How to Fill Out and Submit the University of Alabama Immunization Form

To satisfy the University of Alabama immunization form requirement, download the form from myBama, take it to a healthcare provider who completes and signs it, and upload the signed form along with a self-completed Tuberculosis Risk Assessment through the same myBama portal. Both documents are required for every incoming student, and until they clear, a registration hold blocks you from adding, dropping, or signing up for classes.1The University of Alabama. The requirement applies to freshmen, transfers, and graduate students alike. Fully online distance learners are the only group exempt from submitting immunization proof.2The University of Alabama. Immunization

What the Form Has to Show

Your provider needs to document four vaccinations on the form:2The University of Alabama. Immunization

  • Measles, Mumps, and Rubella (MMR): two doses, the first on or after your first birthday and the second at least 28 days later. Lab proof of immunity is accepted in place of vaccination records.
  • Varicella (chickenpox): two doses, or adequate proof of immunity.
  • Tdap (tetanus, diphtheria, pertussis): one dose within the last ten years.
  • Meningococcal ACWY: one dose received after your 16th birthday. This applies to all entering students under 21 and to any student living in campus housing regardless of age.

The meningococcal dose is where most rejections start. A shot given at the standard pediatric age of 11 or 12 does not count. If your only meningococcal vaccination happened before your 16th birthday, schedule a new dose before you send the form in.2The University of Alabama. Immunization

If you have an official state immunization certificate that already lists all four required vaccinations with dates, you can submit it in place of the university’s form, provided it contains the necessary information. The TB Form That Goes With It

Every incoming student also has to complete the Tuberculosis Risk Assessment Form. It is a self-signed questionnaire and does not need a provider’s signature. If your answers indicate elevated risk, UA will require a TB test performed in the United States before it clears you, and a positive result triggers additional follow-up.2The University of Alabama. Getting the Form and Filling It Out

Both forms live inside the myBama portal, not on a public download page. Log in at mybama.ua.edu, open the Student tab, find the Student Health Center and Pharmacy section, and click the Upload Immunizations icon. From that screen you can print both forms.2The University of Alabama. Immunization

Take the immunization form to your healthcare provider. The provider fills in each vaccination date, signs the form, and places their official office stamp next to the signature. Complete and sign the TB screening form yourself.

Write your Campus Wide Identification number (CWID) clearly at the top of every page. Without a legible CWID, staff cannot match your documents to your student record, and the submission comes back rejected.

Uploading Both Forms

Scan the completed forms or photograph them. PDF is the safer format because it preserves signatures and dates cleanly, but readable photographs are accepted. If you use a phone camera, check that dates and the provider’s stamp are sharp enough to read at a glance.

To submit, log back in to myBama, open the Student tab, and return to the Upload Immunizations section. Select your files and submit. Include both the immunization form and the TB screening form in the same upload.2The University of Alabama. Immunization

Processing Time and Clearing the Hold

The Student Health Center typically processes uploaded documents within 48 to 72 hours. Once your file checks out, you get a confirmation email and the registration hold drops from your account.2The University of Alabama. Immunization

If something is wrong, an illegible scan, a missing signature, a vaccination that does not meet the age requirement, you will receive an email spelling out the reason. Fix the specific problem, re-upload, and the 48-to-72-hour clock starts again. In the weeks right before a semester begins, build in time for a second round in case your first submission has an issue.

For questions about a rejection or your compliance status, email immunizations@ua.edu or call (205) 348-6262 and press 0 for the receptionist. Put your CWID in any email so staff can pull your record immediately.2The University of Alabama. Immunization

Medical and Religious Exemptions

UA allows two grounds for exemption from the vaccination requirements: a documented medical contraindication, or a sincerely held religious belief. Exemptions do not apply to the TB screening. Every student completes the TB Risk Assessment Form regardless of vaccination status.1The University of Alabama. Health Requirement for Registration Policy

For a medical exemption, your healthcare provider completes a Medical Exemption Form with documentation supporting the medical reason. For a religious exemption, you sign a Religious Exemption Form affirming your sincerely held religious beliefs (a parent or guardian signs if you are a minor). Both forms are available through myBama and submitted to the Student Health Center and Pharmacy. The SHCP Medical Director or a designee reviews every waiver request, and that decision is final.

Two limits apply even after an exemption is approved: the university can require you to leave campus during an outbreak of a disease you are not vaccinated against, and certain programs or activities may restrict participation to fully vaccinated students.1The University of Alabama. If You Do Not Submit

Students who do not submit the immunization and TB screening forms before classes begin have a registration hold placed on their account. The hold blocks you from dropping, adding, or registering for future classes until the requirements are satisfied.1The University of Alabama.
