How to Fill Out and Submit the Utah Registration Information Change Form

To change the name or address on a Utah vehicle registration, use the state’s Motor Vehicle Portal at mvp.tax.utah.gov for an address update, or bring proof of a legal name change to a DMV office or mail it to the Motor Vehicle Division. There is no single Utah vehicle registration change form you fill out online; the portal walks you through an address change using your vehicle information, and name changes are handled through document review rather than a self-service form. Utah Code § 41-1a-218 requires you to notify the Division of Motor Vehicles when you move from the address on your registration.1Utah Legislature. Utah Code 41-1a-218 – Notice of Change of Address

Change Your Name on a Registration

A name change can’t be processed through the online portal. You’ll need to present proof of your new legal name at a DMV office or send it with a mailed request. Acceptable documents include:

  • A certified marriage certificate from the state where the marriage was recorded
  • A divorce decree establishing your restored or new name
  • A court order authorizing a legal name change
  • An updated passport or new birth certificate reflecting the new name
  • Adoption papers, if the change results from a legal adoption

These are the same categories the Utah Driver License Division accepts for a license name change, and the Motor Vehicle Division follows a similar approach for registration records.6Utah Driver License Division. Utah charges $4.00 for a replacement standard vehicle or watercraft registration certificate, and $3.00 for an off-highway vehicle registration.7Utah Division of Motor Vehicles. You aren’t required to carry a physical card. Under Utah Code § 41-1a-214, owners are “encouraged” to carry the registration card for the convenience of peace officers but won’t be ticketed for not having it, and a photo on your phone works if you do choose to carry it.8Utah Legislature. Tell Your Insurance Company Too

Updating the DMV does not notify your auto insurer. Insurers use your garaging address, meaning the location where the vehicle is parked most nights, to calculate your premium, and a mismatch between your registration and policy can lead to a denied claim or a cancellation. Contact your insurance company as soon as you move, ideally before or at the same time you update your registration.
