How to Fill Out and Submit the UIUC Immunization History Form

The UIUC Immunization History Form is the one-page record every incoming University of Illinois Urbana-Champaign student uploads to MyMcKinley before classes start. You download the PDF from McKinley Health Center, have a licensed provider fill in your vaccination dates and sign it, then submit it through the portal ahead of your semester deadline. Miss the deadline and McKinley places a hold that blocks course registration.¹

Vaccination Dates You Need to Show

The form is a container for specific dates required under the Illinois College Immunization Code. Before you print anything, gather records that show month, day, and year for each dose.²

Two doses each of measles, mumps, and rubella vaccine, both on or after your first birthday and at least four weeks apart.
For domestic students, at least one Td dose within ten years of your enrollment term. Tetanus Toxoid alone does not count.
For international students, three or more doses of diphtheria-tetanus-pertussis in any combination, with at least one being Tdap, and the most recent dose within ten years of enrollment.
One meningococcal conjugate dose on or after age 16, required only for new students under age 22.²

If dates are missing, a blood titer (serology) can prove immunity for measles, mumps, and rubella. It cannot substitute for the tetanus-diphtheria-pertussis requirement; tetanus titers are not accepted, so a missing Td or Tdap means getting a new dose.³ Vaccines received overseas need a certified English translation of the record.¹

Filling Out the Form

Download the Immunization History Form PDF from the McKinley Health Center forms page and print it.⁴ At the top, enter your full legal name and your nine-digit University Identification Number. Your UIN is the permanent record number that identifies you across every University of Illinois system; check your admission letter or student self-service account if you don’t have it in front of you.⁵

Take the printed form to a licensed healthcare provider. The provider transfers the vaccination dates from your medical records onto the form, signs it, and writes in the office address and phone number. McKinley requires the provider’s contact information on the form.¹

If getting the UIUC-specific form completed by your provider isn’t practical, you can attach copies of school immunization records, physician records, or military records instead. Make sure attachments are legible and show the same date-level detail the form asks for. Put your UIN on every page.

Uploading to MyMcKinley

Scan the signed form or take a clear photo, then upload it at mymckinley.illinois.edu using your university login. This is the primary submission channel McKinley directs students to.¹

If you can’t upload electronically, McKinley also accepts documents by fax at (217) 244-1278, by mail, or by hand delivery to McKinley Health Center at 1109 S. Lincoln Ave., Urbana, IL 61801. Email submissions are not accepted.³

After submission, review can take up to four weeks, especially during peak enrollment periods. You’ll get an email prompting you to check your compliance status in MyMcKinley once processing is done.³

Deadlines

The compliance deadlines are July 1 for fall semester and December 1 for spring semester.¹ Because processing can run four weeks, plan backward from those dates. Submitting on June 28 for a July 1 deadline is technically on time, but you’re betting that review finishes before the hold triggers. Aim for early June or early November instead.

What a Missed Deadline Costs

If your records aren’t processed and approved by the deadline, McKinley places a hold on your student account. The hold blocks adding or dropping classes in the current semester and prevents registration for future semesters until you’re compliant. You’ll get an email warning before the hold posts.³

Clearing the hold means uploading the missing documentation and waiting through the same four-week processing window. There is no expedited lane. Students who discover the hold the week before next-semester registration often lose their preferred course sections.

Religious and Medical Exemptions

For a religious exemption, submit the Petition for Exemption form (available as a PDF on McKinley’s site) to the Dean of Students at helpdean@illinois.edu. The Illinois Administrative Code does not require proof from a religious authority; the petition rests on your sincerely held beliefs.⁶

For a medical exemption, your healthcare provider must supply a signed, dated statement identifying which vaccines are contraindicated, the medical condition behind the exemption, and how long it applies. Upload that statement through MyMcKinley alongside the immunization form.³

Extra Step for International Students

International students have to complete tuberculosis screening in addition to the immunization form. The screening is an electronic questionnaire plus, for students who have lived in countries the university considers high-risk, a QuantiFERON TB Gold or T-Spot blood test. A prior negative QuantiFERON or T-Spot can be uploaded instead of retesting if it was done within six months of enrollment (outside the U.S.) or within one year (domestically).⁷

Sequence matters if you need both TB screening and a vaccination at McKinley: do the TB screening first, then vaccinate. Vaccinating first forces a four-week wait before TB screening. Skipping the screening triggers the same registration hold as skipping the immunization form.⁷
