Where to Find the Form

The appeal request form is a fillable PDF on the Oklahoma Complete Health website, under Members, on the complaints and appeals page.3Oklahoma Complete Health. Grievances, Appeals and State Fair Hearing If you cannot reach the website, call Member Services at 1-833-752-1664 (TTY: 711) and ask them to mail you a copy or take your appeal over the phone.

Providers filing on their own behalf use a separate provider appeal form, found on the provider resources page; that form covers claims that were already rendered and denied.4Oklahoma Complete Health. Manuals, Forms and Resources A provider filing on a member’s behalf uses the member form with the member’s written consent.

What to Have in Front of You Before You Start

Gather these items before you open the PDF so you can finish it in one sitting:

  • Your adverse benefit determination notice. The date printed on this notice starts your 60-day filing clock, and the stated reason for denial is what you’ll be arguing against.
  • Your full legal name and Medicaid ID number, copied exactly from your SoonerSelect ID card. A transposed digit is the fastest way to get a form kicked back.
  • The denied service’s name, procedure code or prescription name, and claim number if one was assigned.
  • Supporting medical records: recent clinical notes, test results, and a letter of medical necessity from your treating provider.
  • Your doctor’s name, phone, and fax, so the appeals team can reach the office for clinical clarification.

If anyone other than you or your treating provider is filing, that person must submit a signed Appointment of Representative form along with the appeal. Without it, the plan cannot discuss the case or share your health information with them.5Oklahoma Health Care Authority. Oklahoma Administrative Code 317:2-3-5 – Member Appeals

You also have the right to review your case file for free while the appeal is pending, including the records and documents the plan used to make its decision.6eCFR. 42 CFR 438.406 – Handling of Grievances and Appeals If you suspect the plan denied the service because records were missing, request the file before writing your narrative.

How to Fill Out the Form

Start with the member identification section at the top: name, Medicaid ID, date of birth, and contact information. Copy these from your SoonerSelect ID card rather than typing from memory.

In the service section, enter the claim number, the date of the denial notice, the type of service or prescription, and the ordering provider. Match these fields word-for-word to the denial letter.

The form asks you to choose standard or expedited review. Choose standard unless waiting the full timeline would seriously jeopardize your life or health. An expedited appeal must be resolved within 72 hours; a standard appeal has up to 30 calendar days.7eCFR. 42 CFR 438.408 – Resolution and Notification If you request expedited review and the plan decides your situation does not qualify, it will process the appeal under the standard timeline and tell you.

The narrative section is where the appeal is won or lost. Do not write “I need this medication.” Tie your argument to specific items in your records: the diagnosis, the test result, the treatments you already tried that failed, and your doctor’s recommendation. If the denial reason was “not medically necessary,” explain in plain terms why your doctor believes it is. Attach the supporting documents and name them in your narrative so the reviewer knows what to open.

Mark any section that does not apply “N/A” so the reviewer knows you did not skip it by accident. Sign and date at the bottom. The form must be received within 60 calendar days of the date on the denial notice, or the appeal will be dismissed.1Oklahoma Complete Health. Appeal Request Form

Keeping Your Benefits While the Appeal Is Pending

If the denial cuts off, reduces, or ends a service you were already receiving under an existing authorization, you can ask the plan to keep providing it while your appeal is reviewed. Make the request within 10 calendar days after the plan mails the denial notice, or before the effective date of the denial, whichever is later.8eCFR. 42 CFR 438.420 – Continuation of Benefits

This right applies only to services you were already getting. It does not cover brand-new requests the plan has never authorized. Ask for continued benefits at the same time you file the appeal. If you win, services continue uninterrupted. If you lose, the plan may bill you for services provided during the appeal period, so weigh that risk before asking.

What Happens After You Submit

For a standard appeal, the plan has up to 30 calendar days from receipt to decide and notify you in writing. For an expedited appeal, the deadline is 72 hours, and the plan will contact you within 24 hours to confirm it received your expedited request.2Oklahoma Complete Health. Member Handbook The plan can extend the standard timeline by up to 14 additional calendar days if it needs more information, and it must tell you the reason.5Oklahoma Health Care Authority. Oklahoma Administrative Code 317:2-3-5 – Member Appeals

Someone who was not involved in the original denial reviews the appeal, and if the question is clinical, the reviewer must have the right clinical expertise. Any new documents you submit must be considered, even if they were not part of the first review.

If the plan reverses the denial, it must authorize or provide the service no later than 72 hours after the reversal.9eCFR. 42 CFR 438.424 – Effectuation of Reversed Appeal Resolutions You do not need to re-request it.

If the Plan Still Says No

When the plan upholds the denial, the written resolution notice will explain your right to request a state fair hearing through the Oklahoma Health Care Authority. A fair hearing is an independent review by a hearing officer outside the plan.

You have 120 calendar days from the date on the plan’s appeal resolution notice to request a fair hearing.10Oklahoma Health Care Authority. Oklahoma Administrative Code 317:2-3-12 – State Fair Hearing for Members You must finish the plan’s internal appeal first; you cannot skip to a fair hearing while the internal appeal is still open.11eCFR. 42 CFR Part 438 Subpart F – Grievance and Appeal System You can request the hearing through the plan or directly through OHCA. If the hearing officer reverses the denial, the plan must authorize the service within 72 hours.
