How to Fill Out and Submit the Oklahoma Driving Record Request Form

To request a driving or vehicle record in Oklahoma, fill out Service Oklahoma’s Form 303RM-M, identify a permissible reason for the request under the federal Driver’s Privacy Protection Act, sign the affirmation, and submit it with payment by mail or in person at a Service Oklahoma location or tag agency. If you only need your own driving record summary, you can skip the paper form and use the online portal instead. The Oklahoma driving record request form covers both driving history summaries and vehicle title and registration records, and Service Oklahoma will not release anything until it can match your request to an authorized use.

What Records the Form Covers

Form 303RM-M handles two categories. The first is the driving record summary, often called a Motor Vehicle Report or MVR, which shows license status, traffic violations, and related history. The second covers vehicle-specific records: certificates of title, registration files, and ownership history stored in the state database.

Oklahoma law treats title applications, registration records, and related computer files as confidential, and only authorized requesters can access them.¹ Individual MVRs can also be pulled online, but broader vehicle record searches and business-volume requests must go through the paper form.²

Who Is Allowed to Request Records

The federal Driver’s Privacy Protection Act bars state motor vehicle agencies from releasing personal information (names, addresses, phone numbers, Social Security numbers, photos, and medical or disability details) unless the requester qualifies under a permissible use.³ General vehicle data like accident history, driving violations, and license status is not “personal information” under the statute, so it is easier to obtain.

The permissible uses that most often apply:⁴

Government functions carried out by an agency, court, or law enforcement body, or a private person acting on its behalf.
Use in a civil, criminal, or administrative proceeding, including service of process, pre-litigation investigation, and enforcing judgments.
Insurance claims investigation, anti-fraud work, rating, or underwriting.
Employer verification of a commercial driver’s record as required by federal motor carrier safety rules.
Work by a licensed private investigative agency or security service for a purpose otherwise permitted under the statute.
A business verifying the accuracy of personal information a customer submitted, limited to fraud prevention, pursuing legal remedies, or recovering a debt.
Matters involving motor vehicle safety, theft, emissions, product recalls, or manufacturer performance monitoring.

If none of these fit, the person named in the record can sign the written consent section on the form to authorize release.² Employers pulling a non-commercial driver’s record must also get written consent from the employee before Service Oklahoma will process the request.⁵

Filling Out Form 303RM-M

The form is organized into labeled sections. An incomplete submission will not be processed, so match your situation to the right block before you sign anything.

If You Are Requesting as an Individual

Start with the section for the subject’s identifying details. Provide the person’s full name, date of birth, and either their driver license number or the vehicle identification number, depending on whether you want a driving record or a title and registration history. More identifiers mean a faster match.

If You Are Requesting on Behalf of a Business

Businesses, insurance companies, law firms, and licensed investigators complete a separate section covering the organization’s name, the requestor’s title within the organization, and the type of business. Then check the box in the “Reason for Request” section that matches your permissible use. Insurers check the claims investigation and underwriting box; employers of commercial drivers confirm the federal motor carrier safety basis; private investigators indicate their purpose falls under an authorized DPPA subsection.²

The Affirmation

Every requester signs the affirmation at the bottom. By signing, you declare under penalty of perjury (under Oklahoma Statutes Title 12, Section 426) that the records are being requested solely for the reason you checked. You also acknowledge the information is confidential under federal and state law and agree to indemnify Service Oklahoma against liability for misuse.² If you share the records with another authorized person, you are responsible for informing them of their own DPPA obligations.

Fees and Payment

A standard driving record summary costs $25. A certified MVR is $28, which is the base fee plus a $3 certification charge.⁶

Vehicle record searches are priced by statute. A computer-based search is $5 per vehicle. A manual search is $7.50 per vehicle. Certified copies of title documents and applications are $10.¹

Mail-in payments can be made by cashier’s check, money order, or personal or business check. Cash is only accepted for in-person visits. The agency does not send records C.O.D.⁷

How to Submit the Form

By Mail

Include a self-addressed, stamped envelope for the return. FedEx and UPS cannot deliver to the P.O. Box, so avoid those carriers.⁷

In Person

You can drop off the form and pay at any Service Oklahoma location or licensed operator (tag agency) in the state. For a standard MVR, any tag agency can usually pull the record on the spot if it is available in the system.⁷

Online, but Only for Your Own MVR

If you need your own driving record summary, skip the paper form. Service Oklahoma runs an online portal at https://pay.apps.ok.gov/dps/mvr/app/individual/individual_start.php where you can request and pay for an individual MVR electronically.² The portal does not handle vehicle title searches, bulk requests, or records sought on behalf of a business. Those still require the paper form.

What Will Not Work

Service Oklahoma does not accept record requests by phone, email, or fax. It can, however, return completed records to you by encrypted email if you include an email address with your request.²

What to Expect After You Submit

In-person requests for standard MVRs are often handled on the spot because those records are stored electronically. Vehicle title histories and any request that needs manual file retrieval take longer. For mail-in requests, plan on at least two to three weeks from the day you mail the form to the day the records come back, and longer during busy periods.

If your request is missing information or does not identify a valid permissible use, the agency will notify you of the problem rather than silently denying it. For questions about a pending request, you can reach the records division at sokrecords@service.ok.gov.

Penalties for False Statements or Misuse

Oklahoma law makes it illegal to knowingly obtain or disclose personal information from a motor vehicle record for any purpose not authorized by state statute, or to make a false representation to get that information.¹ Federally, anyone who knowingly violates the DPPA faces criminal fines.⁸ The affirmation on Form 303RM-M is a sworn declaration, and Service Oklahoma holds you personally responsible for any downstream misuse of records you receive.
