How to Fill Out and Submit the SUNY EOP Financial Information Form

The SUNY EOP Financial Information Form is the document every Educational Opportunity Program applicant must complete to show they meet New York State’s income eligibility rules. You reach it through the SUNY application portal or the Common Application portal after you indicate interest in EOP, and it collects household income, dependency status, and public assistance details so campus EOP offices can decide whether you qualify. Getting it right, and sending the correct supporting documents with it, is what moves your file from “incomplete” to “under review.”

Where to Find the Form

How you access the form depends on which application you filed:

  • SUNY Application: After submitting your application and indicating interest in EOP, you’ll be directed to the online Financial Information Form inside the SUNY portal.
  • Common Application: Go to suny.edu/applysuny after submitting your Common App and follow the EOP Financial Information link.
  • Campus local application: A few campuses that use their own application provide a campus-specific PDF. These are posted on the SUNY EOP Financial Information Forms page.

The online version only has to be completed once. SUNY shares your financial data with every campus you selected for EOP consideration.1The State University of New York. EOP Financial Information Forms A campus-specific PDF, by contrast, generally needs to be sent to each school separately.

Income Limits for 2026–2027

Your household’s total annual income before taxes must fall at or below these limits for the 2026–2027 academic year:

  • 1 person: $28,953
  • 2 people: $39,128
  • 3 people: $49,303
  • 4 people: $59,478
  • 5 people: $69,653
  • 6 people: $79,828
  • 7 people: $90,003
  • 8 people: $100,178

For households larger than eight, add $10,175 for each additional person.2The State University of New York. SUNY Educational Opportunity Program The figures are gross income (everything before taxes and deductions) and cover wages, interest, dividends, pensions, and other earnings. Household size means everyone living in the home who receives financial support from the primary earners, including you.

When the Income Limits Don’t Apply

Some situations exempt you from the thresholds entirely. The income limits do not apply if:

  • Your family receives Family Assistance or Safety Net payments through the New York State Office of Temporary and Disability Assistance or a county Department of Social Services, or Family Day Care payments through the New York State Office of Children and Family Assistance.
  • You are in foster care as established by a court.
  • You are a ward of the state or county.

If any of these describe you, you still complete the form, but the income numbers aren’t measured against the thresholds above.3SUNY Ulster. Educational Opportunity Program (EOP)

Working Through the Form

The form is organized into numbered sections. Not every applicant fills in every section; your dependency status decides which parts apply.

Section 1: Personal Information

Name, address, date of birth, citizenship or permanent resident status, high school CEEB code, and the term you plan to enter. Check your date of birth carefully, because the form uses it to help determine whether you qualify as an independent student.4State University of New York. 2026 SUNY EOP Financial Information Form

Section 2: Financial Aid Application Status and Public Assistance

This section asks whether you filed the FAFSA and whether your family is primarily dependent on public assistance from Temporary Assistance to Needy Families (TANF), including Family Assistance, Safety Net, or cash grants. If you answer yes to the public assistance question, the form asks for your consent to let SUNY share your information with the Office of Temporary and Disability Assistance (OTDA) for verification, along with your Client Identification Number (CIN). It also asks whether you are in foster care or a ward of the state or county.4State University of New York. 2026 SUNY EOP Financial Information Form

Section 3: Dependency Status

This is where the form branches. You answer a series of yes-or-no questions. If any answer is yes, you’re classified as independent, the parent sections drop away, and the form focuses on your personal income alone.

You qualify as independent if any of the following are true:

  • You were born before January 1, 2003.
  • You are married.
  • You are on active duty in the U.S. Armed Forces for purposes other than training.
  • You are a veteran of the U.S. Armed Forces.
  • You have dependents (other than a spouse) who live with you and receive more than half their support from you between July 1, 2026 and June 30, 2027.
  • At any time since age 13, you were an orphan, a ward of the court, or in foster care.
  • A court in New York State determined you to be a legally emancipated minor or placed you in a legal guardianship with someone other than a parent or stepparent.
  • On or after July 1, 2025, you were unaccompanied and either homeless or self-supporting and at risk of homelessness, as determined by a school district homeless liaison, shelter director, federal TRIO or GEAR UP program director, or college financial aid administrator.

If none apply, you’re a dependent student and must report parent information in the sections that follow.4State University of New York. 2026 SUNY EOP Financial Information Form

Section 5: Parent Information (Dependent Students Only)

Enter your legal parents’ names, their relationship to each other (married, divorced, separated, never married, or widowed), and the relevant dates. If your parents are not married and don’t live together, report information for the parent you lived with more during the past 12 months. The income and tax data in later sections then come from that parent’s household rather than from both parents separately.

Income and Tax Sections

The form asks whether you (or your parents, if you’re dependent) filed a 2024 federal tax return and whether that return was joint with a spouse. You report income figures from the 2024 tax year. If the household received non-taxable income such as Social Security benefits, Supplemental Security Income, disability payments, child support, pensions, or unemployment benefits, the form collects those amounts as well. For any category that doesn’t apply, enter zero rather than leaving the field blank. A blank field can flag the form as incomplete.

Supporting Documents to Gather

The form is self-reported. Campus EOP offices verify it by asking for documentation covering the 2024 tax year. Pulling this paperwork together before you start saves time, because missing documents are the most common reason applications stall.

If a Tax Return Was Filed

Provide a signed copy of IRS Form 1040 with all schedules, or an official IRS tax return transcript. Transcripts are free through your IRS Individual Online Account at irs.gov, by calling the automated transcript line at 800-908-9946, or by mailing Form 4506-T. Online delivery is instant; mailed transcripts take 5 to 10 calendar days.5Internal Revenue Service. Transcript Types for Individuals and Ways to Order Them

If No Tax Return Was Filed

Submit all W-2 or 1099 forms received, along with an IRS Verification of Non-Filing Letter or a signed statement of non-filing. The non-filing letter is also free through irs.gov.

Submitting the Form and What Happens Next

Once your financial data is entered, you electronically sign and submit through the portal. That triggers a review at every campus EOP office where you applied. Supporting documents go up through the portal or get mailed directly to the campus admissions office, depending on the school’s instructions. If you used the online version, your financial information is automatically shared with every SUNY campus you selected for EOP consideration.1The State University of New York. EOP Financial Information Forms

If the EOP office spots discrepancies or needs more documentation, they’ll email the address on your student profile. Respond quickly. Delays in sending verification paperwork delay your admission decision. Once financial eligibility is confirmed, your academic records go to a separate review for program placement. Clearing the income test alone does not guarantee admission.

Transferring EOP Status Between Campuses

If you’re already enrolled in EOP at one SUNY campus and want to move to another, you don’t restart financial eligibility from zero, but you do need extra paperwork. The receiving campus requires an Educational Opportunity Program Transfer Verification Form, completed by the EOP director or a staff member at your current institution. It confirms your academic and income eligibility, the direct aid you received, and your overall participation.7SUNY New Paltz. Educational Opportunity Program Transfer Verification Form

The destination campus typically expects the originating institution to return the completed form within seven business days. You’ll also submit a new EOP Financial Information Form and provide proof of income for the year before admission, such as a 1040, Social Services verification letter, or Social Security award letter. Transfer applicants generally need a minimum cumulative GPA of 2.5 based on at least one year of full-time enrollment, though the strongest candidates have a 3.0 or higher.8SUNY Geneseo. Educational Opportunity Program Students coming from outside New York State who participated in a similar opportunity program must attach documentation showing they were both financially and academically disadvantaged when they were originally admitted.
