How to Fill Out and Submit the NJ LCSW Hours Tracking Form

The NJ LCSW hours tracking form is officially titled “Documentation of Supervised Clinical Experience for LCSW Candidates,” and it is the only document the New Jersey State Board of Social Work Examiners accepts to verify the 3,000 hours of post-master’s clinical work required for licensure. You download it as a PDF from the Division of Consumer Affairs LCSW application page, have each supervisor complete and sign it, and upload it through the MyLicense online portal along with several supporting documents. There is no separate “clinical hours tracking form” or “certification of social work experience” variant — this one form is it.

Where to Get the Form

The form is posted on the Division of Consumer Affairs LCSW page as a downloadable PDF. Print or fill it electronically, but plan on a physical or digital signature from each supervisor who oversaw your hours. If you worked under more than one supervisor during your accumulation period, you need a separate completed form for each, because every supervisor certifies only the hours they personally oversaw.

What the Form Asks You to Fill In

The form is organized around your employment history during the supervision period. For each position, you provide:

  • Your full legal name, LSW license number, and contact details.
  • The name, address, and phone number of the agency or practice where you worked.
  • Exact start and end dates for the position. If you changed jobs during your supervision period, each employer gets its own section.
  • Hour totals broken out by category: face-to-face client contact, psychotherapeutic counseling specifically, and other clinical social work activities.
  • Your supervisor’s name, LCSW license number, dates of supervision, and signature verifying the accuracy of the hours and the quality of your clinical work.

The hour totals are the section applicants most often get wrong, so it’s worth understanding how the board reads them before you write anything in.

How to Categorize Your Hours

The 3,000 total hours must be completed in no fewer than two years and no more than four years after your master’s degree. Within that total, the board evaluates three categories separately, and your numbers on the form need to reflect that separation.

  • Face-to-face client contact: at least 1,920 hours of direct sessions involving assessment, diagnosis, and treatment.
  • Psychotherapeutic counseling: at least 960 hours, drawn from within that 1,920. Intake assessments and case coordination don’t count toward this subset even though they count toward face-to-face contact.
  • Other clinical social work services: up to 1,080 hours covering supervision sessions, case management, record keeping, consultation, and similar activities that support your clinical work without direct client contact.

If you tracked psychotherapy separately from other direct contact all along, transferring the numbers is straightforward. If you didn’t, reconstruct them honestly before you put anything on the form; entering 1,920 in the client contact field without being able to defend a 960-hour psychotherapy subtotal will hold up your application.

You also must have held an LSW during the accumulation period, or have worked in a setting exempt from licensure under N.J.S.A. 45:15BB-5, such as a government agency, school, or hospital.

Supervisor Sign-Off

Your supervisor’s signature only counts if they meet the board’s requirements. Confirm each of the following before asking them to sign:

  • They hold an active New Jersey LCSW license and have held it for at least three years.
  • They have completed at least 20 continuing education credits of post-graduate coursework in clinical supervision from a board-approved program. A Seminar in Field Instruction (SIFI) course does not satisfy this, even partially.
  • They provided at least one hour of face-to-face individual or group supervision per week. Video conferencing is allowed for up to half the total supervision hours if it meets HIPAA confidentiality standards.
  • They supervised no more than six social workers pursuing LCSW hours at once, and no more than four in any single group session.

If your supervisor has left the agency, become incapacitated, or is otherwise unavailable, the board allows what its eligibility rules call “the applicant’s affidavit in lieu of a supervisor’s certification” — a sworn statement attesting to your experience. Contact the board directly before relying on this route, since they may have specific requirements for what the affidavit must address.

Attachments You Must Submit With the Form

The form on its own will not get your experience approved. The board expects the following alongside it:

  • A copy of each supervisor’s board-approved 20-hour clinical supervision course completion certificate. The board publishes a list of approved courses on its website.
  • The official job description for every title you held during your supervision period, printed on agency letterhead as issued by your employer’s HR department. A letter drafted by a supervisor from memory does not substitute for the HR-issued description.
  • If any supervisor was not employed at your workplace, a letter from your employer on official letterhead, signed and with contact information, confirming your approval to discuss casework with the outside supervisor. You also need written client consent to share records with an off-premises supervisor.
  • An official transcript showing your master’s degree in social work from a CSWE-accredited program, or a doctorate in social work, sent by your school directly to the board at DCA_Social_Work_No_Reply@dca.njoag.gov.
  • A completed Certification and Authorization Form for the criminal history background check, which triggers separate instructions from the board.

The HR job description trips people up. If you’ve changed roles or your agency has restructured, track down the original description that was in place when you were hired for each title, not a rewrite.

Submitting Through MyLicense

Create an account at newjersey.mylicense.com and complete the LCSW application online. The $75 application fee is assessed at the end of the online form. Upload the completed Documentation of Supervised Clinical Experience form and all supporting attachments through the “Upload License Documents” tab on your profile.

Third-party documents — your official transcript and any out-of-state license verifications — must come directly from the issuing institution to DCA_Social_Work_No_Reply@dca.njoag.gov. If another state’s licensing board does not provide verifications, upload a letter to your profile explaining that and link to the page on that board’s website where the policy is stated.

The board processes applications in the order received and does not publish a turnaround estimate. Check your MyLicense profile regularly for status updates or requests for additional information.

After the Board Approves Your Hours

Once the board reviews and approves your documentation, you receive authorization to sit for the ASWB Clinical exam. Since February 2021, New Jersey has required that all clinical hours be completed before the board will approve you for the exam, so you cannot test while still accumulating hours. The registration fee is $260.

If you sat for a previous attempt in another state, request an official ASWB score transfer instead. ASWB reports scores to state boards within seven to ten business days, and the board issues your LCSW license once your passing score is confirmed and every other application requirement is satisfied.
