dated.¹ The clock starts on the date the school official or parent signs, not on the date the student information was filled in. If your DMV appointment moves, or you fail the knowledge test and have to come back, or anything else pushes you past day 60, the form is dead and you’ll need a fresh one signed.

A simple sequence avoids this: book the DMV appointment first, then get the form signed a week or two before the appointment date. Nevada DMV wait times can run several weeks, so trying to line up the signature and the appointment in the reverse order is how forms expire.

Bringing the Form to the DMV

Take the original, signed DMV 301 to your scheduled appointment. Not a photocopy. Not a phone photo. The DMV requires ink signatures to guard against fraud.² A technician will check that the signatures are present, that the school official’s information is complete for Section 1 filings, and that the name and date of birth match the rest of the packet.

The DMV 301 is one document among several you’ll hand over for a teen instruction permit. Alongside it, you need proof of identity such as a certified U.S. birth certificate, proof of your Social Security number, two documents showing Nevada residency, the Application for Driving Privileges or ID Card (DMV 002), and the Minor Affidavit and Information Sheet (DP-38) signed by a parent or guardian accepting financial responsibility. The licensing fee is $22.50 and the testing fee is $25, which covers the initial knowledge and skills tests.² Applicants under 25 who are experiencing homelessness can request a fee waiver by submitting a Declaration of Homeless Status (DMV 128).

If There’s a Truancy Finding on Record

The DMV 301 also intersects with Nevada’s truancy rules. Under NRS 392.148, a school police officer or designated official can order the suspension of a habitual truant’s license, or bar an unlicensed student from applying, after an investigation and hearing.³ Once a suspension period ends, the student needs a new DMV 301 showing current attendance compliance before the DMV will restore driving privileges. In other words, an old form signed before the truancy finding won’t get you back on the road; go back to the school and have Section 1 signed again.
