How to Fill Out and Submit the Oregon OMMP Application Form

An Oregon OMMP application has four core pieces: the application form itself, an Attending Provider’s Statement signed within the last 90 days, a copy of your government-issued photo ID, and the fee (starting at $200, with reduced tiers as low as $20). You submit the package through the Oregon Medical Marijuana Online System at ommpsystem.oregon.gov or by mail to the OMMP office in Portland. Everything else on the form depends on whether you’re designating a caregiver, a grower, or a grow site, and whether the applicant is a minor.

What You Need Before You Start

First, your condition has to qualify, and your attending provider has to say so in writing. Any Oregon-licensed physician, naturopath, or nurse practitioner who has examined you and reviewed your medical records can sign the Attending Provider’s Statement.1Oregon Health Authority. Attending Providers They don’t need to be a specialist. Clinics that focus on these evaluations typically charge $50 to $100; your regular doctor can sign at no extra charge if they’re willing.

The signature date matters. The Attending Provider’s Statement must be signed within 90 days of the date you submit your application, or OMMP will reject it.2Oregon Health Authority. You also need to be an Oregon resident. The statute requires proof of residency in a form the Oregon Health Authority accepts by rule.3Oregon State Legislature. Oregon Code 475C.783 – Registry Identification Cardholders; Eligibility; Fees; Rules A current Oregon driver’s license or state ID card is the straightforward way to cover both identity and residency. Filling Out the Form

Download the current form from the Oregon Health Authority website or work through it inside the online system at ommpsystem.oregon.gov.2Oregon Health Authority. New Patients – Medical Marijuana The form has sections for patient information, caregiver and grower designations, and grow site details.

Enter your full legal name, date of birth, and residential address exactly as they appear on your photo ID. Mismatches between the ID and the form are a common reason applications get flagged as incomplete. Fill in the caregiver, grower, and grow site sections even if you plan to grow for yourself; the form needs to reflect your choice either way.

The medical section captures information from your Attending Provider’s Statement, including the provider’s name and license. Those entries have to match the attached statement exactly. Select the fee tier that matches your situation and check the box confirming you’ve included the supporting documentation for it.

Both you and any designated caregiver have to sign. An unsigned application is treated as incomplete. Use physical signatures if you’re mailing it in; the online system accepts digital signatures.

Caregiver, Grower, and Grow Site Sections

You may designate one primary caregiver to help you obtain and administer your medicine, and only one at a time. Their name and address go on the application.4Oregon Revised Statutes. ORS 475C.789 – Designated Primary Caregivers

If someone other than you is going to grow your plants, OMMP will run a criminal background check on that person, so include a copy of their photo ID along with their information.2Oregon Health Authority. New Patients – Medical Marijuana

When the grow site sits on property that you or your grower do not own, add a Grow Site Consent Form signed by the property owner. Following changes made in the 2025 legislative session, the owner’s signature must be witnessed by a notary public.2Oregon Health Authority. New Patients – Medical Marijuana You can skip this form if you own the property or aren’t designating a grow site at all.

For an applicant under 18, a custodial parent or legal guardian has to serve as the designated primary caregiver, and a Minor Declaration Form has to accompany the application.2Oregon Health Authority. New Patients – Medical Marijuana The provider statement requirements are the same.

Fees and What to Pay

The standard fee is $200 if you grow for yourself or aren’t listing a grower.5Oregon Health Authority. OMMP Cardholder Fees Reduced fees are available with current proof of enrollment in a qualifying program:

  • SNAP: $60
  • Oregon Health Plan: $50
  • Supplemental Security Income: $20

Each reduced tier applies to the same scenario (you grow or no grower is listed), and you must include current verification of your enrollment.5Oregon Health Authority. OMMP Cardholder Fees

One fee people miss: if you designate a grower other than yourself, there is an additional $200 grow site registration fee on top of the application fee.6Oregon Health Authority. How to Submit

Online is the faster route. Create an account at ommpsystem.oregon.gov, upload the completed application form, the Attending Provider’s Statement, photo ID copies, and any additional forms (grow site consent, Minor Declaration), then pay the fee electronically.2Oregon Health Authority. New Patients – Medical Marijuana If the application appears complete on initial review, you can print a 30-day receipt immediately.

To mail the package, send it to:

OHA/OMMP

Pay by check or money order made out to OHA/OMMP for the exact fee amount. Do not send cash.2Oregon Health Authority. New Patients – Medical Marijuana Mail submissions take longer because of transit time and manual intake.

After You Submit

Once OMMP staff do an initial review and your application appears complete, you receive a receipt letter. That receipt carries the same legal effect as a registry identification card for 30 days from the date printed on it, so you can access dispensaries during that window.6Oregon Health Authority. Frequently Asked Questions – Medical Marijuana The receipt covers the patient only, not a designated caregiver or grower.

Processing times move with application volume. As of early 2026, OMMP was reviewing applications and mailing cards within 7 to 10 business days of reviewing a batch.8Oregon Health Authority. Oregon Medical Marijuana Program The OMMP homepage shows which date range of applications is currently being processed and is updated regularly.

If anything is missing, OMMP sends an Incomplete Letter listing what’s needed. You have 14 days from the date on that letter to supply the missing materials, or the application may be rejected.2Oregon Health Authority. New Patients – Medical Marijuana A rejected applicant cannot reapply for six months.3Oregon State Legislature. Oregon Code 475C.783 – Registry Identification Cardholders; Eligibility; Fees; Rules The most frequent problems are an expired Attending Provider’s Statement, missing signatures, and names that don’t match between the application and the ID.

One Thing to Know Before You Apply

Federal law makes it illegal for an “unlawful user of or addicted to any controlled substance” to possess a firearm or ammunition.9Office of the Law Revision Counsel. 18 USC 922 – Unlawful Acts Marijuana remains a controlled substance under federal law even after the April 2026 rescheduling from Schedule I to Schedule III, and the underlying statute has not been amended. Anyone who uses marijuana regularly enough to qualify as an OMMP patient is likely considered an “unlawful user” under the current ATF interpretation, which defines the term as someone who uses a controlled substance with “sufficient regularity and recency to indicate active engagement.” Buying a firearm from a licensed dealer requires completing ATF Form 4473, which asks about controlled substance use; answering untruthfully is a separate federal felony. If you own firearms, weigh this conflict before you apply.
